Right-to-Left-Shunts Detected by Transesophageal Echocardiography and Transcranial Doppler Sonography

Transesophageal echocardiography (TEE) and transcranial Doppler sonography (TCD) can identify right-to-left-shunts that predispose to paradoxical embolism. In a large cohort we compared the results of both techniques.
222 patients were investigated by both techniques using the contrast agent Echovist-300 and performing each test once without and once with the Valsalva maneuver (VM).
TEE-proven right-to-left-shunts were detected by TCD with a high sensitivity of 94%. In addition, TCD revealed shunts not noted during TEE in about 20% of all patients studied. These shunts are in general smaller than those concordantly identified; however, 9% of the patients without a TEE-proven shunt presented with a shunt that allows a considerable amount of contrast medium to pass. There were 12% more microbubbles detected in the right middle cerebral artery than in the left middle cerebral artery during the TCD test performed with VM, but not during the TCD test performed without VM.
Contrast-enhanced TEE and TCD are complementary methods in the assessment of stroke and stroke-prone patients. The side difference of microbubbles may indicate a selective streaming of cardiac emboli during VM.

    ABSTRACT: OBJETIVO: Avaliar a freqüência de forame oval patente (FOP) em pacientes com acidente cerebrovascular isquêmico (AVCI) criptogênico ou de causa definida, investigados com doppler transcraniano (DTC) e ecocardiograma transesofágico. Paralelamente, examinar a validade diagnóstica do primeiro, comparado ao segundo método. MÉTODO: Retrospectivamente, foram estudados 124 pacientes (< 51 anos), vítimas de AVCI, submetidos a DTC e ou ecocardiograma transesofágico. Os pacientes foram classificados em dois grupos: com ou sem AVCI criptogênico. RESULTADOS: Encontramos importante associação entre AVCI criptogênico e FOP (razão de chance de 4,3; IC95% 1,7 -10,7). Foram diagnosticados apenas 5 casos de aneurisma do septo interatrial entre aqueles com FOP, todos classificados como AVCI criptogênico. Tanto a sensibilidade, especificidade e os valores preditivos positivo e negativo exibiram valores superiores a 85%, semelhantes, pelo menos com base nos intervalos de confiança. CONCLUSÃO: Constatamos, pela primeira vez em nosso meio, forte associação entre AVCI criptogênico e FOP. O DTC é importante recurso diagnóstico nesse contexto, já que sua validade foi considerada muito boa, podendo ser útil no rastreamento de fontes emboligênicas, particularmente alterações do septo atrial. Uma investigação minuciosa desses casos se impõe, notadamente pela perspectiva de fechamento do shunt intracardíaco.

    ABSTRACT: Transcranial Doppler (TCD) is currently used in monitoring subarachnoid hemorrhage, acute stroke, and carotid endarterectomy and in diagnosing intracranial vascular disease. In this article we review other uses of TCD, in systemic conditions related to ischemic stroke. Transcranial Doppler, in fact, can identify right-to-left shunts (RLS), usually due to patent foramen ovale (PFO), a risk factor for cryptogenic stroke in the young. Recently an association between RLS and migraine with aura has been reported; migraine is a risk factor for stroke, for silent brain infarctions, and for silent white matter disease, but there are no data yet on a similar risk for the association of migraine plus PFO. TCD can also be used to stratify the risk of stroke in children with sickle cell disease, to better identify subjects to be transfused to prevent strokes.
