Tailored interventions to promote mammography screening: A meta-analytic review

To evaluate the effectiveness of tailored interventions, designed to reach one specific person based on her unique characteristics, for promoting mammography use.
This systematic review used meta-analytic techniques to aggregate the effect size of 28 studies published from 1997 through 2005. Potential study-level moderators of outcomes (sample, intervention, and methodological characteristics) were also examined.
A small but significant aggregate odds ratio effect size of 1.42 indicated that women exposed to tailored interventions were significantly more likely to get a mammogram (p<0.001). The type of population recruited and participants' pre-intervention level of mammography adherence did not significantly influence this effect. Tailored interventions that used the Health Belief Model and included a physician recommendation produced the strongest effects. Interventions delivered in person, by telephone, or in print were similarly effective. Finally, defining adherence as a single recent mammogram as opposed to regular or repeated mammograms yielded higher effect sizes.
Tailored interventions, particularly those that employ the Health Belief Model and use a physician recommendation, are effective in promoting mammography screening. Future investigations should strive to use more standardized definitions of tailoring and assessments of mammography outcomes.

    ABSTRACT: Sleep restriction is a prevalent issue for adolescents and has been associated with negative cognitive, emotional, and physical health (e.g., poor attention, depressed mood, obesity). Existing sleep promotion programs are successful in improving adolescents’ sleep knowledge but not sleep behaviour. The aim of this randomized controlled trial is to evaluate the effectiveness of Motivating Teens to Sleep More program – a sleep promotion program with embedded sleep education that combines three approaches: motivational interviewing style, tailoring activities, and stage-based intervention – as compared to a sleep education only control in motivating adolescents to go to bed earlier leading to prolonged sleep duration. The Motivating Teens to Sleep More study will be conducted with adolescents at a Montreal high school. Half of the participants will be randomly assigned to the Motivating Teens to Sleep More program condition and the other half to the sleep education control condition. Each condition will consist of four 1-hour sessions spanning four consecutive weeks. Bedtime will be assessed by sleep logs completed for a week prior to the start of the program, in the middle of the program and following the program. Sleep onset and total sleep time will be assessed by actigraphy for one week prior to the start and following the program. The Motivating Teens to Sleep More program is a novel intervention that contributes theoretically to the field of pediatric sleep by merging three approaches to motivate normally developing adolescents to adopt earlier bedtimes. Should the program be successful in advancing bedtimes and increasing total sleep time, the study would offer insights in how to design effective motivational sleep promotion programs for adolescents, which can potentially improve adolescent health and well-being.     ABSTRACT: Men have a significantly increased risk of being diagnosed with, and dying from, colorectal cancer (CRC) than women. Men also participate in fecal occult blood test (FOBT) screening at a lower rate than women. This study will determine whether strategies that target men's attitudes toward screening, and matched to stage of readiness to screen, increase men's FOBT participation compared to a standard approach. Eligible trial participants will be a national sample of 9,200 men aged 50 to 74 years, living in urban Australia and randomly selected from the Australian electoral roll. Trial participants will be mailed an advance notification letter, followed 2 weeks later by an invitation letter and a free fecal immunochemical test (FIT) kit. The intervention is a factorial design, randomized controlled trial (RCT) with four trial arms, including a control. The content of the advance notification and invitation letters will differ by trial arm as follows: 1) standard advance notification and standard invitation (control arm); 2) targeted advance notification and standard invitation; 3) standard advance notification and targeted invitation; and 4) targeted advance notification and targeted invitation. The standard letters will replicate as closely as possible the letters included in the Australian National Bowel Cancer Screening Program (NBCSP). Modified advance notification and invitation letters will incorporate additional messages to target men in the precontemplation (advance notification) and contemplation stages (invitation). The primary outcome is return of the completed FIT within 12 weeks of invitation. Analysts will be blinded to trial assignment and participants will be blinded to the use of varying invitational materials. Subsamples from each trial arm will complete baseline and endpoint surveys to measure the psychological impact of the intervention, and qualitative interviews will be conducted to evaluate attitudes toward the intervention. The outcomes of this study will have implications for the way FOBT screening is offered to men. Findings will help to identify how invitations for men to screen should be framed and delivered in order to maximize participation.Trial registrationAustralian New Zealand Clinical Trials Registry: ACTRN12612001122842.
