Background: The correct diagnosis of sexually transmitted infections (STIs) is the first step in the monitoring and management of these diseases. Objectives: This study was conducted to investigate the clinical and laboratory positive predictive value (PPV) of STI symptom management in Iran. Methods: This cross-sectional study was conducted on 5986 individuals (aged 18 to 50 years) from the general population of Marovdasht (Iran) in several stages: (1) self-reported symptoms of sexually transmitted diseases, (2) evaluation by a doctor, and (3) laboratory examination. Results: Among the population sample, 686 (17.68%) individuals reported at least one of the predefined symptoms. The clinical-based PPV of syndromes in men and women were 67.74 (62.34 - 73.14) and 85.36 (82.76 - 87.96), respectively. In men and women, the highest PPV was observed for abnormal discharge from the anus 32.35 (25.35 - 39.35) and abnormal secretions from the cervix 59.39 (56.39 - 62.39), respectively. The laboratory-based PPV for men and women was 0 and 5.04 (3.04 - 7.4), respectively. Conclusions: The clinical and laboratory-based PPV of STI symptoms in the general population is extremely low. Accordingly, a syndromic-based screening or monitoring approach for STIs is not a reliable tool for screening or monitoring in the Iranian general population. We suggest focusing on special (core) groups for monitoring STIs using laboratory-based methods.

Background: Correctly estimating the prevalence of sexually transmitted infections (STIs) is crucial for monitoring and managing these diseases effectively. Objectives: This study aimed to estimate the prevalence of STI syndromes and evaluate the surveillance systems for STIs in Marvdasht. Methods: We conducted a cross-sectional study in 2018 involving a population-based random sample of 3 879 participants aged 18-50 in Marvdasht city. Genital ulcers and secretions in both genders were used as the primary outcomes to assess the completeness of the system and calculate the correction factor. A mathematical model was developed to estimate the correction factor for the prevalence of selected syndromes in each gender. The correction factor was determined by dividing the cases estimated through the mathematical model by the cases reported by the surveillance systems. Results: The estimated prevalence (with 95% confidence intervals [CI]) of urethral discharge in men and women, based on the model, was 25.32% (23.08 - 27.56) and 47.03% (39.93 - 54.13), respectively, and for genital ulcers, it was 5.16% (4.06 - 6.86) for men and 15.50% (9.5 - 21.5) for women. In both genders and for both syndromes, the reported prevalence by the care system (men: discharge 0.09% [0.07 - 0.11], ulcer 0.04% [0.03 - 0.06]; women: discharge 0.12% [0.1 - 0.15], ulcer 0.19% [0.16 - 0.22]) was severely underestimated. The highest and lowest underreporting correction ratios in the national STI monitoring system were observed for urethral discharge in men (124.5) and genital ulcers in women (7.26), respectively. Conclusions: The study revealed that the reported prevalence of STI syndromes in Iran is significantly underestimated compared to the actual prevalence in the population. Fundamental revisions are necessary to enhance the system's validity and completeness in detecting and reporting STIs.

Background: Reports indicate that probiotics have beneficial effects on both the prevention and treatment of numerous diseases. Objectives: This study aims to explore the overall inclination toward probiotic consumption, its trends, and its association with common gastrointestinal diseases. Methods: We conducted an ecological study utilizing Google Trends (GTs) and Global Concern data. We analyzed search terms related to "probiotic," "Irritable Bowel Syndrome (IBS)," "Peptic," "Constipation," "Diarrhea," and "Stomach Cancer (SC)" across all searches performed on GTs from January 2014 to July 2023. Results: Globally and in Iran, there has been an increasing interest in consuming probiotics over time. This increase is linked to a rise in cases of stomach cancer and irritable bowel syndrome, which, in turn, boosts the propensity to utilize probiotics. Globally, there is a direct correlation between probiotic consumption and IBS (R = +0.69, P = 0.0001), peptic disorders (R = +0.71, P = 0.001), Constipation (R = +0.93, P = 0.001), and diarrhea (R = +0.89, P = 0.001). In Iran, significant direct correlations were found between probiotic consumption and both constipation (R = +0.36, P = 0.001) and diarrhea (R = +0.40, P = 0.001). Among probiotic-containing products, yogurt was preferred over others worldwide and in Iran. Conclusions: The interest in probiotic use is high globally and in Iran. Consequently, there is a need for strategies to educate the public about the areas where the benefits of probiotics are well-supported by evidence and where information is either weak or lacking.

The Effect of Various Types of Exercise on Gut Microbiota: A Systematic Review

Objective: Gut microbiota composition plays a pivotal role in health and emerging evidence supports the ability of exercise training programs to alter the gut microbiota’s composition and function, which could counteract dysbiosis and the effects of chronic diseases. This study aims to investigate the effectiveness of different exercise methods on the composition of intestinal microbiota. Methods: We searched for published peer-reviewed articles in PubMed and Google Scholar databases up until January 2023. We searched using the terms "exercise", "education", "microbiome" and "microbiota". Results: By electronic search until January 2023 in the databases with keywords made in Mesh by mentioning Title/Abstract, several 1814 articles were collected. By repeating and performing filtering at each stage, 15 clinical trials were finally left in the study. The results showed varying degrees of efficacy and high inter-individual variations. In conclusion, the baseline microbiome profile was shown to have a decisive role in microbiome responsiveness to training intervention and training dose and duration seem to be a determining factor in all exercise modalities. Conclusion: In general, can be said that exercise can balance GM. More importantly, exercise is proposed to present a stressor to the gut that stimulates beneficial adaptations and improves long-term gut barrier flexibility over time through regular physical activity. It seems that the GM changes caused by aerobic exercise are reversible after returning to a sedentary lifestyle. Therefore, it is recommended that exercise initially causes a disrupts in GM that, with continued exercise and thus adaptation, revenues to the pre-exercise state.


Search Trends for Micronutrients on Google Trends in Iran for Historical Effect of the COVID-19 Pandemic: A Ecological Study

Background: The use of micronutrients to maintain health, prevent disease, and reduce disease severity is increasing. Objectives: This study aimed to evaluate the impact of the COVID-19 pandemic on the trend of public opinion toward micronutrients in Iran. Methods: An ecological study was conducted in the Google Trend (GT) database. First, users' specified search terms related to “COVID-19,” “corona (Iranian COVID-19),” “vitamin-D,” “vitamin-C,” and “Neurobion” were searched among all searches performed using GTs from January 2017 to 2023. Then, Spearman's correlation coefficient was used to check the correlation between the keywords of Micronutrients with the words COVID-19 during different years. Results: The trend for all three micronutrients (vitamin D and C and Neurobion) increased significantly since 2020, consistent with the search trend for COVID-19, corona, and the waves of COVID-19 in Iran. Vitamin D peaks were observed in 2020 (first wave of COVID-19), vitamin C in 2021 (fifth wave), and Neurobion in 2022 (seventh and eighth waves). There was a high and objective correlation coefficient between COVID-19, vitamin C (R2 = 0.73, P = 0.001), and Neurobion (R2 = 0.63, P = 0.001). However, no correlation was observed with vitamin D (R2 = 0.18, P = 0.12). Conclusions: Based on the results, the search for micronutrients in Iran increased significantly during the COVID-19 pandemic. Public interest might reflect behavior and consumption. GT is a helpful tool for epidemiologists to understand the interests and trends of the general population.

However, we did not observe a statistically significant correlation between keyword searches for COVID-19 and AS & BBS. Conclusions: The trend of AS and BBS usage in Iran is on the rise. The indiscriminate use of supplements is a cause for concern and calls for educational interventions.

Background: Today, the solutions for skin aging and rejuvenation methods are of special importance. Objectives: This study aims to investigate the desire to perform various methods related to facial rejuvenation in Iran and the world. Methods: It is an ecological study conducted using the Google Trends (GT) database. We initially searched for users' specified search terms related to "Botox," "Gel injection," "Laser," "Facelift," "Fillers," and "COVID-19" among all searches performed using GTs in the world and Iran from January 2017 to July 2023. Results: The trend of GT shows that the search volume for laser and filler services is higher than for other services worldwide. In Iran, laser and gel injections had the highest search volume until 2022. However, from 2022 onwards, fillers have become one of the top searched services. In 2020, with the onset of the COVID-19 pandemic, the significant impact of the pandemic on facial rejuvenation procedures worldwide, including in Iran, became evident. There is a significant negative correlation between the volume of searches for COVID-19 and facial rejuvenation procedures worldwide (R2 = -0.52, P = 0.0001) and in Iran (R2 = -0.84, P = 0.0001). Conclusions: Many people worldwide and in Iran are looking for facial rejuvenation. Clinicians can enter this business with a positive outlook. GT data is a powerful tool for predicting the demand for facial rejuvenation procedures.


Prevalence of nosocomial infections in Lordegan Shohada hospital from 2017 to 2022

Introduction: Healthcare-acquired infections or nosocomial infections are a common problem in all countries. Objectives: The aim of this study was to conduct an epidemiological evaluation of nosocomial infections in Lordegan Shohada hospital from 2017 to 2022. Methods: This study was descriptive-analytical cross-sectional. The case finding was based on Centers for Disease Control (CDC) definitions for nosocomial infection and the nosocomial infection questionnaire of the Ministry of Health and medical treatment of Iran. Cases of nosocomial infection were confirmed based on clinical findings and tests then registered in the Iranian nosocomial infection surveillance system. Results: Overall, 48343 patients were at risk for nosocomial infections during the study period; of these, 274 (0.6%) cases of nosocomial infections were detected, of which 132 were men (48%), and 142(52%) were women. The most common type of nosocomial infection in this study was surgical site infections (0.33%), pneumonia (0.27%), and ventilator-associated events (0.10). The highest rate of nosocomial infection was reported from intensive care units (0.2). Conclusion: The prevalence of nosocomial infections in this center has been low compared to the global norm. This is not the reason that the majority of nosocomial infections are low. The most important reason was the irrational use of antibiotics and, therefore, the hiding of infection cases. The prevalence of microbial resistance is increasing since this issue is worrying in nosocomial infection control.
