Background Previous studies have examined the various outcomes of expressive writing intervention in different populations. The purpose of this study was to evaluate the effect of expressive writing on the sexual self-concept of men with multiple sclerosis. Methods This parallel randomized controlled clinical trial conducted on 70 men attending an MS clinic in Tehran in 2022. The participants were randomly assigned to expressive writing intervention and control groups. For two weeks, participants in the intervention group disclosed their emotions and expressed their feelings in writing twice a week. Sexual self-concept was evaluated using the Multidimensional Sexual Self-Concept Questionnaire on three occasions, before, after, and 4-week follow-up. The data ware analyzed using SPSS software version 22 (IBM Corp., Armonk, N.Y., USA). The analysis employed Chi-squared test, Fisher’s exact test, independent t-test, and repeated measured ANOVA. A p-value < 0.05 was considered to be statistically significant. Results The mean of age in the intervention group was 38.6 ± 6.85 years, while in the control group it was 38.94 ± 6.07 years. There were no significant differences between the two groups, in terms of these characteristics. According to the results, the dimensions of SSC did not change over time assessments, except for sexual problem self-blame (p = 0.011) and sexual fear/apprehension (p = 0.042) in the expressive writing intervention group, and sexual motivation in the control group (p = 0.002). On the other hand, before the intervention, sexual motivation was significantly higher in the control group compared to the expressive writing intervention group (p = 0.046). Conclusions Our findings did not support the general effectiveness of a 2-week expressive writing intervention on the sexual self-concept of the men with MS except for the sexual fear/apprehension and sexual problem self-blame dimensions. However, further research is needed to better understand the effects of expressive writing intervention on Sexual self-concept in men with multiple sclerosis, while controlling for the limitations of our study. Women with Spinal Cord Injuries, Anxiety and Sexual Dissatisfaction: Do They Benefit from Sexual Rehabilitation Education?

Background: Spinal cord injury (SCI) affects all body systems, including physiological sexual responses, and negatively impacts sexual satisfaction and anxiety, leading to depression and damage to interpersonal relationships in couples. Therefore, sexual rehabilitation education by rehabilitation nurses is essential to prevent complications and improve performance. Objectives: This article aims to investigate the effect of sexual rehabilitation education on anxiety and sexual satisfaction among members of the Tehran Spinal Cord Injury Association. Methods: In this semi-experimental study, 100 women referred to the Tehran Spinal Cord Injury Association were randomly assigned to intervention and control groups. Data were collected using a demographic form, Beck's Anxiety Questionnaire, and Larson's Sexual Satisfaction Questionnaire. The data were analyzed using SPSS version 21 software and statistical tests, including chi-square, Fisher's exact test, and independent t-tests. Results: The mean anxiety score in the control group before and after the intervention was 19.66 and 20.68, respectively (P = 0.123), whereas in the intervention group, the mean scores were 20.92 and 18.04, showing a statistically significant reduction (P = 0.045). The mean sexual satisfaction score in the control group was 67.32 and 67.22, respectively (P = 0.912), while in the intervention group, it increased from 67.80 to 70.80, a statistically significant improvement (P = 0.010). Conclusions: The results of this study demonstrated that sexual rehabilitation education reduces anxiety and increases sexual satisfaction in women with spinal cord injuries. Implementing sexual rehabilitation education programs by nurses is an effective strategy to reduce anxiety and enhance the sexual satisfaction of women with spinal cord injuries.

The Effect of Music Therapy on the Sleep Quality of Patients with Heart Failure: The Miracle of Nature Music

Background Patients with heart failure experience poor sleep quality due to the nature of disease they suffer from. The aim of this study was to investigate the effect of music on the sleep quality of patients with heart failure. Materials and Methods The present study is a randomized clinical trial, which was performed on 76 patients with heart failure, hospitalized in the cardiac ward of an educational hospital. These patients had obtained a score of 5 or higher on Pittsburgh’s sleep quality index. The block randomization method was used to generate the allocation sequence. Thus, the number of subjects in each block was determined to be 4 people in 18 blocks. Intervention was collection of nature music played through headphones for 30 minutes over three nights, and at the end of the third night, the Pittsburgh’s index was completed again by both groups. Data were analyzed using descriptive and inferential statistics. Results After the intervention, the mean scores of mental quality of sleep ( p = 0.007) and daily function disorders improved significantly in the intervention group ( p = 0.025). Significant differences were seen between the mean scores of mental quality of sleep ( p < 0.001), daily function disorders ( p = 0.002), delay in falling asleep ( p = 0.01), sleep disorders ( p < 0.001), and use of sleeping pills ( p < 0.001) before and after the intervention in the intervention group. Conclusions Playing relaxing music like nature sounds and spending time in nature after discharge can improve the sleep quality and sense of relaxation in patients with heart failure. The implementation of such affordable and amusing interventions can be proposed by nurses in heart failure patients.



Effect of Companion Presence during Skin-to-Skin Contact on Maternal Anxiety: A Randomized Clinical Trial

Objective: To compare the effect of companion presence versus midwife presence during skin-to-skin contact (SSC) at birth on maternal anxiety and satisfaction, and neonatal physiological parameters. Methods: A randomized controlled trial was conducted on 92 pregnant women who were randomized to provide SSC to their newly borns for one hour postpartum, in the presence of a companion (study group) or a midwife (control group). Maternal anxiety (using the Visual Analogue Scale) and the neonatal physiological parameters (including temperature, heart rate, and oxygen saturation) were assessed in four stages viz., immediately after birth, and at 30, 60 and 90 minutes after birth. Maternal satisfaction was also evaluated after transferring the mother to the postpartum ward. Results: We analyzed 86 mother-infant dyads (43 per group). Having a companion significantly reduced maternal anxiety after birth as compared to having a midwife at 30, 60, and 90 minutes after birth (P = 0.04, P = 0.01, and P = 0.04, respectively). There was also a small to medium effect size of the presence of companion compared to midwife in terms of maternal anxiety at 30 minutes (Cohen's d = 0.45; 95% CI = 0.02, 0.87), 60 minutes (Cohen's d = 0.52; 95% CI = 0.08, 0.94) and 90 minutes after birth (Cohen's d = 0.45; 95% CI = 0.02, 0.88). However, there was no significant effect of the same on neonatal physiological parameters. Having a companion versus a midwife led to higher maternal satisfaction rates (P = 0.02); 65.1% of mothers in the study group and 37.2% of mothers in the control group were desirous of the same care in future (P = 0.02). Conclusion: Companion presence during SSC leads to a significant reduction in maternal anxiety compared to midwife presence.

The effectiveness of web-based training for parents on post-traumatic stress disorder in children

Introduction Post-traumatic stress disorder (PTSD) after an injury such as accidents is common in children and can affect their overall physical and mental functioning and quality of life. Early intervention can have significant health benefits for children. This study aimed to investigate the effectiveness of web-based training for parents on post-traumatic stress disorder in children. Method This was a quasi-experimental study with intervention and control group. 110 parents of children aged 10–18 years with PTSD after a traumatic event were selected through available sampling and assigned to intervention and control groups. Data was collected by a researcher-made demographic questionnaire and the Child Revised Impact of Events Scale (CRIES-8). Parents in the intervention group received a 4-week training course through a researcher-designed website, but the control group received routine care by the clinical team, which the main focus of care and training was on the physical aspects of the disease, and no intervention was done for PTSD. Two weeks after the intervention, the level of child stress was measured and compared in both groups. Data were analyzed using SPSS V.22. Results The difference between the mean score of total traumatic stress and its subscales before intervention was not statistically significant (p = 0.23). But after intervention, the mean score of total traumatic stress and its subscales decreased in the intervention group and increased in the control group and this difference was statistically significant (p < 0.001). Conclusion E-learning parent training has the potential to support children with PTSD. This available and cost-effective procedure can be recommended to help children with PTSD and possibly increase recovery in these patients.

Background The aim of this study was to evaluate the effects of thiamine (vitamin B1) on general health and infertility treatment outcomes in women with polycystic ovary syndrome (PCOS). Materials and Methods The study is a triple-blinded, randomized, placebo-controlled clinical trial performed on 64 infertile women with PCOS referred to Sarem Hospital in Tehran, Iran. The primary outcomes of the study were general health and infertility treatment outcomes. Eligible women were randomly assigned to the vitamin B1 group (n=32, vitamin B1 tablet at a dose of 300 mg/day for 4 weeks) or the placebo group (n=32, placebo tablet daily for 4 weeks). A general health questionnaire was completed before and after the intervention by both groups, and treatment success was evaluated at the end of the study. Data were analyzed using SPSS software ver.16 P<0.05 was considered statistically significant. Results The mean age of participants in the vitamin B1 (VB1) group was 30.4 ± 3.27 years and in the placebo (Pl) group was 29.1 ± 2.66 years with the mean duration of marriage 12.7 ± 3.01 and 13.2 ± 2.97 years respectively. Our results showed that there were significant differences between the two groups in overall score (P<0.001) and scores for all domains of the general health questionnaire including somatic symptoms (P<0.001), anxiety and insomnia (P<0.001), social dysfunction (P=0.028), and severe depression (P<0.001) after the intervention. Four weeks consumption of vitamin B1 also resulted in higher numbers of positive pregnancy tests (P=0.006), although the number of fetuses was not significantly different between the two groups after the intervention. Conclusion The results of the current study support a possible favourable effect of vitamin B1 on improving general health, infertility treatment outcome, and retrieved follicle count without changing the number of fetuses in women with polycystic ovary syndrome (registration number: IRCT201510266917N3).
