Healthcare providers have to maintain records related to medical encounter across multiple stages of individual pregnancy, for medical and legal purposes. Predominantly low-resource healthcare providers use paper-based medical records and tools for documentation and patient monitoring. Obstetric case sheet is a record in standard format documenting past obstetric history, medical and surgical history, etc. during the antenatal period. Partograph is a tool developed by WHO for monitoring of labor wellbeing and progress. The digitalization of these documents and monitoring processes presents various opportunities and challenges. Janitri Innovations has developed a digital labour monitoring tool called "Daksh" for paperless monitoring of labour and documentation of pregnancy cases. Daksh was piloted in seven health care centers of Pavagada district in Karnataka, India. This paper presents critical learnings from the pilot study, using the participatory method. This article has implications for organizations working in the field of maternal healthcare and developing digital solutions for resource-constrained environments.
