Background: Celiac plexus block has been commonly utilized for the treatment of chronic pancreatitis-associated abdominal pain. Prospective studies suggest efficacy in 30 to 50% of patients, although no randomized sham-controlled trials have been performed. The objective of this study is to assess the effect of endoscopic ultrasound (EUS)-guided celiac plexus block on abdominal pain in patients with documented chronic pancreatitis. Methods: This is a two-arm randomized sham-controlled trial with blinded evaluators. The study will be conducted at multiple academic sites in the United States who are members of the United States Pancreatic Disease Study Group (USPG). Patients referred for EUS to exclude chronic pancreatitis as a cause of abdominal pain as well as those with established painful chronic pancreatitis undergoing EUS for another indication will be eligible. At the time of EUS with confirmation of chronic pancreatitis by standard EUS diagnostic criteria, patients will be randomized to either celiac plexus block or sham whereby an anesthetic and steroid combination will be injected into the celiac plexus or saline will be injected into the gastric lumen with the same type of needle as used for celiac plexus block, respectively. The main outcome measure will be a 50% reduction in abdominal pain using the Brief Pain Inventory Short Form (BPI-SF) at 1 month post-intervention. A number of secondary outcomes will be measured including visual analog scale (VAS), Comprehensive Pain Assessment Tool Short Form (COMPAT-SF) pain scores, and quality of life using a pancreas-specific validated measure (PANQOLI). Discussion: In this study, the effect of celiac plexus block on abdominal pain in patients with chronic pancreatitis will be compared to a sham intervention. This randomized trial will offer a definitive assessment of the role of celiac plexus block for the treatment of abdominal pain in this setting. TRIAL REGISTRATION {2}: ClinicalTrials.gov NCT06178315. Registered on December 21, 2023.

... Another example of a wider use of neuroanatomy by health care professionals different from neurologists/neurosurgeons is the implementation of US to study the peripheral nervous system: on top of the most obvious diagnostic application for diagnosis of neuropathy [3,4], US can be used, in fact, to guide procedures such as local anesthesia and pain management targeting a specific branch of the peripheral nervous system [11][12][13][14]. ...

... In this regard, it should be emphasized that the documentation of an abnormal sphincteric basal pressure in type 2 SOD may be difficult in clinical practice because SOM has been mostly abandoned due to its invasiveness and poor diagnostic reliability and the secretin test is not available in most centers [46,47]. However, despite the encouraging results published years ago, in recent studies on large series of patients, the percentage of recurrent AP was reported to vary from 17.4% to 40% of cases in both groups [48,49]. In these cases, sphincter dysfunction was probably not the cause of RAP, although it was documented. ...

... Increasing evidence supports the effectiveness of rectal NSAIDs, aggressive hydration, and prophylactic pancreatic stents in preventing PEP [2,3,14]. The combined use of these measures has been shown to further reduce PEP risk [32,33]. Second, the publication of the first clinical practice guidelines for PEP in 2015 [14], along with the revised guidelines for the management of AP in the same year [34], may have increased awareness of PEP risk factors and preventive strategies among Japanese endoscopists. ...

... Over the past decade, these challenges have been overcome through advancements in ultrasound resolution and the availability of smaller needle electrodes [46]. As a result, RFA, including percutaneous, endoscopic, and intraoperative approaches, is now considered a feasible, safe, and effective option for patients with insulinomas who are unable or unwilling to undergo surgical resection [6,[14][15][16]20,[23][24][25][26][27][28][29][30]32,33,[35][36][37][38][39][40][41][42][43][46][47][48][49][68][69][70]. This study demonstrates that percutaneous ultrasound-guided RFA is an effective and safe procedure for treating insulinomas and their metastases in dogs, potentially offering a new alternative approach for managing neuroendocrine pancreatic tumors. ...

... Radiofrequency ablation (RFA) has emerged as a promising minimally invasive technique for managing pancreatic cancer, particularly in unresectable cases. RFA uses focal thermal energy to induce cell death through coagulative necrosis and protein denaturation [155]. Recent studies have demonstrated RFA's potential to reduce tumor progression, promote an abscopal effect, and induce significant remodeling of the tumor stroma and immune response [156]. ...

... EUS clearly identifies the origin of the lesion, reveals the stratification of the digestive tract wall, and provides information on echo intensity, echo uniformity, and the boundary of the lesion. Despite these advantages, the endoscopic ultrasound appearance of glomus tumor has similar manifestations with other submucosal tumors, which are often difficult to identify (Table 1) (2,8). In color Doppler images, blood flow within the glomus tumor is sometimes visible. ...

... 6,42 Chronic pancreatitis is more common in men than in women and in Black individuals than in Caucasians, peaking at 46-55 years of age. 9,43 Etiology Although several causes of chronic pancreatitis have been described in the literature, most patients eventually develop typical clinical and morphological features. 10 Risk factors include alcohol use (in 42%-77% of cases), smoking (over 60% in some studies), and genetic mutations (present in 10%). ...
