Redesign of a Computerized Clinical Reminder for Colorectal Cancer Screening: A Human–Computer Interaction Evaluation

Based on barriers to the use of computerized clinical decision support (CDS) learned in an earlier field study, we prototyped design enhancements to the Veterans Health Administration's (VHA's) colorectal cancer (CRC) screening clinical reminder to compare against the VHA's current CRC reminder.
In a controlled simulation experiment, 12 primary care providers (PCPs) used prototypes of the current and redesigned CRC screening reminder in a within-subject comparison. Quantitative measurements were based on a usability survey, workload assessment instrument, and workflow integration survey. We also collected qualitative data on both designs.
Design enhancements to the VHA's existing CRC screening clinical reminder positively impacted aspects of usability and workflow integration but not workload. The qualitative analysis revealed broad support across participants for the design enhancements with specific suggestions for improving the reminder further.
This study demonstrates the value of a human-computer interaction evaluation in informing the redesign of information tools to foster uptake, integration into workflow, and use in clinical practice.

    ABSTRACT: Colorectal cancer is an important public health problem in Spain. Over the last decade, several regions have carried out screening programmes, but population participation rates remain below recommended European goals. Reminders on electronic medical records have been identified as a low-cost and high-reach strategy to increase participation. Further knowledge is needed about their effect in a population-based screening programme. The main aim of this study is to evaluate the effectiveness of an electronic reminder to promote the participation in a population-based colorectal cancer screening programme. Secondary aims are to learn population's reasons for refusing to take part in the screening programme and to find out the health professionals' opinion about the official programme implementation and on the new computerised tool. This is a parallel randomised trial with a cross-sectional second stage. Participants: all the invited subjects to participate in the public colorectal cancer screening programme that includes men and women aged between 50-69, allocated to the eleven primary care centres of the study and all their health professionals. The randomisation unit will be the primary care physician. The intervention will consist of activating an electronic reminder, in the patient's electronic medical record, in order to promote colorectal cancer screening, during a synchronous medical appointment, throughout the year that the intervention takes place. A comparison of the screening rates will then take place, using the faecal occult blood test of the patients from the control and the intervention groups. We will also take a questionnaire to know the opinions of the health professionals. The main outcome is the screening status at the end of the study. Data will be analysed with an intention-to-treat approach. We expect that the introduction of specific reminders in electronic medical records, as a tool to facilitate and encourage direct referral by physicians and nurse practitioners to perform colorectal cancer screening will mean an increase in participation of the target population. The introduction of this new software tool will have good acceptance and increase compliance with recommendations from health professionals.Trial registration: Clinical identifier NCT01877018.

    ABSTRACT: In this paper we describe the development of a standardized computer satisfaction usability questionnaire for use with speakers of the Turkish language, the Turkish Computer System Usability Questionnaire, Short Version (T-CSUQ-SV). This new questionnaire, based on the English-language CSUQ, underwent careful translation and transformation through comprehensive psychometric evaluation. The results of the psychometric evaluation revealed an acceptable level of reliability, appropriate construct validity, and sensitivity to manipulation, indicating that Turkish usability practitioners should be able to use the T-CSUQ-SV with confidence when conducting user research.

    ABSTRACT: Biomedical applications are often built on top of knowledge bases that contain medical images and clinical reports. Currently, these bases are being used to improve diagnosis, research and teaching, but in many cases, the infrastructure required has a prohibitive cost for many medical centres. However, resources can be attached from existing e-Science infrastructures. Therefore, many efforts have been made to establish best practices that allow the use of such infrastructures. However, e-Science relies on open, distributed, collaborative environments, built on top of very specialized technologies, such as Grid and Cloud computing, which require reasonable technical skills for their usage. Therefore, science gateways have become essential tools that assist users in interacting with e-Science applications. This paper describes TRENCADIS, a technology that supports the creation and operation of virtual knowledge bases. To this end, it provides developers with components and APIs for building secure data services that can be annotated and queried through ontology templates, based on DICOM and DICOM-SR. This technology was used in this paper to build a gateway for assisting diagnosis and research in breast cancer. We also present here the results of a study conducted to evaluate the gateway, from the point of view of the usability perceived by a group of physicians and radiologists.
