Abstract and Figures

Background and objectives: The front-of-pack label Nutri-Score has met a lot of scientific opposition. In the Netherlands, these were major concerns that Nutri-Score was not in line with the Dutch food-based dietary guidelines. In 2022, the algorithm behind the Nutri-Score was updated with the intention to bring it more in line with the general European food-based dietary guidelines. Methods: In this study, the renewed 2022 algorithm for solid foods is applied to the Dutch Food Composition Database (NEVO) to calculate the Nutri-Score values. Subsequently, the Nutri-Score values of all-solid foods were compared to the Dutch food-based dietary guidelines (the Wheel of Five). The foods that are included in the Wheel of Five are considered as “healthy”, i.e., would qualify for labels A or B, while the foods that receive labels C/D/E are considered “unhealthy”. Results: In total, 1980 solid foods were selected from NEVO. Despite the intended outcome, 19% of the unhealthy (non-Wheel of Five) products still received a Nutri-Score A or B. In addition, 25% of the healthy products in the Wheel of Five were scored as “unhealthy’, i.e., Nutri-Scores C/D/E. So grossly, circa one quarter of the foods will be wrongly labelled if the new algorithm is applied. Discussion: If the Nutri-Score is applied with the updated algorithm, this will mean that an average supermarket in the Netherlands will contain thousands of products with an inappropriate score. These results confirm the worries of the >200 Dutch food scientists and the associations of dietitians, life style coaches and weight councilors that the Nutri-Score will confuse Dutch consumers upon introduction. In their request to the Dutch Ministry of Health, they suggest to first bring the Nutri-Score essentially in line with our Wheel of Five before introducing the Nutri-Score system in the Netherlands. Kurzer Hintergrund. Im Februar verö8entlichten Peters und Verhagen in der Fachzeitschrift PharmaNutrition eine Literaturstudie zur Validierung und Wirksamkeit des Nutri-Score-Choice-Logos. Diese zeigt, dass die Wirksamkeit von Nutri-Score theoretisch gut belegt ist, es jedoch keine Beweise dafür gibt, dass Nutri-Score ist wirksam bei der Anpassung des Kaufverhaltens der Verbraucher. Darüber hinaus haben wir festgestellt, dass es einen Publikationsbias gibt: Die meisten positiven Studien zu Nutri-Score wurden von seinen französischen Entwicklern durchgeführt. Unabhängig von den französischen Entwicklern durchgeführte Studien zeigen jedoch ein gemischtes Bild [1]. Die Entwickler von Nutri-Score haben eine Antwort auf unseren Artikel geschrieben und diese an PharmaNutrition übermittelt. Antecedentes - O sistema de rotulagem nutricional Nutri-Score é atualmente proposto como o sistema de escolha em sete países da UE. Desde Abril Portugal é o 8.º país a introduzir o polémico logótipo. No entanto, ainda há muito debate científico sobre a validação e eficácia do Nutri-Score e há muita discussão sobre a afiliação dos autores e o resultado do estudo. - Métodos - Recentemente publicamos nosso artigo: Nutri-Score e viés de publicação: uma revisão completa da literatura sobre a comprovação da eficácia do logotipo da frente da embalagem Nutri-Score Peters & Verhagen, PharmaNutrition 27C (2024) 100380. Este artigo recebeu um artigo comentado pelos desenvolvedores do Nutri-Score: M. Touvier et al. 2024 “Refutação ao artigo publicado por S. Peters e H. Verhagen”. Fornecemos aqui um comentário convidado a esse artigo de refutação, que apoia ainda mais o viés de publicação observado. - Resultados - Nesta resposta à refutação, respondemos principalmente às questões científicas levantadas na refutação e explicamos mais sobre o nosso alegado conflito de interesses e a nossa motivação para escrever o artigo. Além disso, agradecemos basicamente aos autores do artigo de refutação por, talvez ironicamente, mas essencialmente, confirmarem a nossa análise: há um viés de publicação versus afiliação. - Discussão - No geral, as evidências disponíveis são claramente limitadas e tendenciosas, e são necessárias mais pesquisas para substanciar ou refutar a eficácia do Nutri-Score. Questionamos a situação atual de que o desenvolvimento científico, a avaliação do desenvolvimento científico e a atualização de erros de algoritmos devam ser realizados por apenas um grupo. Nutri-Score en publicatie-bias. Vanuit welke kant je het ook bekijkt: de suggestie van publicatiebias achter de wetenschap van Nutri-Score kan niet worden ontkend. Korte achtergrond. In februari publiceerden Peters en Verhagen een literatuurstudie naar de validatie en de effectiviteit van het keuzelogo Nutri-Score in het wetenschappelijke journal PharmaNutrition. Hieruit blijkt dat de effectiviteit van Nutri-Score theoretisch goed onderbouwd is, maar dat er geen bewijs is dat Nutri-Score effectief is in het bijsturen aankoopgedrag van de consument. Bovendien vonden we dat er sprake is van publicatie-bias: de meeste positieve studies van Nutri-Score zijn uitgevoerd door de Franse ontwikkelaars ervan. Echter, studies die onafhankelijk van de Franse ontwikkelaars zijn uitgevoerd, laten een wisselend beeld zien. De ontwikkelaars van Nutri-Score hebben een reactie geschreven op ons artikel en deze ingediend bij PharmaNutrition. Es nuestra respuesta a la refutación escrita por los desarrolladores de NutriScore en nuestro artículo anterior sobre Nutri-Score y el sesgo de publicación [1]. - Antecedentes - El logotipo nutricional Nutri-Score se propone actualmente como el sistema de elección en siete países de la UE. Sin embargo, todavía hay mucho debate científico sobre la validación y eficacia de Nutri-Score y hay mucha discusión sobre la afiliación de los autores y los resultados del estudio. - Métodos - Recientemente publicamos nuestro artículo: Nutri-Score y sesgo de publicación: una revisión completa de la literatura sobre la fundamentación de la efectividad del logotipo en el frente del paquete Nutri-Score Peters & Verhagen, PharmaNutrition 27C (2024) 100380. Cependant, le Nutri-Score soulève toujours de nombreux débats scientifiques concernant sa validation et son efficacité. De plus, il existe également de vives discussions relatives à l’affiliation des auteurs et aux résultats d’études. - Méthodes - Nous avons récemment publié notre article intitulé : « Le Nutri-Score et les biais de publication » : une revue complète de la littérature sur la justification de l'efficacité du logo Nutri-Score sur la face avant des emballages (Peters & Verhagen, PharmaNutrition 27C (2024) 100380). Cet article a fait l’objet d’un commentaire rédigé par les développeurs du Nutri-Score : M. Touvier et al. 2024 « Rebuttal to the paper (« Réfutation de l’article ») publiée par S. Peters et H. Verhagen ». Nous fournissons, par la présente, un commentaire invité en réponse à l’article de réfutation, qui soutient en outre les biais de publication observés. - Résultats - Dans cette réponse à l’article de réfutation, nous réagissons principalement aux questions scientifiques soulevées dans cet article en donnant plus d’informations concernant notre conflit d’intérêt allégué et notre motivation pour rédiger l’article. The subject under consideration is the voluntary Nutri-Score labelling system called FOP (front-of-pack) which is an increasingly common method of labelling the nutritional value on the front of the packaging of food products. While it has found recognition in many EU Member States, its use continues to provoke much controversy and doubt. The Nutri-Score system is used quite extensively not only in Poland, but also throughout the EU. Many Member States have regulated its use by law. The aim of this paper is to evaluate the Nutri-Score system and to determine whether it meets the requirements of food law in terms of labelling foodstuffs and, in particular, whether it is likely to mislead consumers. In the author’s opinion, constant modification and updating of the system that has been going on has already allowed for at least some elimination of the earlier reservations. It is also recommended to adapt the Nutri-Score system to the requirements of food law, to develop a modified FOP labelling systems, or consider a development of a completely new solution. However, simplifying the nutritional assessment of food products to a single symbol so that it is clear, not misleading, and respects the generally applicable requirements of food law is extremely difficult to do.
