Sugar in infant food: spanish and European regulations, who are they protecting?

The high level of sugar intake in infancy and childhood contributes to the current epidemic of tooth decay and childhood obesity, influencing negatively, as well, on low rates of breastfeeding. One of the measures to control this consumption is the determination by the health authorities of the maximum amount of existing sugar in infant foods produced by the food industry. This article inspects the law in Europe and Spain governing the presence of sugar in baby foods other than infant formula: Directive 2006/125/EC and Royal Decree 490/1998. After reviewing the legislative and compilation or consolidation technical rules, the respect of deadlines and the suitability of the procedures, we can conclude that the lack of accuracy is evident. Regarding the amount of sugars allowed in baby foods, they are far from ideal, to the extent that they can be considered an obesogenic factor and an issue that promote bad dietary habits throughout childhood. Therefore, we can question if the law protects infant’s health or the interest of food industry. © 2016, Spanish Association of Primary Care Pediatrics. All Rights Reserved.

The health benefits of daily consumption of at least 5 portions of a variety of fruit and vegetables are well documented. The World Health Organization (WHO) public health goal, established in 600 grams per person/day, is not achieved in Spain, so it is important to improve access to these foods, maintain their potential nutritive value and overcome barriers for their consumption. The objectives of this paper are: facilitate responsible decision-making regarding health; maximize the nutritional value of fruit and vegetables, help overcome barriers for fruit and vegetables consumption and report on how their storage at home, their handling and cooking affect to nutritional value of fruit and vegetables.In order to minimize nutrients loss and improve their bioavailability during fruit and vegetables handling, the Association for the promotion of fruit and vegetables “5 a day” (Spain) recommends: avoid prolonged storage of fruit and vegetables in the fridge; use layers and outer leaves; peel and/or cut them just before consumption; wash the whole pieces and then chop them; controlling the soaking time of cut pieces; prefer cooking techniques that do not require direct contact with water; a shorter cooking time, less loss of nutrients; the correct frying retains nutrients very well, although it is a technique of which should not be abused; add a few drops of vinegar or lemon juice to the cooking water if it does not change the taste and dish acceptance; use the water of cooking to elaborate other foods (e.g. sauces, soups, purees, etc.) except those coming from chard, spinach or beets. The “5 a day” Association (Spain) recommends increasing consumption of fruit and vegetables, and state that theloss of nutrients during their home handling should not be understood as a barrier for their consumption.

Although the consumption of vegetables is recommended for adults, infants and young children, there is a risk that their consumption results in a high intake of nitrates that increases the risk of methemoglobinemia. Even though the average nitrate intake in children does not exceed the European safety threshold, an analysis conducted by the European Food Safety Authority (EFSA) in 2010 revealed that the consumption of spinach may be involved in certain cases of infant methemoglobinemia. The Spanish Agency for Food Safety and Nutrition (AESAN) found in 2011 that chards, because of its remarkable consumption in Spain, may also pose a risk for children. In addition to the AESAN recommendations, that any health care professional in the field of infant nutrition should know, it is prudent to add that a high consumption of borage (Borago officinalis, a vegetable widely consumed in Navarra, La Rioja and Aragón in homemade purees) may also be involved in infant methemoglobinemia. In any case, and as EFSA and AESAN point out, the benefits of eating vegetables prevail over the risks of the exposure to nitrates linked to their consumption.

Introduction: Caffeine and catechins contained in green tea may have a thermogenic effect favoring weight and body fat loss. The aim of this study is to evaluate the magnitude of the effect of green tea or its extracts (caffeine and catechins) on body weight and body composition. Material and methods: A systematic review and metaanalysis was conducted to determine the magnitude of the effect of green tea or its extracts on body weight (kg), body mass index (BMI) (kg/m²), fat mass (%), and waist and hip circumference (cm). We included studies published between 2000 and 2013, retrieved from PubMed/Medline with the following characteristics: randomized controlled trials (RCTs) of parallel groups (intervention and placebo), randomized, double-blind, and a minimum 12-week follow-up, in healthy individuals of either gender, 18 years or older, with a BMI of 25-40 kg/m². Quality and risk of bias was assessed for every included study, and the statistical analysis was performed with the Crochrane Collaboration RevMan 5.1.6 software, according to the random effects model with a confidence interval of 95% (95%). Dietary intake of nitrate in Spanish infants and children and risk of methemoglobinemia

Nutrition and health claims are an added value that can position and differentiate a product or brand. Fraud issues in what scientific analysis is required are hardly refutable by the consumer. Legislation and official scientific bodies should ensure that the communication is truthful. Regulation 1924/2006 and the European Food Safety Authority (EFSA) have a key role, being the focus on the creation and final acceptance of nutrition and health claims. However, the coexistence of specific legislation applicable to foods for infants and young children to the Regulation 1924/2006, has complicated the interpretation and application of the regulation to these foods of special consideration. Directives 2006/141 and 2006/125 contain some specific sections on nutrition and health claims not subject to approval by the EFSA, and sometimes the Regulation 1924/2006 conflicts with these specific directives in this area. In this review, we reveal some of the roughest stories involving the development of a necessary, ambitious and complex legislation.

Food advertising has a great ability to influence the behavior of the population. Promises made through advertising should be consistent with reality. Tangible fraud can be easily recognized. However, non-tangible fraud (such as nutrition and health claims) is difficult to be detected by the “reasonably well-informed, and reasonably observant and circumspect consumer”. Regulation 1924/2006 (1) establishes the legal framework for such statements. While nutritional claims are well established in the regulation annex, health claims need to be specifically evaluated by an EFSA scientific panel before they can be used. Can Regulation 1924/2006 and EFSA assessments prevent all nutritional and health claim frauds? Are there loopholes that could potentially allow misleading claims? Can EFSA deny a health claim that may be scientifically well founded? Can a denied health claim be accepted in the future? Can EFSA panels have conflicts of interest? (2) The study of Regulation 353/2008 (3), the EFSA scientific opinions issued up to date, and the evidence-based medicine (4) are the key points to answer these questions. Healthcare professionals should ensure that the information delivered to consumers, whether legal or not, is scientifically proven and promotes public health.
