The geochemical conditions of landscapes are the content in the environment of certain chemical elements and their compounds, the lack or excess of which causes deviations in the state of human health. This problem has arisen in connection with the extraction of alluvial diamonds and the forthcoming development of the Tomtor deposit of rare-earth metals in the territories where the indigenous peoples of the North live. The study included 107 indigenous people of the North, belonging to the ethnic group of Dolgans living in the village of Yuryung-Khaya, Anabar district, Yakutia of Russia. The method of mass spectrometry was used to study the content of 13 trace elements in blood serum (P, Sc, Ti, Cr, Mn, Fe, Ni, Cu, Zn, Rb, Sr, Cs, Pb). The study revealed an increase in the content of the macroelement phosphorus (148 mg/L) and trace elements of chromium (277 μg/L), manganese (133 μg/L), iron (5219 μg/L), nickel (57 μg/L) in serum of Dolgans, which may affect the development of diseases of the cardiovascular system and other diseases among indigenous inhabitants of the Arctic under conditions of industrial development of territories.

Objective: Inrtroduction: The epidemiological situation for hepatitis D has changed significantly. Reduced population authors infection due to a sharp decline in hospitalizations from Central Asia regions, the Caucasus and Moldova, which are known to be endemic for hepatitis D. Currently, the incidence of chronic hepatitis D (HGD) in Russia is 1%, while in the countries of Central Asia, and in particular in Turkmenistan, the share of HGD among chronic viral hepatitis is 8%. The aim of research was to establish the clinical features, depending on the activity of the replication of hepatitis viruses B and D. Patients and methods: Materials and Methods: We studied 26 patients with viral hepatitis D with a determined activity replicative virus by PCR (polymerase chain reaction). The age of patients ranged from 28 to 78 years. The patients performed the ELISA (enzyme-linked immunosorbent assay) study for the presence of markers of parenteral viral hepatitis (HBsAg, a-HCV and a-HDV), the standard general clinical biochemical blood tests. of the instrumental methods survey used ultrasonography (ultrasound), EGD (fibrogastroduodenoscopy). Grading the severity of liver cirrhosis established by Child-Pugh (eng. Child-Pugh, Child-Turcotte, Child-Turcotte-Pugh, sometimes Child-Paquet) is designed to assess the severity of cirrhosis. The severity of liver cirrhosis is assessed on a point system, which are calculated from 5 or 6 parameters. Results: Results: It is established that most HGD more prevalent among young people bodied (69%) and occurs mainly in severe symptoms and portal hypertension leading to the rapid development of liver cirrhosis (53%). It showed that hyperenzymemia reaches high levels of ALT to 1715 U / L. with a high viral DNA load virus (HBV) 2648226,0 ± 953892,7 copies / ml in the presence of an RNA virus D (HDV +). Conclusion: Conclusion: Thus, the main feature of chronic hepatitis D is its predominant tsirrogennost.

Introduction: By results of a single-step epidemiological research indicators of a lipid range at 355 Evenks of the Olenyoksky District of the Republic of Sakha (Yakutia) are estimated. The aim of this research was assessment of lipid exchange and character of a delivery of Evenks of the item of Zhilinda. Material and methods: 355 evenks aged over 20 years (170 men, women-185). Total cholesterol (OCHS), cholesterol (low-density lipoproteins (LDL-CHOLESTEROL), highdensity lipoprotein cholesterol ((XC-LVNP), triglycerides (TG), the calculated coefficient of haemoglobin (Ka). The measurement is taken at biochemical autoanalyzer Architest 80000 model Systems, firm Abbot (United States) generally accepted method. Food has been studied using questionnaire CYNDI. Results of the Lipid profile compared to the Evenks of Yakut and Russian population of the Republic's population has a high concentration of CS LPHD, low content of TG and haemoglobin index. Food Evenks is characterized by significant disbalance-¼ surveyed every day consume meat and fish every day consumes but 1/5 surveyed. the shortage of basic products is replaced with bread, pasta and sweets. Conclusion: The identified indicators of lipid range Evenks have more healthy than the Yakut. Nutrition Evenks departs from the traditional protein-lipid in carbohydrate-protein.


Distinctive features of systolic function of the left ventricle in patients with chronic obstructive lung disease in extremely cold climate conditions

The purpose of this study was to identify the characteristics of systolic dysfunction and the formation of chronic pulmonary heart disease in patients with COPD living in the central zone of the Republic of Sakha (Yakutia). This article describes the echocardiographic indices of systolic dysfunction of the heart in 229 patients with chronic obstructive pulmonary disease of the Republic of Sakha (Yakutia). According to our data, severe decompensation of chronic pulmonary heart disease with symptoms of heart failure of right ventricular type in most cases combined with severe left ventricle systolic dysfunction, not related to the primary pathology of the left heart and leading to a further decline in the quality of life of patients and increase in the risk of fatal diseases.


Features of Herd Immunity Against Hepatitis B (HBV) in the Territory of Republic of Sakha (Yakutia), Russian Federation

There is new data on the prevalence and characteristics of population immunity against HBV among the indigenous population of the Republic of Sakha (Yakutia) of the Russian Federation. HBV infection in the risk group is significantly higher than in the normal population, in patients with type 2 diabetes, there is a high parenteral infection of HBV, HCV and HBV + HCV (71,5%, 10,8% and 7,5% respectively). It was shown that every other 3- 8 years among children and adults of Yakut population living in the Republic of Sakha (Yakutia) (hyperendemic region on HBV), vaccinated at birth against HBV, the frequency of detection of anti - HBs in protective concentrations (> 10 IU / l and higher) is - 72.2%. Saving anti-HBs in vaccinated for 8 years indicates a high efficacy of vaccines against hepatitis B.

CARDIOVASCULAR THERAPY AND PREVENTION

Aim. To compare clinical effectiveness of the long-term ambulatory therapy (bronchodilators; bronchodilators and fenspiride; bronchodilators, fenspiride and an ACE inhibitor perindopril) among patients with chronic obstructive pulmonary disease (COPD) and chronic cor pulmonale (CCP). Material and methods. In total, 132 patients with COPD and CCP were examined. All participants were divided into three groups, according to the administered therapy (see above), and underwent clinical examination, echocardiography (EchoCG), computed spirography, 6-minute walk test, and other procedures. The examination took place at baseline, as well as 12 and 24 months after the study started. Results. In COPD and CCP patients, long-term therapy with bronchodilators and fenspiride was associated with a significant decrease in bronchial obstruction syndrome severity, a substantial improvement ill lung function (LF) and physical capability, and some improvement in EchoCG parameters, but did not influence mortality and chronic heart failure (CHF) progression. Adding an ACE inhibitor perindopril to the combined therapy resulted in a significant clinical improvement, even among the patients with severe COPD and decompensated CCP. Conclusion. Long-term treatment with a combination of inhaled bronchodilators, fenspiride, and an ACE inhibitor perindopril was highly effective and safe in patients with COPD and CCP, regardless of the disease severity.

Informative value of clinical, electrocardiographic, and echocardiographic diagnostic criteria of compensated chronic pulmonary heart (CPH) in patients with chronic obstructive pulmonary disease (COPD) was considered. The study included 229 patients with COPD of which 105 (group 1) showed no signs of CPH, 71 (group 2) had compensated and 53 (group 3) uncompensated CPH. They were examined by the standard echoCG method using an Acuson-128 HR apparatus (USA) and 12-lead ECG during a 2 year-long follow-up period. Direct cardiac clinical and electrocardiographic signs of right ventricular eccentric hypertrophy were found to have high informative value (100%) but very low sensitivity (7-53%). Indirect diagnostic criteria of compensated CHP are such non-specific signs as age of COPD patients above 50 yr, duration of the disease over 8 yr and broncho-obstructive syndrome (episodes of low-productive cough, dyspnea under small physical load and at rest, X-ray signs of lung emphysema, substantial reduction of FEV and FEV/FVC ratio) in 73-94% of the patients were associated with compensated CPH. The most valuable (75.8-90.5%) cardiographic diagnostic criteria for compensated CPH were MPAP > 22 mmHg (at rest), LVEDD > 24 mm, LVEDS > 17 mm, RVWT = > 5 mm, RAEDD > 32 mm. Criterion LVEDD > 24 mm has the optimal ratio of sensitivity (94.4%), specificity (85.7%), and positive predictive value (86.6%). Comprehensive clinical assessment of COPD character and duration, patients' age, manifestation of broncho-obstructive syndrome and direct clinical signs suggesting involvement of the right half of the heart permits to predict CPH in patients with COPD with a probability of 75%. The definitive diagnosis is verified by echoCG or other instrumental methods.
