Pediatric Emergency Departments are More Likely Than General Emergency Departments to Treat Asthma Exacerbation With Systemic Corticosteroids

To determine whether systemic corticosteroids are under-prescribed (as measured by current NIH treatment guidelines) for children in the United States seen in the emergency department (ED) for acute asthma, and to identify factors associated with prescribing systemic corticosteroids.
We used data from the 2001-2007 National Hospital Ambulatory Medical Care Survey. The study population was children ≤ 18 years old in the ED with a primary diagnosis of asthma (ICD-9-CM code 493.xx) who received bronchodilator(s). The primary outcome was receipt of a systemic corticosteroid in the ED. Independent variables included patient-level (e.g., demographics, insurance, fever, admission), physician-level (provider type, ancillary medications and tests ordered), and system-level factors (e.g., ED type, geographic location, time of day, season, year). We used multivariable logistic regression techniques to identify factors associated with systemic corticosteroid treatment.
Systemic corticosteroids were prescribed at only 63% of pediatric acute asthma visits to EDs. Over the study period, there was a trend toward increasing systemic corticosteroid use (p for trend = .05). After adjusting for potential confounders, patients were more likely to receive systemic corticosteroids when treated in pediatric EDs than in general EDs (OR = 2.45; 95% CI: 1.26-4.77).
Systemic corticosteroids are under-prescribed for children who present to EDs with acute asthma exacerbations. Pediatric EDs are more likely than general EDs to treat asthma exacerbations with systemic corticosteroids. Differences in the process of care in pediatric ED settings (compared to general EDs) may increase the likelihood of adherence to NIH treatment guidelines.

    ABSTRACT: Background. National Asthma Education and Prevention Program recommended emergency department (ED) asthma treatment includes both providing systemic corticosteroids in the ED and a steroid prescription at discharge. Objective. To examine the prevalence of three types of substandard ED asthma care-providing a discharge prescription only, providing corticosteroids in the ED only, and providing neither- and how care varies with exacerbation severity. Methods. We used the National Hospital Ambulatory Medical Care Survey-Emergency Department (NHAMCS-ED) (2007, 2008, and 2009) to identify ED asthma visits (International Classification of Diseases-9(th) Revision Clinical Modification (ICD-9-CM codes 493.xx)) for patients aged 1 to <65 years. The primary outcome was the percent of visits receiving each type of substandard care, both overall and by exacerbation severity. Multinomial logistic regressions with predictive margins were used to obtain estimates adjusted for patient, visit, and hospital characteristics. Results. For 27.1% (confidence interval (CI): 24.0-30.2%) of visits, patients received corticosteroids both in the ED and as a discharge prescription. A discharge prescription only was provided for 12.3% of visits (CI: 10.2-14.6%), corticosteroids were provided in the ED only for 18.2% (CI: 15.6-21.2%), and no corticosteroids were provided for 42.4% (CI: 38.8-46.2%). Even among visits by patients with abnormal overall respiratory status (fast respiratory rates, pulse oximetry values <97%, or both), only 32.3% (CI: 27.8-36.8) were provided corticosteroids both in the ED and as a prescription, while the remainder received some type of substandard care. Adjusted and unadjusted results were similar. Conclusions. Substandard ED asthma care is common, even among visits by patients with more severe asthma exacerbations.

    ABSTRACT: Recent publication of national guidelines by the Pediatric Infectious Diseases Society (PIDS) and the Infectious Diseases Society of America (IDSA) provide recommendations around diagnostic testing and antibiotic treatment for children with community-acquired pneumonia (CAP). These guidelines emphasize limited use of chest radiograph (CXR) and complete blood count (CBC) and routinely performing viral testing and use of narrow-spectrum antibiotics. The objective was to estimate the rate of emergency department (ED) visits for pediatric CAP in the United States and to describe management of patients prior to publication of consensus national guidelines. Data were obtained from the National Hospital Ambulatory Medical Care Survey (NHAMCS) for ED visits from 2001 through 2009 for children with CAP. During the study period there were an estimated 375,000 ED visits for CAP annually; 85% occurred within a general, rather than pediatric, ED. Overall, 20% of children with CAP were hospitalized. Among children discharged from EDs with CAP, CBC was performed during 30% of visits, CXR during 83%, and viral testing in only 13%. Twelve percent of children discharged from EDs with CAP had blood cultures obtained. No major differences were observed in the rates of laboratory testing or antibiotic administration between children treated in general versus pediatric EDs. During the study period, only 21% of children discharged from EDs with CAP received amoxicillin, the guideline-recommended antibiotic. Most ED visits for CAP in the United States occur in general EDs. To encourage care that is consistent with national guidelines, efforts should be made to reduce the performance of certain diagnostic testing, such as CBC and CXR, among children discharged from EDs with CAP. Additionally, the use of narrow-spectrum antibiotics should be encouraged.

    ABSTRACT: Most children requiring emergency care in the United States are seen in community hospital settings that may lack pediatric expertise and/or ready access to pediatric subspecialists. A subset of these patients will need transport to a tertiary care center for definitive treatment. Pediatric transport teams from academic medical centers can play a crucial role in outreach and education in the community, thus helping to ensure that seriously ill or injured children receive the best possible care before transport as well as the safest and highest-quality transport to a pediatric center. This article presents an overview of the state of pediatric emergency care in smaller community hospitals and the patients they transfer for further care and concludes with suggestions on how transport services can implement effective, mutually beneficial, pediatric outreach activities.
