'Participatory' action research - when is it, when is it not?

Quite often, I see action research studies that say they are 'participatory' when they don't appear to be and, moreso, action research studies that clearly are participatory - but do not claim to be. Is it as simple as 'participation' in action research makes it 'participatory' - or is it more complex than that? For instance, based on social critical research theory - does organisational and individual emancipation and empowerment need to be in place for it to be 'truely' participatory?

What are the effects/ the impact of user-centered methods and user integration methods?

There are some often mentioned benefits associated with user-centered or user integration methods and approaches (e.g. participatory design, co-design, empathic design), such as better product usability, lower risk of market failure, better company image or a larger market share. To name just a few. These effects are sometimes not measurable at all, not quantitatively or qualitatively, and in some cases they are not even recorded.
My questions regarding this topic are:
1. What effects/ benefits or impact of user-centered design and user integration methods do you know? Maybe from your own experience.
2. Are there publications where effects have been measured quantitatively or qualitatively?
3. What are the downsides? Probably higher cost; but is this always the case or does it depend on other conditions?

How to notate a multiphase mixed methods study?

I carried out a multiphase mixed methods study for my doctorate to develop a specific EAP (English for Academic Purposes) course. It was action participatory research and the different phases were (i) a pilot (ii) a needs analysis (iii) course development (iv) course evaluation (v) reflection.
So the different phases led to each other. I have 2 questions:
(a) Do I show the notation for each phase separately? If no, how do I indicate the progression from the one phase to the other? Some phases were depending on the previous one, while others took place concurrently.
(b) The questionnaires collected qualitative as well as quantitative data. The qualitative data was collected by open ended questions and also by providing a space after some questions for elaboration. Are those examples of embedded QUAN(qual) or explanatory  QUAN-qual? Any input would help,
