Clinical evaluation of drops and gel with provitamin B5 in postoperative treatment of cornea and conjunctiva wounds

Abstract

Vitamins B group are important ingredients of coenzymes. Provitamin B5 dexpanthenol is necessary in the processes of reconstruction of epithelium, has regenerative and anti-inflammatory properties. Objective of the study was to determine how the wounds of cornea and/or conjunctiva heal upon the application of D-panthenol/the medicine has been developed in the Faculty of Pharmacy of the Medical University of Gdańsk/. The activity of the medicine was tested in 40 eyes in the test group, a 40 eyes in the control group did not receive D-panthenol. The degree of the objective and subjective symptoms was assessed. The differences between the two groups commenced on the second day following the operation. Better effects were observed in patients receiving D-panthenol. Congestion and oedema of conjunctiva withdrew, the edges of wounds demonstrated smoothness and better adherence. Subjective feelings improved. 1. Provitamin B5 contained in 5% drops and 5% gel of D-panthenol effectively accelerates the processes of healing the wounds of conjunctiva and cornea.

Purpose: To study the effect of D-panthenol (provitamin B5) on corneal epithelial healing, in cases of surface laser ablation. Patients and Methods: 45 eyes, of 45 patients undergoing laser surface ablation, received D-panthenol 2% in one eye and artificial tear drops of similar composition not containing D-panthenol in the other eye, postoperatively, for 2 months. Patients were examined daily for 3 days after the procedure. They were then examined weekly for 1 month. An additional examination was made after 2 months. Visual acuity (Log MAR) was assessed at every visit. Rate of healing (% of covered area) and subjective sensation of discomfort (scale 0-5) were assessed in the 1st 3 visits. Residual manifest cylinder (D) (as a parameter of corneal irregularity) and corneal clarity (epithelial and stromal haze) were assessed from week 1 to month 2. Results: During the first 3 days, both groups showed statistically nonsignificant (P > 0.05) results. From week 1 to month 2, eyes receiving D-panthenol showed better vision and less residual cylinder (P < 0.05) at week 1. For all other parameters, and at different examinations, both groups showed a statistically nonsignificant (P > 0.05) difference. Still, eyes receiving D-panthenol showed better values at the majority of the parameters tested. Conclusion: D-Panthenol effect on corneal epithelial regeneration is of minimal clinical relevance. A different dosage and a larger sample of patients might reveal a statistical relevance. This trial is registered with https://doi.org/10.1186/ISRCTN81441126.

Results: There was no statistically significant difference between the groups in respect of pain, stinging and watering on the 1st, 3rd and 7th days. A statistically significant decrease in these complaints was observed on days 3 and 7 in the group using dexpanthenol. There was no statistically significant difference between the groups on days 1, 3 and 7in respect of redness and graft edema. In both groups, a statistically significant increase was determined in these findings on days 3 and 7. Conclusion: After pterygium surgery, many agents increase the comfort of patients. Autologous serum is the most reliable and most investigated agent. Dexpanthenolmay be an alternative to anterior segment surgery in respect of patient comfort and wound healing. In 2017 by the results of TFOS DEWS, dry eye syndrome (DES) was recognized as a disease that requires treatment. The leading role in DES development is given to inflammation and defects of an eye surface, instability and hyperosmolarity of tear film and neurosensory disorders. While choosing the tear substitute for the therapy of DES the focus is on an involvement of various components of DES pathogenesis: disbalance of a tear film structure and its’ disfunction, inflammation and defects of eye tissues. Purposes of treatment are restoration of tear homeostasis, breaking of a “vicious circle” of DES and achievement of a prolonged effect of the therapy, including the subjective one. Taking into consideration that inflammation is associated with most of ophthalmological diseases causing DES, it is a clear choice to prescribe tear substitutes with anti-inflammatory, wetting and regenerative properties. Treatment with combined tear substitutes affecting different components of DES pathogenesis simplifies the instillation regimen and increasing compliance.

The CREST syndrome is characterized by calcinosis, Raynaud's syndrome, esophageal dysmotility, sclerodactyly, and telangiectasia. It has signs and symptoms in the stomatognathic system that deserve consideration in its treatment. The objective of this paper is to report the oral manifestations of a case of CREST syndrome and describe the prosthetic treatment performed in the Department of Oral Rehabilitation in the National University of Colombia. The case is presented of a patient female aged 42 years, diagnosed with CREST syndrome by the treating doctor. She came to the Faculty of Dentistry for restorative treatment. The prosthodontic management included metal-ceramic crowns, fixed partial denture, and removable partial denture. The results demonstrate that conventional restorative alternatives, similar to that used in a patient without this systemic condition, while maintaining strict control of it, can be recommended.

The spectrum of cold injuries ranges from frostnip, chilblains to severe frostbite. Cold injuries occur upon prolonged exposure to freezing temperature and are pathologically a combination of ice crystal formation in the tissue resulting in inflammation, thrombosis and ischemia in the extremities, often necessitating limb amputation in extreme cases due to tissue necrosis. Severe forms of frostbite are a cause of major concern to patients as well as the treating physician. Due to the lack of effective treatment modalities and paucity of research on prophylaxis and therapeutics of cold injuries, we developed a novel herbosomal-loaded PEG–poloxamer topical formulation (n-HPTF) employing quality-by-design (QBD) approach. Natural compounds exhibiting potent therapeutic potential for the management of cold injuries were incorporated in novel lipid vesicles (herbosomes) loaded in PEG–poloxamer polymers. The herbosomal formulation effectively creates an occlusion barrier that promotes epithelial regeneration, desmosome scale-up and angiogenesis and thus promotes rapid healing, indicating controlled release of herbosomes. Optimized novel herbosomes showed entrapment efficiency > 90% and < 300 nm mean particle size and in vitro drug permeation of about 2 µg/cm2 followed Higuchi’s release kinetics. Skin irritancy study on female Sprague–Dawley rats showed no edema or erythema. In vivo bio-efficacy study revealed significant efficacy (p < 0.05) when compared to the standard treatment groups. To determine the efficacy of dexpanthenol applied early after urethral trauma for preventing inflammation and spongiofibrosis. Twenty-seven rats were randomized and divided into 3 groups, with 9 rats in each group. The urethras of all rats were traumatized with a pediatric urethrotome knife at 6-o' clock. For 14 days, group I was given 0.9% saline twice a day (control group), group II was given dexpanthenol 500 mg/kg ampules once a day and 0.9% saline once a day, and group III was given dexpanthenol 500 mg/kg ampules twice a day intraurethrally using a 22 ga catheter sheath. On day 15, the penises of the rats were degloved to perform penectomy. The mean fibrosis scores were 2.4, 2.2, and 1.4, and mean inflammation scar scores were 2, 1.4, and 1.3 in groups I, II, and III, respectively. There was a significant difference between groups I and II for inflammation (P = .011); however, the difference for fibrosis was not significant (P = .331). The differences between groups I and III were statistically significantly different both for inflammation and fibrosis (P = .004 and P = .003, respectively). Groups II and III were not different significantly for inflammation (P = .638); however, there was less fibrosis in group III, in which high-dose dexpanthenol was administered. We showed that dexpanthenol applied early after urethral trauma significantly decreased inflammation and spongiofibrosis. We hope that our study will help to decrease strictures after urethral trauma and contribute to pharmaceutical investigations aiming to improve the success of the surgery for urethral strictures. Functional constipation is not a life-threatening disease, but as a chronic state it worries the patient and causes him discomfort and often leads him to self-medication with potentially dangerous drugs. Ro 01-4709 contains as active substance dexpanthenol, which is the alcohol of pantothenic acid, a vitamin of the B-complex. In the cells, dexpanthenol is readily oxidized to pantothenic acid, which stimulates peristalsis when administered in therapeutically effective doses. Ro 01-4709 has already proven its efficacy in the prevention and treatment of adynamic ileus. Recently, several open and two double-blind studies have been carried out, investigating the efficacy of oral Ro 01-4709 in the treatment of chronic functional constipation. The two double-blind studies showed Ro 01-4709 to be superior to placebo in all parameters measured. The studies with an open design also demonstrated a favourable effect of Ro 01-4709 in the treatment of chronic functional constipation. Owing to its physiological action-which is in a favourable contrast to that of normal laxatives. Ro 01-4709 can be recommended for the treatment of functional constipation in pregnant women, children and the elderly.

In this controlled, randomized, double-masked study the effect of dexpanthenol-containing artificial tears (Siccaprotect) on patients with dry eyes was examined. 50 patients applied either dexpanthenol-containing artificial tears (Siccaprotect) or the identical, but free of dexpanthenol, eye drops five times daily into the conjunctival sac. No other ophthalmics were administered. The corneal epithelial permeability was measured by fluorophotometry and Schirmer-Test, Rose Bengal staining, tear film break-up time and the patients' subjective complaints were determined before and after 6 weeks of treatment. The dexpanthenol-containing artificial tears (Siccaprotect) improved disturbances of the corneal epithelial permeability significantly in comparison to the dexpanthenol-free eyes drops. The other parameters didn't show relevant differences. These data suggest that, in dry eyes, treatment with dexpanthenol-containing eye drops leads to a favorable and comparing with dexpanthenol-free eye drops superior improvement in disturbances of corneal epithelium permeability.
