We aimed to investigate the clinical results following poor-quality embryo transfer and the parameters to foresee the prognosis. In this study, 2123 cycles that had day 3 and day 5 single-fresh embryo with poor-quality embryo transfers and good-quality embryo transfers were compared. The cycles according to transfer day were evaluated by conducting a subgroup analysis. The correlation between all the obtained demographic characteristics, controlled ovarian stimulation parameters, and cycle results were analysed. Clinical pregnancy was established in 53 patients that underwent transfer in the poor-quality embryo group (14.9%). Of these patients, 36 had live birth (live birth rate per clinical pregnancy 67.9%). In cleavage-stage embryos, live birth rates per clinical pregnancy were higher in poor-quality blastocyst transfer. When analysing the factors affecting live births in the poor-quality embryo group, as the total gonadotropin dose increases, the probability of live birth decreases, as in the probability of hCG positivity. In conclusion, although the probability of pregnancy is low, when clinical pregnancy is established, there is a high chance of having a live birth after poor-quality embryo transfers. This could be regarded as an acceptable option in cycles when only poor-quality embryos are available.

Clinical Importance of Serum BDNF (Brain-Derived Neurotrophic Factor) Level for the Management of Pregnancies Complicated With Meconium-Stained Amniotic Fluid

Objective The aim of this study was to investigate the association between poor neonatal outcomes and BDNF (brain-derived neurotrophic factor) levels. We aimed to predict the need for an emergency cesarean and prevent unnecessary interventions in cases complicated with meconium-stained amniotic fluid (MSAF). Methods This study was designed as a case-control study including three groups. Group A included pregnant women who underwent cesarean due to fetal distress. Group B included the women who delivered vaginally. Groups A and B had cases with the presence of meconium in the amniotic fluid. Group C as a control group had clear amniotic fluid. Demographic features, fetal outcomes, and maternal serum and fetal cord blood BDNF levels (Human BDNF ELISA Kit; Synonyms: ANON2, BULN2; Catalog no: E-EL-H0010 96T) were evaluated. Results No significant difference was found between patients with meconium and without meconium in terms of BDNF levels. However, the BDNF level was found to be significantly lower if fetal distress had occurred with MSAF. Conclusions In conclusion, the study demonstrated that the level of maternal and fetal cord blood BDNF are both significantly lower when fetal distress occurs with the presence of MSAF.


Association between masked hypertension and late-onset fetal growth restriction at advanced maternal age

Fetal growth restriction (FGR) is a major risk factor for complicated pregnancy including stillbirth. The all-pathophysiology results in uteroplacental insufficiency. Hypertension chronic or pregnancy-related complicates the all-prenatal periods. The relationship between fetal growth restriction and hypertension is well-known and this risk increases with maternal age. Late-onset FGR is diagnosed >32 weeks. Masked hypertension (MH) is a phenotype of hypertension when clinic blood pressure is in the normal range in the office but elevated outside the office. This study examined the risk of late-onset FGR and masked hypertension at advanced maternal age. Sixty pregnant women over 40 years old were included in the study who delivered at our hospital with late-onset fetal growth restriction diagnosis. After taking detailed anamnesis from all individuals included in the study, detailed physical examination, obstetric examination and ambulatory blood pressure monitoring (ABPM) were performed. Women with maternal chronic hypertension, pregestational or gestational diabetes mellitus, chronic kidney disease, rheumatoid disease history, tobacco use, cardiac arrhythmia, fetal genetic abnormality or placental abnormality were excluded from the study. The prevalence of masked hypertension was statistically significantly higher in the group with late intrauterine growth. (n:13, 43% vs n:5, 16.7% p=0.02). Office systolic BP (125.5±4.1 mmHg vs 118.5±5.7 mmHg p=0.016), 24-hour systolic BP measurement (125±9 mmHg vs 119.7±5.7 mmHg p=0.03), and ambulatory nocturnal diastolic BP measurement (66.7±10 mmHg vs. 61.±5.5 mmHg p=0.03) was higher in the group with late FGR. Hypertension can lead to complications in the perinatal period. There is a bad association between FGR and hypertension. Masked hypertension should be considered especially in advanced age pregnant individuals with FGR.

Introduction: Gynecological pathologies are an important cause of anemia in women. In this study, we aimed to evaluate women who had were hospitalized because of anemia (Hb level< 10 g/dl) caused by gynecologic pathologies and treated with either intravenous iron (ferric carboxymaltose) or blood transfusion. Material -method: This retrospective cross-sectional study was performed in a tertiary care center. Women who were hospitalized with the diagnosis of anemia with Hb level<10 g/dl and abnormal uterine bleeding between March 2015- September 2017 in the gynecology clinic were enrolled in the study. Hemoglobin levels, hemoglobin changes, uterine pathology and treatment of patients were recorded and compared. Results: One hundred and fifteen women received red blood cell transfusion and 100 women were treated with intravenous ferric carboxymaltose. The mean age of the women was 45.1 ± 6.1 (22-57) years. Although the mean Hb levels were higher in the iv iron replacement group at the end of the one month (p=0.001), the mean increase in Hb levels was similar between two treatment modalities (p=0.101). Among the anemic women who required surgery, iv iron replacement was the first choice in 75.9% of women; 34.1% received red blood cell transfusion in the preoperative period. Conclusions: Gynecological pathologies are a common cause of anemia in reproductive age women and intravenous carboxymaltose treatment is a safe and cheaper alternative of blood-transfusion in appropriate cases to elevate the Hb levels in the preoperative period.

Over kanseri jinekolojik kanserler arasında kansere bağlı ölümlerin önde gelen nedenidir. Kanser hastalarında özellikle ileri evrede, venöz tromboembolizm yokken bile hemostatik aktivasyona bağlı hiperkoagülasyon ve artmış fibrinoliz sıklıkla gözlenir. Bu çalışma ile yüksek dereceli seröz karsinom olgularında operasyon öncesi ölçülen plazma d-dimer düzeyinin klinik önemi ve prognostik değerini incelemeyi amaçladık. Retrosptektif olarak yapılan çalışmamızda Dr. Zekai Tahir Burak Eğitim ve Araştırma Hastanesi Jinekolojik Onkoloji Kliniği’nde 2013-2017 tarihleri arasında primer yüksek dereceli seröz over kanseri (HGSK) tanısı alan ve operasyondan önceki 15 gün içinde D-dimer testi yapılan 142 hasta dahil edildi. Tanı anındaki yaş, tedavi öncesi plazma D-dimer düzeyi, asit varlığı, rezidüel tümör, CA125 seviyesi, FIGO evresi ve demografik bilgileri karşılaştırıldı. Ortalama yaşları 55.25 ±10.84 olan hastaların 52’ sinde (%36,6) normal, 90’ ında(%63,4) D-dimer seviyeleri yüksek olarak saptandı. Aim In our study, the frequency of sleep disturbances in pregnancy, the reasons underlying the low quality of sleep, clinical factors, and the effects on quality of life were investigated. Methods The study was planned as a prospective study and included 189 pregnant women. Clinical features, laboratory results, socio-demographic status, obstetric and medical anamnesis were evaluated. The Pittsburgh Sleep Quality Index, Epworth Sleepiness Scale, the Berlin Questionnaire, Beck Depression Inventory, SF-36 quality-of-life questionnaire, and restless leg syndrome (RLS) diagnosis criteria were used for data collection. Results We investigated sleep disorders in 110 pregnant women (58.2%). A history of premenstrual syndrome and patients with hyperemesis gravidarum, obstructive sleep apnea syndrome, Vitamin B12 deficiency, and higher TSH levels in the laboratory were found to be associated with sleep disorders. Sleep disorders and daytime sleepiness were associated with depression, physical and social functioning, pain, and disturbance in general health perception. The number of pregnant women who had two or more RLS complaints was 84 (44%). Conclusion The results of our study show that sleep disorders in pregnancy can be managed with easily applicable methods according to the risk factors and related problems.


Ultrasonographic assessment of lower uterine segment thickness and perinatal Outcomes in patients with a previous cesarean history

Objective: The purpose of the study was to establish the validity of the abdominal evaluation of lower uterine segment (LUS) thickness in patients having a previous cesarean section (CS) in predicting uterine rupture, and to evaluate if there is any effect of the number of a previous surgeries on adverse maternal and perinatal outcomes. Materials and methods: A prospective case-control study was carried out between December 2017 and June 2018 in Dr. Zekai Tahir Burak Women’s Health Education and Research Hospital. 555 patients were classified into three groups; Group 1: patients having one previous CS, Group 2: patients having two previous CS, Group 3: patients having more than the two previous CS). Ultrasonographic measurements of the LUS, intraoperative findings and, adverse pregnancy outcomes were assessed. Results: LUS thickness of group 2 was significantly less than the LUS thickness of the other groups (p=0.022). The feeling of pain was significantly more in the patients of group 2, when compared with the other groups (p=0.019). Pregnancy interval was the only parameter that had significant predictivity for adverse pregnancy outcomes for group 1(#CS=2) and group 3 (#CS≥4) (p=0.042, and p=0.021, respectively). In group 2 (#CS=3), age, the thickness of LUS, and thickness of subcutaneous adipose tissue were found to have significantly high predictivity for adverse pregnancy outcomes (p=0.012, p≤0.001, and p=0.007, respectively). Conclusion: Measurement of LUS, in the patients who had previous CS, can be used for risk assessment and management. It is a non-invasive, reliable and, easily applicable method. Standardization of the measurement technique is a necessity.
