Background: The rapid morbidity and high mortality rates associated with COVID-19 have placed significant strain on healthcare systems. Mazandaran, as the third province, has been affected by this infectious pandemic. Objectives: The present study aimed to explore the experiences of healthcare providers and managers during the COVID-19 pandemic. Methods: The present qualitative research study was conducted using the content analysis approach between April 2022 and August 2022 to provide deep insights into the experiences of healthcare providers and managers during the COVID-19 pandemic. The participants included 15 hospital managers, comprising 2 clinical nurse supervisors, 3 matrons, and 10 chief executive officers. The participants were purposefully selected from hospitals affiliated with Mazandaran University of Medical Sciences, Iran. Data were collected through semi-structured individual interviews and the assessment of documents from the vice-chancellors of treatment, food and drug, development, and human resources. Results: The category of "challenges and strategies" emerged during the process of reducing and abstracting the codes. The challenges category evolved from three subcategories, including care and cure challenges, challenges related to providing physical resources and personal protective equipment (PPE), and challenges faced by healthcare providers. Similarly, the strategies category emerged from three subcategories, including strategies to improve patient care and treatment outcomes, strategies for procuring PPE and physical resources, and strategies for human resource management. Conclusions: The results of the present study revealed several challenges and strategies experienced by healthcare managers at Mazandaran University of Medical Sciences and hospital managers. Additionally, based on the existing infrastructure of Mazandaran University and the managerial skills of the managers, successful strategies were employed to tackle these challenges. These experiences can be utilized to inform revisions to health-related policies. Documenting these experiences can aid in better management of future infectious pandemics.

Background: Respiratory complications are prevalent in coronary artery bypass graft (CABG) surgery. Breathing exercises are efficient and cost-effective techniques to eliminate this problem. Objectives: This study investigated the effect of simultaneous breathing exercises on patients' respiratory condition after CABG surgery. Methods: In a randomized controlled clinical trial, 52 CABG candidates were allocated to two intervention and control groups. The control group received routine breathing exercises (incentive spirometry and vibration). Besides routine exercises, the intervention group performed exercises such as pursed-lip breathing, diaphragmatic breathing, incentive spirometry, and effective coughing for three days after the surgery. The incidence of atelectasis, oxygen saturation, lung auscultation, and the patients' satisfaction with the respiratory function was measured and recorded before and after the surgery in both groups. The collected data were analyzed in SPSS v. 22 at a significance level of < 0.05. Results: The patients' mean age was 60.9 ± 9.5 years, and 26 (51%) were male. There was no significant difference between the patients in underlying parameters. None of the patients in the two groups had atelectasis before the surgery. However, four days after the surgery, 40% of the patients in the control group and 23% of those in the intervention group suffered from atelectasis complications. The visual analog scale (VAS) score increased after surgery in the intervention group, although the difference was insignificant. Moreover, the arterial oxygen saturation percentage and breathing rate increased in the intervention group. No significant difference was observed between the two groups regarding crackles at different hours after surgery, although they were relatively less in the intervention group. Conclusions: Breathing exercises, especially combined breathing exercises, improve respiratory function after open heart surgery.

Background & Objective: Learners prefer to use a system for learning that improves their interaction; therefore, self-regulated learning is important. The present study aimed to determine online self-regulated learning (OSRL) and its related factors among medical science students during the COVID-19 pandemic, 2021. Materials & Methods: This cross-sectional study was conducted on 249 students affiliated with Mazandaran University of Medical Sciences, Mazandaran, Iran, using a non-randomized quota sampling method. The required data were collected using a demographic form, an Online Self-Regulated Learning Questionnaire, and a researcher-made questionnaire of related factors of OSRL based on a literature review. The data were analyzed using descriptive and inferential statistical methods, such as ANOVA, t-test, Pearson, and multivariate linear regression using the Enter method. Results: The mean of the total score of self-regulated learning was 80.2±15.5 and in a good range. Among the related factors, students' gender (β=-0.239), marital status (β=-0.210), age (β=0.195), and time management (β=-0.125) explored 16.5% of the variance OSRL. The other variables had no significant relationship with OSRL. Conclusion: In the present study, marital status, gender, age, and time management significantly correlated with the variance of OSRL. Different studies are necessary to investigate the variables that explore OSRL so that educational managers can provide a better virtual learning environment for students by considering these variables.



Death anxiety and its predictors among the companions of hospitalized patients with coronavirus disease 2019

Background and aims: Death anxiety (DA) is associated with stress and impaired immunity and thereby, puts individuals at risk for coronavirus disease 2019 (COVID-19). The aim of this study was to assess DA and its predictors among the companions of hospitalized patients with COVID-19. Methods: This descriptive-analytical study was conducted in 2020 on 200 companions of patients with COVID-19 hospitalized in Imam Khomeini, BouAli, and Razi hospitals in Sari and Qaemshahr, Iran. Sampling was performed conveniently. Data were collected using a demographic and clinical characteristics questionnaire for patients, a demographic and clinical characteristics questionnaire for companions, and Templer DA Scale. The SPSS software (v. 21.0) was used for data analysis via the independent sample t test, chi-square test, Pearson’s correlation analysis, and multiple linear regression analysis. Results: The mean score of DA was 47.21±10.62 (in the possible range of 15–75). Participants’ DA had significant relationship with their gender, their patients’ anorexia and arterial oxygen saturation, and COVID-19 among other family members (P<0.05). Participants’ gender, patients’ anorexia and arterial oxygen saturation, and COVID-19 among other family members significantly explained 0.066% of the total variance of DA (P=0.002). The strongest predictors of DA were arterial oxygen saturation (β=0.17; P=0.01) and gender (β=0.15; P=0.0). Conclusion: The companions of hospitalized patients with COVID-19 experience high levels of DA and hence, interventions are needed to reduce their DA.


An investigation of hospital anxiety and depression and associated factors in COVID-19 patients

Context: The high incidence of anxiety and depression in patients with corona can be associated with serious complications in recovery and increased mortality. Aim: This study was performed with the aim of determining the anxiety and depression of patients with corona and related factors. Setting and Design: This study was conducted in general and special wards (coronary care unit) in Iran (Imam Khomeini and Bu Ali Sina in Sari and Razi hospitals in Ghaemshahr) in 2020. Materials and Methods: Patients were selected by accessible sampling. The Hospital Anxiety and Depression Scale was used to assess anxiety and depression. Statistical Analysis Used: This descriptive-analytical study was performed on 150 patients with Covid-19 admitted. Data were analyzed using SPSS software version 21, descriptive statistics and inferential statistics such as t-test, Chi-square, Fisher, Pearson correlation coefficient, and multiple linear regression test. Results: The mean score of patients' anxiety was 6.83 ± 5.16, and the mean score of depression was 7.09 ± 4.37. According to the multiple linear regression test, depression with standard β (ß =0.53) and with standard β (ß =0.21) explore 42% of variance of anxiety. Anxiety (β =0.56), temperature (β =0.2), anorexia (β =0.18), and respiratory rate (β =0.12) explore 45% of variance of depression, respectively. Conclusion: The results of this study indicated a high prevalence of anxiety and depression in hospitalized patients with corona disease. Therefore, further study of related factors and planning appropriate interventions according to the results of the study is recommended.



Comparing the Effect of Acupressure and Reflexology on Sleep Condition Following Coronary Artery Bypass Graft Surgery

Background and purpose: Sleep disorders after heart surgery lead to increased heart rate, increased myocardial oxygen demand, and cause dysrhythmia that worsen heart ischemia. The purpose of this study was to compare the effect of acupressure and reflexology on sleep condition following coronary artery bypass graft surgery. Materials and methods: A randomized controlled trial was performed in 60 patients. They were randomly assigned into three groups. Two intervention groups; reflexology (n=20) and acupressure (n=20), and a control group (n=20). For three to six days after the surgery, foot reflexology and acupressure were done for 30 min and 21 min, respectively. St. Mary's Hospital Sleep Questionnaire was administered for five consecutive days for each group. Data was analyzed in SPSS V25. Effect of Eucalyptus Vapor on Hoarseness and Sore Throat Following Intubation after Coronary Artery Bypass Surgery

Background and purpose: Sore throat and hoarseness are some postoperative complications following endotracheal intubation and inflammation of the pharynx, larynx, and trachea. The aim of this study was to determine the effect of eucalyptus vapor on sore throat and hoarseness after tracheal extubation in patients undergoing coronary artery bypass graft (CABG). Materials and methods: In a clinical trial, 100 patients undergoing CABG were selected using convenience sampling. They were randomly divided into two groups (control and experimental). Demographic and clinical data, and the information on sore throat and hoarseness were recorded in both groups, before the intervention and after tracheal extubation. The patients in experimental group were exposed to eucalyptus vapor for about ten minutes after tracheal extubation. This was performed at 1 and 12 hours after extubation. The severity of sore throat and hoarseness was recorded in the two groups at 0, 1, 6, 12 and 24 hours after extubation. Results: The severity of hoarseness in case group showed no significant difference at the times of immediate to 24 hours after extubation. But, the severity of sore throat significantly reduced at 6 and 12 hours after extubation (P= 0.02 and P= 0.02, respectively). Conclusion: The eucalyptus vapor in this study could not significantly decrease the severity of hoarseness, but reduced the sore throat.
