In-school HIV and AIDS counselling services in Botswana: an exploratory study

Abstract

This exploratory study describes the provision of HIV & AIDS counselling services in Botswana junior secondary schools as perceived by teachers. A total of 45 teachers (age range = 20-55; teaching experience range = 0-21 years) from three schools participated. The participants completed a questionnaire on the types of HIV & AIDS-related counselling services provided in the junior secondary schools services, their self-rated HIV & AIDS counselling training needs and their perceived importance of the HIV & AIDS-related counselling services. Descriptive statistical analysis revealed a broad range of HIV & AIDS-related counselling services including life-skills education, care and support education and stigma reduction counselling. Teachers perceived a greater need for training in HIV & AIDS counselling skills and also in the use of information technology to support counselling.

There is a huge interest by faith-based organizations (FBOs) in sub-Saharan Africa and elsewhere in HIV prevention interventions that build on the religious aspects of being. Successful partnerships between the public health services and FBOs will require a better understanding of the conceptual framing of HIV prevention by FBOS to access for prevention intervention, those concepts the churches of various denominations and their members would support or endorse. This study investigated the conceptual framing of HIV prevention among church youths in Botswana; - a country with one of the highest HIV prevalence in the world. Participants were 213 Pentecostal church members (67% female; age range 12 to 23 years; median age = 19 years). We engaged the participants in a mixed-method inductive process to collect data on their implicit framing of HIV prevention concepts, taking into account the centrality of religion concepts to them and the moderating influences of age, gender and sexual experience. After, we analysed the data using multi-dimensional scaling (MDS) and hierarchical cluster analysis (HCA) to map the ways the church youths framed HIV prevention. The findings suggest the church youth to conceptually frame their HIV prevention from both faith-oriented and secular-oriented perspectives, while prioritizing the faith-oriented concepts based on biblical teachings and future focus. In their secular-oriented framing of HIV prevention, the church youths endorsed the importance to learn the facts about HIV and AIDS, understanding of community norms that increased risk for HIV and prevention education. However, components of secular-oriented framing of HIV prevention concepts were comparatively less was well differentiated among the youths than with faith-oriented framing, suggesting latent influences of the church knowledge environment to undervalue secular oriented concepts. Older and sexually experienced church youths in their framing of HIV prevention valued future focus and prevention education less than contrasting peer cohorts, suggesting their greater relative risk for HIV infection. A prospective HIV prevention intervention with the Pentecostal church youths would combine both faith and secular informed concepts. It also would need to take into account the ways in which these youth interpret secular-oriented health concepts in the context of their religious beliefs.

Much of the literature on caring teachers in extreme settings, such as contexts of high HIV prevalence, centres around the material/structural challenges faced by teachers in attempting to take on a caretaker role as proscribed by policy (Hoadley, 2007;Machawira & Pillay, 2009;Theron, 2009) and documents the critical need for better training and resources for teachers if they are to play that role (Hattingh & De Kock, 2008;Henning & Chi, 2012;Holderness, 2012;Sefhedi, Montsi, & Mpofu, 2008;van Wyk & Lemmer, 2007;Wood & Goba, 2011). Schools are increasingly seen as key sites for support to HIV-affected and other vulnerable children, and teachers are assigned the critical role of identifying and providing psychosocial support. Drawing on the life-work history narratives of 12 teachers in Zimbabwe, this paper explores the psychosocial processes underpinning teachers' conceptualisations of these caring roles. The influence of prolonged adversity, formative relationships, and broader patterns of social and institutional change in teacher identity formation processes speak to the complex and embodied nature of understandings of 'care'. In such extreme settings teachers prioritise the material and disciplinary aspects of 'care' that they see as essential for supporting children to overcome hardship. This focus not only means that emotional support as envisaged in international policy is commonly overlooked, but also exposes a wider ideological clash about childrearing. This tension together with an overall ambivalence surrounding teacher identities puts further strain on teacher-student relationships. We propose the current trainings on providing emotional support are insufficient and that more active focus needs to be directed at support to teachers in relation with their students.

Researchers in the social sciences generally encounter problems in getting information when dealing with topics that are regarded as sensitive in school management. Principals regard certain issues as sensitive, and find it difficult to discuss them with researchers. We report on an investigation into such sensitive issues in Limpopo province, South Africa, as well as the reasons why principals regard them as sensitive. Interviews with 12 individual principals were conducted to gain an understanding of issues that they regard as sensitive. A 33-item self-report questionnaire was administered to 180 principals drawn from 272 secondary schools. Results reveal a range of issues regarded as sensitive to report on that pervade the management of schools. Principals have reported on the various reasons why they regard each of the aspects as sensitive, thereby making it difficult for them to provide the required information. Analysis of the different sensitive issues shows that certain biographical characteristics are significant mediators in principals' perceptions of sensitive Issues In school management. When researchers wish to elicit information on the issues identified in this study, they should consider methodological issues that may influence the investigation so that understanding and giving voice and visibility is not jeopardized.

This article proposes a model of sexual activity among secondary school-going Zambian girls. The model is tested for two age categories of adolescents using Covariance Structure Analysis. A sample of teenage girls was selected randomly from seven secondary schools that were selected randomly from schools located in the urban provinces of Copperbelt and Lusaka-Central in Zambia. The article identifies the role of dating as an intervening variable in explaining the variation in sexual activity among teenagers. In Zambia, courtship activities between young men and women traditionally took place under the supervision of parents and community elders. Presently, schools are an important setting for the young to meet and initiate sexual relationships. The theoretical and policy implications of this are discussed.
