What an important question! Often clinical skills especially in the hospital setting are taught without attention to counseling skills which are just as important for a patient's overall experience. Practitioners need to be aware that though they do these procedures on a daily basis, patients do not and need procedures to be explained and emotions to be affirmed as well. Family Centered Care does a great job of this. Treating families as complete humans instead of treating specific body parts.

has suggested a number of journals that will publish protocols but as far as I am aware most (if not all) will still charge a fee although they may offer discount on publications of the full paper.
One option might be to register your review (Prospero is free I think: https://www.crd.york.ac.uk/prospero/), which you should do anyway, including relevant details from the protocol and upload the protocol itself on a pre-print server (or another site) that can provide you with a doi (e.g. https://www.medrxiv.org/).

Progression of Childbirth events are categorized into three stages. Stage 1indicates the full dilatation of uterine cervix. Stage 2 indicates the events of vaginal delivery and stage 3 the delivery of placenta and membranes. Subsequently the mother is monitored for two hours in the labour suite. This is equally an essential component of maternal wellbeing. This phase should be categorized as part of childbirth process. I value your opinion.

This is the reason. Many maternal deaths or disability occur in this period because of poor care. I think we must strongly advocate inclusion of fourth stage in the labour process to ensure safe motherhood

I would like to review the literature on the intrinsic and extrinsic risks to women that can negatively influence their childbirth experience.
I have a difficulty in developing my research protocol to describe the variables of interest because they include environmental, social, psychological and other factors...
In the JBI manual for evidence syntheses, it is written: "It should be clearly reported in this section what the exposure or risk factor (or protective factor) is, and how it may be measured/identified including the nature of the exposure and its intensity and/or the duration of exposure, if relevant". This assumes that we already know the exposure factors. In my case, I'm trying to know them. How do I proceed?

In South Africa, I would highly recommend midwifery-led units which are close by to hospitals in case of an emergency. The midwives in South Africa are trained well and highly skilled to recognise if there is a need for referral. Most of the deliveries even in hospital are done by midwives.

Before I retired from work, I used to see ladies with continence problems and I used to provide conservative treatment. Very often, this was successful, but sometimes, those – particularly with post-childbirth stress incontinence – needed further intervention.
This was sometimes in the form of tension-free tapes, and the results were good.

In my opinion each one has its own advantadges and disadvantadges. TVT's Pros are their better long-time outcome and Cons their heigher rate of blader perforations. TOT are easier to perform but have more pelvic postoperative pain.

Some medical professionals in our country believe that if amniotic sac sac stays intact during giving birth, it needs to be artificially broken at least at cca 8 cm of cervical opening during the first stage of labor. They believe pushing with intact membranes can otherwise CAUSE placental abruption, which is dangerous for both mother and a child. What is the origin of this claim ? Is there any evidence supporting or refuting it ?
(I have seen a Cochrane review, which does NOT recommend a routine amniotomy during first stage of labor. Frustratingly, though, in that article, amniotomy is framed only as a measure to speed up labor. Placental abruption is not among the outcomes. )

Amniotomy may hasten Labour progression by allowing fetal axis pressure to come into play early and hasten the release of prostaglandins , all which may impact positively on Labour progression but not necessarily Labour outcome has been well documented.
Similarly amniotomy is a well documented integral and cardinal component of management of placental abduction but not it's prevention. Of crucial significance is the fact amniotomy should be performed in a controlled fashion in order to prevent sudden decompression of intrauterine pressure particularly in cases of uterine overdistension due to e.g. polyhydramnios which may then predispose to placental abduction.

The present scales measure negative aspects of pregnancy or childbirth such as fear of childbirth, the anxiety of pregnancy, and worry in pregnancy. I wonder whether there is a scale in English which measure opposite aspects such as the pleasure of pregnancy and childbirth.

I know the Mackey Childbirth Satisfaction Rating Scale to measure women's satisfaction with labor and delivery. In my country, there are some studies that have used this scale. Chronbach's alpha coefficient reported for the Spanish version is 0.94 for the total scale, ranging from 0.72 to 0.96 for the subscales.Esta scale can be useful in measuring the satisfaction of women with the experience of the childbirth. Technically, the answer to your question is "Yes".
However, there is a more focused question that you should be asking:
"If I use expenditure on childbirth as a proxy for the cost of healthcare, will I learn anything useful about the economics of the overall healthcare system?"
The answer to that second question is "No".
Expenditure on a particular healthcare service is strongly influenced by the availability and cost of staff and resources. If operating theatres and obstetric surgeons are freely available, then the rate (and cost) of caesarian sections will be high; if access to theatres and surgeons is limited, the rate will be much lower. And as Judy has pointed out above, childbirth without medical intervention is always an option.
You also need to be quite clear about whether you are attempting to evaluate "the cost of healthcare" (as stated in your headline question) or "the cost efficiency of health sector" (as mentioned in the explanation that follows).

The focus of the study is how can the physical environment of childbirth influence restoration of  women? I am looking for how best to collect data on the experiences of women who give birth in the hospital obstetric units. Literature suggests that for a well woman who had a normal delivery, the duration of discharge from hospital is between 24-48hours sometimes, some women could be discharged less than 24 hours. Given this situation, how best do you think I can collect the data and in addition, how can one possibly avoid the experience of childbirth from interfering with that of recovery/restoration?

This sounds like a useful and informative study - assuming the methods you select are feasible for your circumstances. My first thought, when I read the word restorative, is the work of Kaplan and Kaplan who study attention restoration and developed a comprehensive theory ART (attention restoration theory) - though this is not what you are describing - I thought I'd point out the overlap. Meanwhile, I am one of the colleagues Dr Newnham referred to above. I have multiple publications regarding the physical birth unit design's influence on women's supporters' experiences. It is a 'wicked' problem - better understanding the space in which such a number of different experiences occur (caregivers, the woman, the supporters). There isn't necessarily one best method - but making sure that you are connecting your well-developed research question very clearly with the population of interest and the existing literature and the possible outcomes is important.

The Childbirth Attitudes Questionnaire was validated in Farsi. it is recommended to use cut off value=28 for identifying fear of childbirth. I could not find any references indicating so. It is consisted of 14 items. Each item is scored 1–4 (1=not at all, 4=very much) with total points of 14–56.

From the findings of my research women are talking to each other about severe morbidities after traumatic vaginal deliveries and becoming 'fearful' of lifestyle consequences like sexual dysfunction, urinary and fecal incontinence, pelvic organ prolapse. This appears to be the more pertinent reason they are requesting caesarean sections. Women who sustained injuries like levator ani avulsion or OASI noted that they would have preferred  information and informed consent prior to deliveries that resulted in unexpected and unexplained long term  physical and psychological repercussions. Whilst vaginal deliveries can have positive outcomes for some women it is integral that clinicians be aware others suffer severe damage and up until recently have not had a voice and been dismissed as having 'women's problems that will get better'. Yes the paradigm has changed but not due to the fact women are less resilient.

I've heard many stories from women about how pain meds prescribed following childbirth caused a relapse or led to opiate addiction.  I couldn't find any research that looked at prevalence or risk factors.

I am wondering if the intrauterine pressure increased by uterine contractions exerted anywhere in an amniotic cavity if there is no rupture of the membranes. Any publications mentioned  Pascal's law in an amniotic cavity?

The uterine cavity with intact membranes and closed cervix is a closed container and have no choice but to obey Palcal's law. Considering that contractions of normal labor start at the cornua region of the uterine fundus and then spread towards the cervix, every contraction exerts pressure in the amniotic sac/uterine wall structures in one direction and receives counter pressure from the rest of the container walls. If the cervix dilates and the amniotic sac looses its protection, then the amniotic sac will prolapse into the cervical canal and eventually will rupture. During this transitional period, the exposed amniotic sac obeys Pascal's law only partly and once membranes rupture, the uterus becomes an open container and does not obey Pascal's law anymore. The development of funneling in patients with in patients with premature cervical shortening is part of this function relationship.

In the third cesarean section, should uterine incision be performed above the former uterine incision or under the former uterine incision or in situ of the former uterine incision?

In my hospital we only allow 3 CS for any one person but very rarely do we encounter a 4th CS. I recommend above the scar because of the space available for extension of the incision and I personal tear up and down on a horizontal incision (publication attached ) so the extended tear usually extend upwards on either side, like a "smile". this is because of the alignment of the muscle fibers. since we only allow 3 CS, incision on the upper segment is not a issue because Bilateral Tubal ligation is done with the patients approval.

I think whatever position you feel most comfortable. Personally I usually stand but as long as you have a solid base which does not move I honestly don't think it matters. I stand so that the patient sees me and I can talk to them whilst performing the delivery but we also use a wedge so the patient is propped up a little and not lying flat.

This depends on the viability of the detached part of the cervix. If it is viable reconstruction of the cervix can be attempted. If the tissues are not viable excision and re epithelialisation of the raw surface should be done.
