Background: Erosive oral lichen planus (EOLP) causes pain, burning sensation, swelling, and bleeding and can affect the quality of life because of pain and discomfort. Here, we report a case of EOLP in an elderly patient, which decreased the quality of life, and a comprehensive management of the case. Case Report: A 75-year-old woman was referred to the Oral Medicine Department of Dental Hospital, Faculty of Dentistry, Universitas Indonesia, with oral discomfort. She complained of pain during meals for 1 year, which results in weight loss. The patient admitted to the hospital because of psychological stress and hypertension. The clinical examination of the right buccal mucosa showed reticular plaque, and erosive and erythema lesions on the left buccal mucosa and soft palate. The patient was diagnosed with EOLP and treated with a topical corticosteroid and multivitamin, and also was treated for the management of stress. Conclusion: EOLP treatment in the elderly patients has its own challenges, because of chronic disease, affects the quality of life, and requires long-term therapy. Treatment must be comprehensive and must pay attention to the systemic and psychological condition of the patient.

The symptoms of recurrent oral herpes infection may vary, from mild discomfort to life threatening. Dentists are more likely to be consulted in this oral infection, hence the ability to diagnose and treat this disease is mandatory. This article described manifestation and therapy of recurrent oral herpes infection. In the first case, a 41-year-old woman came with a complaint of painful sore mouth in her lower gingiva. She experienced canker sores for two to three times a year on the tongue, lips and palate. Laboratory results showed positive IgM anti HSV-2, which matched with the primary herpes infection. However, based on the history and clinical manifestations, she was diagnosed with recurrent intra oral herpes infection. In the second case, a 70-year-old man came with a very painful canker sores in his entire mouth. The patient had to be hospitalised for three days and received analgesic, antibiotic and gel containing triamcinolone. After hospitalisation, canker sores did not heal. Extraorally, we found a crust in the vermillion border of the upper lip and intraorally, we found multiple ulceration of keratinised and non-keratinised mucosa. The use of topical steroid in this patient may aggravated ulceration due to its ability to cause rapid spreading of the virus. The first patient was given chlorhexidine gluconate 0.2% and the second patient was prescribed with doxycycline rinse. Both patients received multivitamin containing zinc. Complete history taking, objective and adjunctive examination played a role in establishing the diagnosis and treatment of recurrent oral herpes infections.

Introduction: Oral lichen planus is a common inflammatory dermatological disease of the oral cavity. It is characterized by bilateral white striation (Wickham's striae) or plaque on the buccal mucosa, tongue, or gingiva, and has a unique multifactorial etiology in which stress may play an important role. We report a case of oral lichen planus caused by psychological stress in response to environmental change and describe our treatment approach. Case Report: A 30-year-old male patient came to Universitas Indonesia Dental Hospital with a chief complaint of a burning sensation during spicy food consumption and thickened white plaque on the buccal mucosa, mucobuccal fold, and tongue. He also complained of tongue rigidity in the morning. These symptoms started 6 months earlier after he moved to Jakarta where city life and hot weather made him uncomfortable. The systemic disease was not indicated in the laboratory results. Treatment with topical steroids initially and environmental changes later improved the patient's symptoms and clinical lesion. Conclusion: Environmental change that leads to psychological stress might trigger oral lichen planus. Successful treatment, in this case, required appropriate anamnesis to identify and eliminate the triggering factor and select a pharmacological therapy.

Necrotizing ulcerative stomatitis (NUS) merupakan suatu penyakit inflamasi dengan lesi ulserasi nekrosis pada mukosa mulut yang jarang terjadi. Karakteristik lesi berupa ulser akut dengan kerusakan masif, nekrosis pada jaringan lunak yang dapat meluas ke jaringan sekitarnya. Pada stadium lanjut, dapat berkembang menjadi cancrum oris (noma). Kasus ini melaporkan NUS terkait AIDS yang terdapat pada mukosa mulut tanpa perluasan lesi ulserasi nekrosis dari gingiva dan periodonsium. Perbaikan awal diperoleh setelah 8 hari perawatan dengan antibiotik metronidazol 1500 mg sehari dan berkumur chlorhexidine gluconate 0,2% dua kali sehari.Kata kunci : Necrotizing ulcerative stomatitis, gambaran klinis

The government is obliged to guarantee the availability of health care facilities and assistance for elderly individuals. There have been only a few studies of oral lesions in the elderly in Indonesia. To describe the oral health status of elderly residents of state institution in Jakarta. Participants were recruited via the consecutive sampling method. Of 220 elderly residents, 49 met the inclusion criteria. Socio-demographic data and medical histories were obtained from the medical records of the institution. All the patients underwent a clinical oral examination, which included Oral Hygiene Index-Simplified (OHI-S), Mucosal Plaque Index (MPI), and Salivary Flow Rate (SFR) measurements. The participants were also questioned about their oral habits, and any oral lesions were recorded. The prevalence of oral lesions was 55.1% (n=27), with only 20.24% (n=10) of elderly residents lesion free. The most common oral lesion was melanin pigmentation. None of the residents had severe pathological lesions, such as a malignancy or deep infection. This study found the most elderly subjects in the state institution was in poor oral health, implying that the government’s aim to promote the well-being of elderly population has not been achieved.
