BACKGROUND: The use of the implantation method in dental practice is constantly growing. The indications for its implementation are expanding, including in cases of significant atrophy of the alveolar process in order to restore bone volume using osteoplastic materials. The study of metabolic processes remains a pressing issue. AIM: To evaluate reparative osteogenesis in the augmentation zone with allogeneic lyophilized cancellous bone tissue by analyzing metabolic criteria. MATERIALS AND METHODS: The study included 41 people in the main group and 17 in the control group at the stage of osseointegration with an observation period of 1 to 12 weeks, the object of the study was oral fluid. Allogeneic lyophilized spongy bone tissue was used in the augmentation zone. RESULTS: At 1–2 weeks, no statistically significant differences from the control values were found. However, at 3–4 weeks, a slight increase in collagen C-telopeptide, osteocalcin, and a decrease in the activity of alkaline phosphatase and parathyroid hormone were noted. These changes indicate the development of the secondary remodeling stage by forming an osteoid matrix. After 12 weeks of observation, metabolic parameters are restored, indicating reparative osteogenesis. CONCLUSION: The use of lyophilized spongy bone tissue ensures physiological osteogenesis.


Clinical experience of tooth replantation

Relevance. Modern dental practice strives, above all, to preserve teeth. A variety of operations aimed at this include dental replantation. Replantation is used in cases where there are difficulties with the root canals, such as their pathological bending or previous treatment with the resorcinol-formalin method, and also when root resection is impossible due to the risk of damage to surrounding structures, for example, the maxillary sinus or the inferior alveolar nerve. The purpose of this study was to study the osseointegration of a replanted tooth subjected to coronoradicular separation. Materials and methods. Thirty-one-year-old patient K. came to the dental clinic with the problem of pain in the projection of tooth 4.6, which occurs during chewing. A detailed examination revealed that the crown of tooth 4.6 had previously undergone restoration using a filling material that had defects. It was decided that tooth 4.6 should be removed and subsequently replanted. The tooth was extracted according to established procedures, while maximally preserving the surrounding periodontal and periosteal tissues. To preserve periodontal tissues, alveolar curettage was carefully performed. During the extraction of the tooth, it was longitudinally divided into two parts – the medial and distal roots. After this, the root canals were treated using extraoral therapy. The roots of tooth 4.6, treated with an antiseptic, were returned to their anatomical positions in the alveolus. The final stage was the creation of a control x-ray to verify the correctness of the replantation. Conclusions. After two years of observation of tooth 4.6, which was subjected to replantation, no pathological changes in root stability were detected. It was reported that the soft tissue surrounding the roots of the tooth formed a structure similar to the periodontal sulcus, which facilitated the use of retraction cord during tooth preparation.


Comparative analysis of oral fluid metabolism parameters patients with complicated and uncomplicated implantation

Relevance. Modern scientific research continues to develop promising areas for diagnosing inflammatory complications after dental implantation. Analysis of studies shows that inflammatory complications depend on many factors, the establishment of which can be challenging. In this regard, saliva diagnostics opens up broad prospects for studying the pathogenetic basis of the development of inflammatory processes. Key words: implantation, metabolic status, oral fluid, inflammatory process. Purpose: to study in a comparative aspect the metabolic status of complicated and uncomplicated implantation. Materials and methods of research. 88 patients were examined, of which 30 with peri-implant mucositis (main group I), 28 with uncomplicated implantation (comparison group II) and 30 control group, age 45.3±2.4 years, follow-up period 1-3 years. Results. Based on the results of the studies and comparative analysis of complicated and uncomplicated implantation, previously unknown data on their distinctive metabolic characteristics were obtained and presented. With complicated implantation, these include a characteristic increase in C-reactive protein, iron, uric acid and a decrease in calcium levels. Conclusions. A set of new information has been obtained that reveals prognostically significant factors of gastric cancer metabolism during complicated and uncomplicated implantation. The identified changes in metabolism during implantation must be taken into account and differentiated from early metabolic signs of inflammation. The study of gastric cancer is objective and promising in the diagnosis of the inflammatory process and its prognosis.


ROLE OF THE BIOLOGICAL ENVIRONMENT OF THE ORAL CAVITY IN DIFFERENTIAL DIAGNOSIS OF CHRONIC PERIODONTITIS AND DENTAL PERI-IMPLANTITIS

The relevance of the study is caused by the need to search for metabolic criteria for inflammatory destruction of bone tissue. Purpose of the study: differential diagnosis of chronic periodontitis and dental peri-implantitis according to metabolic criteria of the biological environment of the body (oral fluid). The study included patients who were divided into 3 groups: 35 people with chronic periodontitis (group 1); 30 – with dental peri-implantitis (group 2); 20 – control group. The authors studied the protein and mineral metabolism of oral fluid. The authors conducted a study of the biological environment and found that the concentrations of total protein in all groups were not statistically different. However, total albumin concentration was lower in patients with dental peri-implantitis compared to controls and chronic periodontitis (p < 0.001). The values of the effective concentration of albumin and the toxicity index in the first and second groups are significantly lower compared to the control (p < 0.001), and the albumin binding reserve in the group of dental peri-implantitis is reduced more significantly (p = 0.003). The authors found statistically significant high concentrations of calcium and magnesium in groups I and II in comparison with the control, and when comparing the groups with each other, p < 0.826. Thus, disturbances in protein and mineral metabolism during dental peri-implantitis are statistically more significant against the background of periodontal diseases, which must be taken into account when performing dental implantation.


ANALOG AND DIGITAL PROTOCOLS FOR EVALUATING SELECTIVE GRINDING OF DENTITION WITH OCCLUSIVE DISORDERS BEFORE DENTAL IMPLANTATION

For successful orthopedic treatment using dental implants, an objective diagnosis of pathological changes in teeth and periodontal tissues is necessary. The identification and optimization of occlusive disorders at various stages of preparation for dental surgery contributes to the normalization of osseointegration processes, expands indications for periodontal diseases. The aim of the study was to assess the normalization of occlusive disorders before dental implantation using analog and digital protocols. The study included 57 patients aged 30 to 65 years and divided into 3 groups: the first group –23 patients with chronic generalized periodontitis of mild severity; the second group–20 patients with nonphysiological location of individual teeth due to partial defects of the dentition; the control group–14 patients. Results. The assessment of occlusive disorders was carried out using occlusography methods and in the articulator "Stratos -300". In the first group–62.6% of supracontacts, in the second group – 49.3%. Occlusive correction of premature contacts was performed. The control was carried out by electronic axiography. Occlusal harmony of occlusal-articulatory relations has been achieved after selective polishing. A comfortable closing of the teeth and smooth movement of the lower jaw have been achieved. The qualitative and quantitative indicators of the axiogram have improved.


PHYSICAL-CHEMICAL AND METABOLIC PROCESSES IN ORAL CAVITY: THEIR INTERACTION IN DENTAL PERI-IMPLANTITIS AND CHRONIC PERIODONTITIS

Intraosseous dental implantation is considered to be the most effective and technologically advanced method employed to restore individual and multiple dentition issues. It offers potential for significant improvement of comprehensive rehabilitation in dental patients. However, positive outcomes of the implantation that enable restoration of the dentition integrity, complete compensation of the patient’s chewing function and aesthetic looks, does not rule out complications involving inflammation. It still remains relatively high, within the range of 8 to 15%. Peri-implantitis, if taken as inflammation affecting the soft tissues around the implant, bears signs of bone destruction and accounts for one of the most common complication. The comprehensive study described here was aimed at evaluating the dental status, as well as the physical-chemical and metabolic parameters of the oral fluid in patients with dental peri-implantitis (n=31), chronic localized periodontitis (n=30) and chronic generalized periodontitis (n=30). The control group included patients (n=30) with no signs of dental pathology, all comparable in terms of their age and gender. In patients with dental peri-implantitis, chronic localized periodontitis and chronic generalized periodontitis, we revealed unidirectional dynamics of the oral fluid parameters (electrical conductivity, redox potential, concentration of calcium, magnesium, ammonium cation), the values being different, though. The identified features of the metabolic status of oral fluid will increase our understanding of pathogenetic mechanisms underlying peri-implantitis.


MATHEMATICAL ANALYSIS BASED STUDY OF DENTAL IMPLANT BIOMECHANICS WITH OCCLUSAL LOAD ON BONETISSUE

The article contains theoretical studies aimed at identifying the load and detecting the factors that have a negative impact on implants. One of the reasons behind implant disintegration is bone tissue resorption, which is due to overload. Direct measurements of the oral cavity stresses affecting the tissues around the implants are basically impossible. The purpose here is to choose the optimal inclination of the implant in relation to the vertical axis in view of mismatch of the mandibular and maxillary alveolar arches. The study here employed the finite element method (FEM) relying on the universal FEM ANSYS package. Three patterns have been identified. First, the dependence of the destructive force on the bone tissue on the implant inclination angle (up to 5° — it is twice as low compared to the initial value, and above 20° — it becomes destructive). Second, the implant deviation in the transversal plane results in a greater decrease in strength, compared to its deviation in the sagittal plane. Third, the greatest destruction resulting from the stress-strain bone tissue status is observed in the cortical, and not in the spongy bone. The study allows proper selection of implants, their installation options, as well as scientifically reliable forecast of the respective long-term outcomes.


ASSESSMENT OF REGULATORY AND REPARATIVE OSTEOGENESISIN AUGMENTATION AREA BASED ON ORAL FLUID METABOLIC PARAME

Cases of serious atrophy of the alveolar processes occurring during dental implantation could be restored saving the respective bone volume. There is also a need for postoperative monitoring of the physiological osteogenesis effectiveness in the peri-implant zone. Our paper offers an assessment of regulatory and reparative osteogenesis in the augmentation zone subject to the oral fluid (OF) metabolic parameters obtained from 59 patients. Biochemical methods for oral fluid examination are noninvasive approaches that allow identifying pathologies at the tissue and cellular level. The proposed metabolic indices of collagen C-telopeptide, osteocalcin, alkaline phosphatase (AP) and parathyroid hormone helped identify augmentation zone of disturbed reparation in 6.8% of patients, whereas the worst complications were to be observed one month after the implantation. Through this period, however, orthopantogram featured no pathological change. Given that, the biochemical markers of bone tissue remodeling are employed for early identification and developing a forecast regarding treatment effectiveness.


IMPLANT TREATMENT IN PATIENTS WITH SEVERE TOOTH WEAR ON THE OCCLUSAL SURFACE

Examination along with implant treatment was carried out in 21 patients with severe tooth wear on the occlusal surface. This disorder features a variety of clinical symptoms including changes in the facial features and the lower jaw movement amplitude, disturbed canine and incisor guidance, supercontacts, asynchronous operation of the masticatory muscles, etc. We have proposed a treatment algorithm aimed at eliminating these issues. The temporary non-removable dentures allowed creating occlusal marks (Stage 1) followed with their transfer onto permanent orthopedic structure (Stage 2) using implants. The complexity of the approach to treating this disease, as well as the effectiveness of our method has been confirmed by clinical data and the outcomes obtained through an instrumental study involving electromyography and electronic axiography. The extra additional research methods employed, such as electromyography and electronic axiography, allowed to assess the activity of masticatory muscles and the lower jaw articulation, thus to ensure occlusal-articulation interaction when restoring the lower facial height.


Analysis of osseous tissue temperature during preparation of recipient bed for cylindrical implants

In the early 21st century, robot-assisted dental implant surgery became a popular alternative to classic implant placement protocols. Postoperative complications are often provoked by poor compliance with surgical guidelines for implant placement, resulting in the overheating of the osteotomy site. The aim of this study was to measure the temperature of osseous tissue at the dental implant site during classic and robot-assisted dental implant placement performed at different cooling modes. Avital skeletonized mandibles of Vietnamese pot-bellied pigs were used as an experimental model. The recipient bed was prepared following the classic surgical protocol. Three cooling modes were tested: no irrigation, irrigation with sterile saline at 25–30 ml/min and standard 75 ml/min irrigation recommended by the standard surgical protocol. The temperature of the isotonic solution was 25 °С. The study showed that both classic and robot-assisted dental implant placement techniques are safe if there is sufficient irrigation and good compliance with the surgical protocol.
