Disparities in Maternal Outcomes among Four Ethnic Populations

ABSTRACT

To examine racial disparities in adverse maternal outcomes among four ethnic groups in the United States.
A total of 1,030,350 women aged 13 to 55 who delivered in 1998 and 1999. Data were abstracted from the National Inpatient Sample.
Race was categorized as White, African-American, Hispanic, and Asian/Pacific Islander. Maternal outcome measures included preterm labor, hypertensive disorders of pregnancy, gestational diabetes, antepartum hemorrhage, membrane disorders, cesarean section, and postpartum hemorrhage.
African Americans were more likely to have preterm labor (odds ratio [OR] 1.71), preeclampsia (OR 1.59), transient hypertension of pregnancy (THP) (OR 1.13), pregnancy-induced hypertension (PIH) (OR 1.38), diabetes (OR 1.26), placenta previa (OR 1.78), placental abruption (OR 1.52), premature rupture of membranes (PRM) (OR 1.19), infection of the amniotic cavity (IAC) (OR 1.95), and cesarean section (risk ratio [RR] 1.08); Hispanics were more likely to have diabetes (OR 1.44), placenta previa (OR 1.20), IAC (OR 1.15), and cesarean section (RR 1.06); and Asian/Pacific Islanders were more likely to have diabetes (OR 2.05), placenta previa (OR 1.57), PRM (OR 1.26), IAC (1.79), and postpartum hemorrhage (OR 1.19).
African Americans had the worst outcomes when adjusted for sociodemographic characteristics and comorbidities. Infection, gestational diabetes, and control of hypertension, especially among African Americans, are the most significant preventable risk factors contributing to disparities. By increasing access to health services, health education, and appropriate perinatal care in minority populations, improvements are likely to occur across multiple measures including frequencies of preterm labor, cesarean section, and gestational diabetes.

    ABSTRACT: This hypothesis proposes pre-eclampsia is caused by intra-abdominal hypertension in pregnancy. Sustained or increasing intra-abdominal pressure >12mmHg causes impaired venous return to the heart, systemic vascular resistance, ischemia reperfusion injury, intestinal permeability, translocation of lipopolysaccharide endotoxin to the liver, cytotoxic immune response, systemic inflammatory response, pressure transmission to thoracic and intra-cranial compartments, and multi-organ dysfunction. This hypothesis is predicated on Pascal's law, evidence founded in the intra-abdominal hypertension literature, and the adapted equation ΔIAP-P=ΔIAVF/Cab, where ΔIAP-P=change in intra-abdominal pressure in pregnancy, ΔIAVF=change in intra-abdominal vector force (volume and force direction) and Cab=abdominal compliance. Factors causing increased intra-abdominal pressure in pregnancy include: progressive uterine expansion, obstetrical factors that increase intra-uterine volume excessively or acutely, maternal anthropometric measurements that affect intra-abdominal pressure thresholds, maternal postures that increase abdominal force direction, abdominal compliance that is decreased, diminished with advancing gestation, or has reached maximum expansion, habitation at high altitude, and rapid drops in barometric pressure. We postulate that the threshold for lipopolysaccharide translocation depends on the magnitude of intra-abdominal pressure, the intestinal microbiome complex, and the degree of intestinal permeability. We advance that delivery cures pre-eclampsia through the mechanism of abdominal decompression.

    ABSTRACT: Purpose To examine racial differences in gestational weight gain (GWG) and pregnancy-related hypertension. Methods Logistic regression models tested racial differences in adequacy of GWG and pregnancy-induced hypertension in all singleton live births from the South Carolina 2004-2006 birth certificates. Results Compared to white women, black and Hispanic women had 16%-46% lower odds of gaining weight above the recommendations. However, the odds of inadequate GWG was ∼50% higher in black and Hispanic women with a pregnancy body mass index (BMI) <25kg/m2. Furthermore, compared to women with adequate GWG, women with excessive GWG had higher odds of pregnancy-related hypertension (underweight: 2.35, 95% CI(1.66, 3.32); normal: 2.05, 95% CI(1.84, 2.27); overweight: 1.93, 95% CI(1.64, 2.27); obese: 1.46, 95% CI(1.30, 1.63)). Among women with a BMI <25 kg/m2, black women had higher odds of pregnancy-related hypertension than white women (underweight: 1.64, 95% CI(1.14, 2.36); normal weight: 1.28, 95% CI(1.15, 1.42)), while among women with a BMI ≥25 kg/m2, Hispanic women had 40% lower odds. Conclusion Programs are needed to curb excessive GWG in all racial groups and to help some sub-groups ensure adequate GWG. Maternal obesity and GWG are two factors that should be used in combination to reduce racial differences in pregnancy-related hypertension.

    ABSTRACT: Background Asians are at increased risk of morbidity at a lower body mass index (BMI) than European Whites, particularly relating to metabolic risk. UK maternal obesity guidelines use general population BMI criteria to define obesity, which do not represent the risk of morbidity among Asian populations. This study compares incidence of first trimester obesity using Asian-specific and general population BMI criteria. Method A retrospective epidemiological study of 502,474 births between 1995 and 2007, from 34 maternity units across England. Data analyses included a comparison of trends over time between ethnic groups using Asian-specific and general population BMI criteria. Logistic regression estimated odds ratios for first trimester obesity among ethnic groups following adjustment for population demographics. Results Black and South Asian women have a higher incidence of first trimester obesity compared with White women. This is most pronounced for Pakistani women following adjustment for population structure (OR 2.19, 95% C.I. 2.08, 2.31). There is a twofold increase in the proportion of South Asian women classified as obese when using the Asian-specific BMI criteria rather than general population BMI criteria. The incidence of obesity among Black women is increasing at the most rapid rate over time (p=0.01). Conclusion The twofold increase in maternal obesity among South Asians when using Asian-specific BMI criteria highlights inequalities among pregnant women. A large proportion of South Asian women are potentially being wrongly assigned to low risk care using current UK guidelines to classify obesity and determine care requirements. Further research is required to identify if there is any improvement in pregnancy outcomes if Asian-specific BMI criteria are utilised in the clinical management of maternal obesity to ensure the best quality of care is provided for women irrespective of ethnicity.
