p>Pharmaceutical Care is a practice that the main purpose is to improve the patient’s quality of life who use frequently drugs, especially whose chronic use. Hypertension is considered a public health problem, with significant risk factors to increase cardiovascular issues in a population. Patients with this type of chronic disease can use a specific medication to stabilize their clinical system. In addition, these patients often use the medication improperly as a different drug amount or quality and stored incorrectly in their home. Through this pharmaceutical care practice, the pharmacist may interact directly with the patient to identify their needs related with drugs. Given these considerations, this study aim to develop a safer and more effective antihypertensive pharmacotherapy providing information about the correct and safe drugs use. This is the way to avoid the patients with early return in a hospital for treatments.</p
