Long-term antibiotics for preventing recurrent urinary tract infection in children

Bladder and kidney infections (urinary tract infection - UTI) are common in children, especially girls. They cause an uncomfortable illness that can include vomiting, fever and tiredness. In some children kidney damage may occur, as can repeat illnesses. With repeated infections the risk of kidney damage increases. Some doctors prescribe long-term antibiotics to try to prevent infections recurring, but this may cause the child to be unwell in other ways, e.g. vomiting. This review of trials found evidence that long-term antibiotics did prevent some infections, but these infections occurred without the child being unwell, that is they may not be real illnesses and thus don't need prevention. Nitrofurantoin was more effective than trimethoprim but had more adverse effects.

    ABSTRACT: There are many ongoing controversies surrounding vesicoureteral reflux (VUR). These include variable aspects of this common congenital anomaly. Lack of evidence-based recommendations has prolonged the debate. Systematic reviews (SRs) and meta-analysis (MA) are considered high-level evidence. The purpose of this review article is to summarize and critically appraise the available SR/MA pertaining to VUR. We also discuss the strength and pitfalls of SR/MA in general. A thorough literature search identified 9 SRs/MAs relevant to VUR. Both authors critically reviewed these articles for contents and methodological issues. There are many concerns about the quality of the studies included in these SRs. Clinical heterogeneity stemming from different patient selection criteria, interventions, and outcome definitions is a major issue. In spite of major advances in understanding different aspects of VUR in the last few decades, there is a paucity of randomized controlled trials in this field.

    ABSTRACT: Resumen Se presentan las recomendaciones actualizadas para el manejo diagnóstico y terapéutico de las infecciones del tracto urinario en la infancia. Se revisan las técnicas de recogida de orina más apropiadas en función de la capacidad del niño para controlar la micción y la urgencia del diagnóstico. Se ofrecen directrices para la interpretación de los distintos parámetros del perfil urinario, recomendando sustentar el diagnóstico de infección urinaria en el cultivo de orina. La indicación de ingreso hospitalario o de tratamiento intravenoso en la infección urinaria no depende de la localización de la infección urinaria, sino del grado de repercusión y el riesgo de complicaciones del paciente. Se establecen pautas de tratamiento antibiótico de primera elección y alternativas para su administración parenteral u oral, así como recomendaciones para la elección de la vía, el ritmo y la duración del tratamiento, en función del nivel de riesgo del paciente. No se recomienda la profilaxis rutinaria con antibióticos, tras un primer episodio de infección urinaria, en niños con tracto urinario normal o en niños con reflujo vésico-ureteral de bajo grado; la indicación de profilaxis en pacientes con infecciones recurrentes o con reflujos de alto grado debe ser individualizada. Se recomienda cambiar la actual estrategia de uso rutinario de pruebas de imagen por otra en la que sólo se indiquen de forma individualizada, considerando el nivel de riesgo de cada paciente.

    ABSTRACT: Urine tract infections with or without vesicoureteral reflux (VUR) is a common disease in children. Although antibiotic prophylaxis is internationally accepted as a therapy for these children, there is no evidence supporting a protective effect. Recently published studies doubt the role of antibiotic prophylaxis in prevention of recurrent urine tract infections (UTI) in children with VUR. Although there is strong heterogeneity in methodology of the included studies, most results indicate a lack of protective effect by prophylaxis in these children. Furthermore, antibiotic prophylaxis is not protective in preventing long term renal damage. Children using prophylaxis can develop recurrent UTI caused by multi-resistant bacteria.
