Important note: No researcher should be compelled to share anything on this form, parcularly personal experience. This is a tool to help put in
place mechanisms to support researchers, not to pressure them into disclosure. We recommend that both supervisors and researcher ll out their
own forms and share them with each other, this helps to build trust in the relaonship. Nothing should be shared with a third person without the
permission of the person wring the form (unless a severe risk of harm protocol is put into acon, and even then, please try to gain permission
from the person who lled out the form). If, once you have lled out the form, you do not feel comfortable sharing all the informaon with your
supervisors/colleague then delete it before handing it in. It is also ok to decide to reconsider and share that informaon later.
