INTRODUCTION: In relation to torsion, mass appearance may occur due to hemorrhage into the cyst, and blood supply of the over tissue may not be well evaluated radiologically. For this reason, the diagnosis of torsion and non-torsion MCT continues to be a serious dilemma. METHODS: The laboratory and imaging methods of ovarian teratomas with or without torsion were compared with the statistical method. The results were evaluated statistically by using SPSS version 24. Normality checks were performed with the Shapiro-Wilk test. The groups were compared with the independent samples t-test. Statistical significance was p <0.05. RESULTS: 40% of the cases with CRP level elevation, torsion were significantly higher (> 5 mg / L). The most common surgical approach was fertilization protective cystectomy applied in 14 cases (77 %). DISCUSSION AND CONCLUSION: The first choice in benign ovarian masses is fertility protective surgery. CRP, WBC and CA19-9 can be a marker, especially in torsion cases. The planning of the study with prospective and large number of patients will be appropriate.

Situs inversus totalis (SIT) is an unusual anomaly that characterized by complete mirroring of organ locations in the opposite side of the body. Acute appendicitis, intussusception, duodenal atresia, esophageal atresia, and occasionally hydrocolpos are frequently encountered in pediatric surgeons in clinical practice. As a result; the surgeon can easily apply current treatment procedures in surgical conditions complicated with SIT as long as the surgeon is aware of the anatomical and spatial state. The diagnosis and management of pediatric pancreatitis is often difficult. Blunt abdominal trauma is one of the most common causes of pancreatitis in childhood. This is a review of patients with blunt pancreatic trauma. Medical records of all children with blunt pancreatic trauma who were admitted to our clinic between July 2007 and December 2017 were analyzed retrospectively. There were 17 patients (7 girls, 10 boys) with a mean age of of 9.89±2.57 years (range, 3-14 years). The time from trauma to admittance to the clinic was mean 17±21.43 days (range, 0-63 days). The mechanism of injury was bicycle injuries in 7 (41.66%), a fall from height in 6 (35.29%), motor vehicle accidents in 3 (17.64%), and a fall from a horse in 1 (5.88%). All patients had serum pancreatic amylase elevation on admission (average, 1002.71±966.36 IU/L; range, 116-3163 IU/L). All the childen were initially managed non-operatively. In 8 patients (52.94%) the traumatic pancreatitis was complicated by development of a pseudocyst. The average diameter of the pseudocyst was 11.5±3.45 cm. The pseudocysts were resolved in 5 cases with percutaneous ultrasonography-guided fluid drainage and the remaining 3 required open surgery (2 cystogastrostomy, 1 surgical drainage). The average length of hospital stay was 18±15.91 days (range, 2–48 days). There were no deaths and no complications in any patients. Non-operative management of traumatic pancreatitis in the absence of complete duct transection is safe in children. Percutaneous drainage of traumatic pancreatic pseudocysts can be used as an effective and useful treatment option. [Med-Science 2019; 8(1.000): 225-8]

A New Enema for Treatment of Intussusception with Hydrostatic Reduction: Olive Oil

Objectives: Intussusception is routinely treated using ultrasound-guided hydrostatic reduction (USGHR) with normal saline in our paediatric surgery department. With this study, olive oil was added to normal saline in ultrasound-guided reduction of intussusception. Materials and methods: Forty patients who were diagnosed and treated for intussusception in Van Yuzuncu Yıl University Faculty of Medicine Pediatric Surgery Department from March 2017 to May 2017 were included in the study. During this retrospective study, randomly chosen 20 patients that treated with USGHR using normal saline were marked as Group 1. Moreover, 20 patients that treated with USGHR using a mixture of olive oil and saline (10% olive oil 90% normal saline) were marked as Group 2. Patients' age, gender, symptoms, treatment techniques, complications and hospitalisation periods were retrospectively noted. Results: Forty patients were included in the study. In Group 1, 14 patients were treated in the first session, 4 patients were treated in the second session and 2 patients required laparotomy. Mean reduction time in this group was 15 min mean fluid volume used in each reduction was 80 ml/kg and mean hospitalisation period was 38 h. In Group 2, 19 patients were treated in the first session, and only 1 patient required a second session. Mean reduction time was 12 min, used fluid volume was 58 ml/kg for each reduction and hospitalisation period was 24 h. Conclusion: The average volume of fluid used for reduction, average reduction time, numbers of recurrent reductions and hospitalisation were less when a mixture of olive oil and normal saline were used in comparison with when normal saline was used alone to reduction the intussusception under ultrasound guidance. Using olive oil mixed with normal saline as a new enema fluid is likely to increase the success rate of ultrasound-guided reduction of intussusception.



Ileocecal stenosis with agenesis of vermiform appendix: A rare cause of congenital bowel obstruction

A 3-month old male patient, with chronic constipation and abdominal distension complaints, was intraoperatively diagnosed with ileocecal stenosis with agenesis of the vermiform appendix. Agenesis of the vermiform appendix is a very rare anatomical finding with an incidence rate about 0.001% in patients who receive laparotomy due to suspected acute appendicitis. No previous cases of ileocecal stenosis with agenesis of the vermiform appendix were reported in the literature. Ileocecal stenosis with agenesis of the vermiform appendix, which present with more shallow symptoms as opposed to ileocecal atresia, can be a cause of congenital bowel obstruction during neonatal period. Stenotic segment was resected without ileocecal valve reconstruction and then ileocolic anastomosis was performed. Development of the baby was normal during post-op 6-month follow-up period. The main objective of this case report is to attract the attention of pediatric surgeons in differential diagnosis of ileus and treatment strategies during laparotomy.
