Assessment of the Efficacy of Nonablative Long-Pulsed 1064-nm Nd:YAG Laser Treatment of Wrinkles Compared at 2, 4, and 6 Months
Abstract
Rhytides represent an aesthetic problem for a large percentage of the population. Many methods, both noninvasive and invasive, have been used for the treatment of wrinkles. Recently, the long-pulsed 1064-nm Nd:YAG laser has been shown to enhance dermal collagen synthesis without damaging the epidermis. The purpose of this preliminary study is to evaluate the use of the long-pulsed Nd:YAG laser in the nonablative treatment of periocular and perioral wrinkles. Ten patients with facial wrinkles were treated with the long-pulsed 1064-nm Nd:YAG laser, at a spot size of 5 mm in diameter, energy density of 13 J/cm2, exposure time per pulse of 300 microseconds, and a repetition rate of 7 Hz. All patients had a total of three treatments, once every 2 weeks. Subjective (patient satisfaction index [SI]) and objective (both physician- and computer program-based clinical index [CI]) assessments were performed before the first and third treatment sessions, and at 2, 4, and 6 months after the last treatment. At 6 months after the final treatment session, the patients' subjective SI was maintained at 40%, and had peaked at 50% 2 months after the final session. Physician assessment showed a CI of 40% at the 6-month assessment point and the computer program showed a 50% CI. The greatest level of effect with long-pulsed Nd:YAG laser nonablative skin rejuvenation for facial wrinkles was seen 2 months after the final treatment. Effects were still visible at the 6-month period, but showed a tendency to decrease. Maintenance treatments are required to achieve good patient satisfaction.
El ultrasonido de alta frecuencia es una herramienta no invasiva que ofrece marcadores que cuantifican los cambios cutáneos del proceso de senescencia fisiológica. Entre ellos, la epidermis, dermis y la banda subepidérmica de baja ecogenicidad (SLEB), son variables cuyo espesor y ecogenicidad cambian con el envejecimiento cutáneo. Este estudio tuvo como objetivo estimar el espesor epidérmico, dérmico y SLEB en seis sitios faciales diferentes utilizando ultrasonido de alta frecuencia a través de un protocolo en pacientes candidatos a tratamiento de rejuvenecimiento facial con láser no ablativo. Se tomaron imágenes ultrasonográficas empleando ecógrafo de alta resolución de 16 MHz en seis puntos faciales: región frontal, temporal, surcos nasogenianos y mentón de dos pacientes. Entre los hallazgos ecográficos más relevantes luego de aplicar el tratamiento, se reportaron después de los 90 días de iniciada la terapia y fueron un aumento del grosor de la dermis, así como una homogeneización de la ecogenicidad de la epidermis, dermis y SLEB. Asimismo, se observa cómo estos cambios dérmicos son reflejados en la mejora de la calidad de la piel en cuanto a arrugas, presencia de poros y pápulas o comedones en la piel. En conclusión, el ultrasonido de alta frecuencia es una herramienta no invasiva que puede evaluar la estructura cutánea y los cambios dérmicos relacionados con la edad, que ofrece in vivo marcadores imagenológicos en el seguimiento de intervenciones para rejuvenecimiento facial.
During pregnancy, endocrinological, immunological, metabolic, and vascular changes occur. The use of short-pulsed or flash-scanned CO2 lasers to resurface skin has rapidly joined chemical peels and dermabrasion as an accepted procedure. The purpose of this study was to evaluate a mid-infrared pulsed Erbium: YAG laser prospectively to determine its clinical efficacy for resurfacing of the face, neck, and hands. Postoperative changes and recovery period were also evaluated. A total of 21 patients were evaluated on a prospective basis with Er: YAG laser resurfacing (12 crow's feet, five upper lips, three hands, and one neck). Additional nonstudy clinical experience is also reported.
Posttreatment crusting or scabbing lasted an average of 2.7 days, pain an average of 3 days, erythema an average of 5.2 days, and swelling an average of 3 days. Blinded subjective grading was performed 2 months postoperatively. This grading revealed a 52% combined improvement from all areas. The appearance of crow's feet was improved by 58%; upper lip, 43%; dorsal hand, 48%; and neck, 44%.
Overall the Er: YAG laser consistently produced reduction in rhytids and improvement in the appearance of sun-damaged skin. The times for reepithelialization and duration of erythema were strikingly shorter than those typically observed with current CO2 laser resurfacing. This report details the study, reviews Er: YAG laser technology, and compares our findings with those observed with standard CO2 laser resurfacing.
Bidimensional shape detection is a process with high computational complexity. In this work, an algorithm, based on the generalized Hough transform (GHT), is presented in order to calculate the orientation, scale, and displacement of a image shape with respect to a template. To reduce the complexity, the uncoupled of the parameter calculation is carried out. The generation of the invariant information needed by the uncoupled is implemented by using three transformation functions that pair shape edge points. Differences between gradient vector angles are used to choose the paired points. An “a priori” study of the template shape is carried out to select the most suitable values for the difference angles.
Noninvasive techniques for skin rejuvenation are quickly becoming standard in the treatment of mild rhytids and overall skin toning. Multiple laser wavelengths and modalities have been used with varying degrees of success, including 532-nm, 585-nm, 1064-nm, 1320-nm, 1450-nm, and 1540-nm wavelengths.

Private dermatologic surgery and laser practice.
A total of 150 patients, with skin types I through V, were treated with long-pulsed KTP 532-nm and long-pulsed Nd:YAG 1064-nm lasers, separately and combined. For the KTP 532-nm laser, the fluences varied between 7 to 15 J/cm2 at 7- to 20-millisecond pulse durations with a 2-mm handpiece and 6 to 15 J/cm2 at 30- to 50-millisecond pulses with a 4-mm handpiece. The 1064-nm Nd:YAG laser fluences were set at 24 to 30 J/cm2 for a 10-mm handpiece. These energies were delivered at 30- to 65-millisecond pulse durations. All subjects were treated at least 3 times and at most 6 times, depending on patient satisfaction level, at monthly intervals and were observed for up to 18 months after the last treatment.
All patients were asked to fill out a "severity scale" on which redness, pigmentation, rhytids, skin tone/tightness, texture, and patient satisfaction were noted before and after each treatment. Redness, pigmentation, rhytids, skin tone/tightness, and texture were also evaluated by the physician and another observer.
After 3 to 6 treatments, 50 patients treated with the 532-nm KTP laser alone showed improvement of 70% to 80% in redness and pigmentation, 30% to 50% in skin tone/tightening, 30% to 40% in skin texture, and 20% to 30% in rhytids. Another 50 patients treated with the 1064-nm Nd:YAG laser alone showed improvement of 10% to 20% in redness, 0% to 10% in pigmentation, 10% to 30% in skin tone/tightening, 20% to 30% in skin texture, and 10% to 30% in rhytids. The third group of 50 patients treated with both KTP and Nd:YAG lasers showed improvement of 70% to 80% in redness and pigmentation, 40% to 60% in skin tone/tightening, 40% to 60% in skin texture, and 30% to 40% in rhytids. Skin biopsy specimens taken at 1-, 2-, 3-, and 6-month intervals demonstrated new collagen formation.
All 150 patients exhibited mild to moderate improvement in the appearance of rhytids, moderate improvement in skin toning and texture, and great improvement in the reduction of redness and pigmentation. The KTP laser used alone produced results superior to those of the Nd:YAG laser. Results from combination treatment with both KTP and Nd:YAG lasers were slightly superior to those achieved with either laser alone.
Carbon dioxide laser resurfacing has recently come into favor for the treatment of photodamaged skin. While the clinical and histologic effects of high-energy short-pulse carbon dioxide lasers on human skin have been investigated, the ultrastructural effects of these lasers have not been documented. Our objective was to study the ultrastructural effects of a high-energy pulsed carbon dioxide laser on photodamaged human skin.
Before laser surgery, the ultrastructural changes characteristic of photodamaged skin were evident. Immediately after treatment, there was extensive coagulation necrosis of the epidermis and papillary dermis. Thirty days after treatment, there was no evidence of intercellular or intracellular edema, and ordered differentiation of the epidermal keratinocytes, with a loss of keratinocyte dysplasia, was seen. Increased numbers of desmosomes and tonofibrils were noted. New deposition of collagen was present in the papillary dermis. The ultrastructural findings seen at 90 days after treatment were similar to those seen at 30 days, apart from increased organization of collagen fibers in the papillary dermis.
Treatment with the high-energy pulsed carbon dioxide laser appears to reverse the epidermal and dermal changes of photoaging on an ultrastructural level. These changes appear morphologically to be consistent with previously described clinical and histologic changes following laser resurfacing.
The efficacy of a new dual wavelength Er:YAG and CO2 laser system was tested for skin resurfacing results on rabbits' ears and human facial skin. The dual wavelength laser delivers simultaneous pulses of low-energy CO2 and high-energy Er:YAG energies. Theoretically, combining the strengths of both laser types in one console should lead to improved clinical outcome.
The use of the laser for skin resurfacing and remodeling has dramatically increased over the past few years. The CO2 laser was the first laser to be used in this field, followed more recently by the E:YAG laser. Both lasers offer unique advantages and disadvantages.
The present study consists of an ear chamber experiment, conducted on eight rabbits, to examine vascular network formation, after laser resurfacing with a standard CO2 laser and the Er:YAG/CO2 laser. Resurfacing was also performed on human patients with the Er:YAG/CO2 laser and the results were compared with previously published results of CO2 laser resurfacing.
Significant advantages, including an attenuation in the degree of edema and erythema and a shorter reepithelialization time compared to results with conventional CO2 systems, were observed with the Er:YAG/CO2 system. Improved tissue reorganization and good clinical results in nine of the ten patients (six ratings of "very good" and three "good") were observed. The clinical outcome of the remaining patient was rated as "fair." Minimal side effects were reported and observed in only three patients. The learning curve required to maximize the efficiency of the system is steep, however, requiring a thorough understanding of the different laser/tissue interactions associated with the two wavelengths.
It is the authors' opinion that the dual wavelength Er:YAG/CO2 laser system offers a particularly efficient and flexible system to perform standard CO2 procedures for skin resurfacing with an improved clinical outcome, as well as other applications with the CO2 or Er:YAG energy delivered separately.
