Bledsoe and Izenstark [3], in an anatomical study, showed that displacement occurred after the injection of a radiolucent fluid first into the anterior part of the joint, and when the intra-articular pressure increased, the posterior fat pad became visible on radiographs. This could be attributed to the fact that the lateral elbow radiographs were taken with the elbow flexed, and this relaxes the brachialis anteriorly and tenses the triceps posteriorly [12].
