Conocimiento de los médicos pasantes sobre la implementación de la telemedicina en las instituciones de salud

Abstract

Los estudiantes que utilizan nueva tecnología requieren modelos de aprendizaje distintos a los tradicionales, y en este sentido la telemedicina se presenta como una herramienta de apoyo para la educación. El objetivo de este estudio es evaluar el conocimiento de la telemedicina entre los alumnos que están realizando su servicio social. Se trata de un estudio descriptivo, observacional y transversal en el que se recopilaron datos mediante un instrumento estructurado y validado utilizando Microsoft Forms, aplicado a 113 pasantes de servicio social. Los resultados indican que el 52.2% (59) tiene un conocimiento limitado sobre la telemedicina. En conclusión, se destaca la importancia de que las escuelas de medicina incorporen de manera formal la telemedicina en su plan de estudios.

The research examined the implementation of telemedicine and new technologies in healthcare in Colombia. A mixed-method approach was employed, combining qualitative and quantitative methods to explore the topic. Relevant documents and studies were analyzed, identifying challenges and opportunities in telemedicine. Findings highlighted the importance of healthcare personnel training, integration of administrative and financial aspects, and ongoing evaluation of outcomes to ensure the success of telemedicine. Barriers such as the digital divide and ethical issues were identified. The research concluded that telemedicine offers an innovative solution to enhance access, efficiency, and quality of healthcare in Colombia but requires addressing significant challenges for equitable and effective implementation

Introduction: the characteristics of telehealth and telemedicine pose a challenge for its successful insertion into the professional training curriculum in the areas of health. To date, no research has been carried out that addresses the integration of telehealth and telemedicine in the academic curriculum of Chilean universities.Objective: design an instrument that allows measuring the self-perception of students in the area of health sciences in relation to their level of knowledge of basic aspects of telehealth and digital skills.Method: a 12-item instrument was designed with a Likert scale from 0 to 7, made up of two dimensions: Basic elements associated with telehealth (7 items) and skills associated with the use of technologies (5 items). It was applied to 308 students from five Chilean universities in the area of health sciences, the sample was intentional non-probabilistic.Results: a Cronbach's Alpha of 0,9035 was obtained for dimension 1 and 0,9729 for dimension 2. The factor analysis confirmed the existence of the two proposed dimensions.Conclusions: the instrument designed allows us to measure the perceived level of health sciences students in relation to the basic elements associated with telehealth and the skills for using technologies

RESUMEN El concepto de calidad ha evolucionado en el tiempo, pasando del ámbito industrial al sanitario, en este punto los modelos clásicos de calidad se hacen valer con cuerpos legales en procesos de acreditación específicos para el área de salud. Objetivos: comparar el Proceso de Acreditación Sanitario chileno de atención cerrada con el Modelo de Excelencia 2000 de la Fundación Europea para la Gestión de la Calidad. Metodología: cualitativa, a través de un diseño fenomenológico con un alcance descriptivo con el uso de la técnica, revisión bibliográfica analizando trece categorías emergentes dentro de los recursos bibliográficos encontrados. Resultados: en ambos dura 3 años el periodo de acreditación, también presentan 9 criterios más amplios y una subdivisión de casi 35, como grandes diferencias es que el modelo chileno de acreditación sanitaria se enfoca en asegurar los procesos clínicos, mientas que el EFQM tiene una propuesta, hacia el entorno y la sostenibilidad. Conclusiones: ambos modelos son muy parecidos en términos generales, sin embargo, resalta, la flexibilidad del EFQM por sobre el modelo de acreditación chileno, comprendiendo que el primero es un modelo de aplicación voluntaria, con énfasis en la mejora de gestión en general a través del cambio de cultura y procesos de la organización, mientras que el segundo tiene un enfoque más normativo, además de lo obsoleto del modelo de acreditación chileno (2009), siendo que el EFMQ se actualizo en 2020. his paper attempts to highlight the role of ICT in higher education for the 21st century. In particular the paper has argued that ICTs have impacted on educational practice in education to date in quite small ways but that the impact will g row considerably in years to come and that ICT will become a strong agent for change among many educational practices. It is evident from the study that use of ICT in education is increasing ve ry rapidly in various states of India. One of the most common problems of using Information and Communication Technologies (ICTs) in education is to base choices on technological possibilities rather than educational needs. In developing countries where higher education is fraught with serious challenges at multiple levels, there is increasing pressure to ensure that technological possibilities are viewed in the context of educational needs. The use of ICT in education lends itself to more student centred learning settings and often this creates some tensions for some teachers and students. But with the world moving rapidly into digital media and information, the role of ICT in education is becoming more and more important and this importance will continue to grow and develop in the 21st century. Thus, the paper suggests that ICT in higher education is not a technique for educational development but also a way of socio-economic development of the nation.

Studies indicate that there is demand for services performed responsibly. This study aims to present theoretical bases for the proposal of an indicator of responsible consumption for hospitality companies. It is the outcome of desk and documentary research on scientific bases for corporate social responsibility and sustainability indicators for hospitality companies. This research identifies the urgent need to review the way that corporate social responsibility actions are handled in this sector. It also identifies a need for change in the way that the social relationship with the client stakeholder is conceived from its inception. The research also confirms the importance of discovering different ways to act effectively towards sustainability. This study presents a theoretical proposal as a basis for Indicators of Responsible Consumption in Hospitality Companies (IRCHC). The specific goals of this research include: to present a theoretical proposal of IRCHC that adds value to CSs; to make sustainable the lifecycle of responsible service; to respect all sustainability dimensions; to have strategic applicability for HCs; and to be easily understood by involved parties. RESUMEN Los estudios indican que existe una demanda de servicios realizados de forma responsable. Este estudio tiene como objetivo presentar las bases teóricas para la propuesta de un indicador de consumo responsable para las empresas de hostelería. Es el resultado de una investigación documental y de gabinete sobre las bases científicas de los indicadores de responsabilidad social corporativa y de sostenibilidad para las empresas de hostelería. Esta investigación identifica la necesidad urgente de revisar la forma en que se gestionan las acciones de responsabilidad social corporativa en este sector. También identifica la necesidad de cambiar la forma en que se concibe la relación social con el cliente desde su inicio. La investigación también confirma la importancia de descubrir diferentes formas de actuar eficazmente hacia la sostenibilidad. Este estudio presenta una propuesta teórica como base para los Indicadores de Consumo Responsable en Empresas de Hostelería (IRCHC). Los objetivos específicos de esta investigación son: presentar una propuesta teórica de IRCHC que añada valor a las EF; hacer sostenible el ciclo de vida del servicio responsable; respetar todas las dimensiones de la sostenibilidad; tener aplicabilidad estratégica para las EF; y ser fácilmente comprensible para las partes implicadas.

La telemedicina es una tecnología que facilita la mediación a distan-cia entre el paciente y el personal médico. Aunque los desarrollos en este campo son más evidentes en Europa y Norte América, en América Latina tendría un mayor impacto, dadas las condiciones geográficas y socioeconómicas de la población. En ese orden de ideas, el propósito de este artículo es elaborar una revisión de lite-ratura acerca de los usos, desarrollos e innovaciones tecnológicas realizadas en torno al campo de la telemedicina, donde el empleo de esta tecnología facilita las consultas médicas a distancia y permite llevar los servicios de salud a la población rural, tanto en los países desarrollados, como los que se encuentran en vías de desarrollo.

Introducción: En la actualidad las intervenciones en salud ya sean de tipo diagnóstico o terapéutico no son distribuidas equitativamente, por lo cual garantizar la prestación de servicios de salud representa un reto incesante para muchos programas en diferentes países; sin embargo, gracias a los avances tecnológicos en telecomunicación y procesos de imágenes, es ahora posible ofrecer varios servicios de salud a un mayor número de la población a distancia con un buen nivel de calidad; esto se define como telemedicina. Método: A través de una búsqueda sistemática de la literatura médica en las bases de datos: Medline, ieeexplore, Scielo, por medio de las palabras mesh: Medical informatics, technology management, telemedicine, E-health, Latinoamérica, se buscó reconocer los datos relevantes con respecto a la historia, las aplicaciones actuales y futuras de la telemedicina. Desarrollo: Después de seleccionar la mejor evidencia según los objetivos de esta revisión, se procedió a analizar la importancia de la historia, aplicaciones en especialidades de medicina, ventajas y desventajas, barreras que limiten el desarrollo, y experiencias de la telemedicina en Latinoamérica. Conclusión: Actualmente el uso de la telemedicina avanza rápidamente y podría representar una buena alternativa para complementar en términos de costo-efectividad a muchos de los programas de salud que se ven alrededor del mundo, ya que facilita la toma de decisiones terapéuticas y diagnósticas en diferentes escenarios clínicos y complementa la formación académica en las escuelas de medicina; sin embargo, aun existen barreras de infraestructura principalmente los altos costos de inicio e instalación, que deben ser tenidos en cuenta, en especial para los países en desarrollo.

Although evidence of the effectiveness of telemedicine is accumulating, knowledge of how to make best use of telemedicine is limited. This article presents results from a multi-stakeholder project that developed a new concept, a 'shared service centre' for telemedicine that is envisioned as working across different telemedical initiatives to support the implementation and wider adoption of telemedicine. One year of participatory design and analysis of the shared service centre concept involved stakeholders, such as clinicians, patients, technicians, policy makers, lawyers, economists and information technology architects. More than 100 people contributed to the findings. Most of the ideas generated for potential centre support for telemedicine could be categorised under four service categories. The need for such support services was verified in the cases investigated, and by agreement among stakeholders from regional health authorities, municipalities, and general practice. Therefore, it is probable that a shared service centre could help enable the wider deployment of telemedicine. In this article, we use 'telemedicine' as an umbrella term for all the 'tele-' labels that are sometimes used rather indiscriminately to denote the use of information and technology to support healthcare services, including 'telehealth', 'telemonitoring', 'telehomecare', 'e-health', and so on. As per our definition, telemedicine may be synchronous and/or asynchronous, and may apply to any information and technology-based means of connecting healthcare actors and the patient, such as video communication, e-mail, electronic monitoring equipment, and Internet portals. Furthermore, the term 'telemedical initiative' covers projects in which telemedicine is conducted by a temporary project organisation, as well as self-contained telemedicine services used in daily, clinical practice in existing organisations. © The Author(s) 2015.

The application of information technologies and telecommunications in healthcare in southern countries can improve the health of millions of people living in poverty without access to high-quality health services. Specifically, telemedicine not synchronized with internet platforms is a very efficient option for international cooperation projects. Since its inception, the project Health 2.0: professionals from Africa-Spain connected, of the Spanish organization, Foundation Recover, Hospitals for Africa, has managed to connect health professionals from both continents with the goal of improving health in Cameroon. Thanks to a social network with the scientific format of case management and clinical updates, Spanish physicians collaborate through volunteer e-health in the training of their African colleagues to improve the quality of care provided to residents and also create sources of knowledge and research.

Una de las responsabilidades apremiantes de la educación en México es dar respuesta de calidad a las necesidades en materia de formación de profesionales competentes para la solución de los problemas que se presentan en las distintas áreas del quehacer social. De ahí que una preocupación permanente de las instituciones educativas es establecer sistemas de evaluación que evidencien que se logra el objetivo en materia de formación profesional. En el Centro Interdisciplinario de Ciencias de la Salud, Unidad Milpa Alta, del Instituto Politécnico Nacional (CICS-UMA. IPN) se está aplicando un sistema de aprendizaje basado en la investigación con responsabilidad social, mismo que nos ha permitido operar las unidades de aprendizaje, integradas en la etapa terminal del Programa Académico de Optometría, aplicarlas a la solución de problemas sociales mediante la práctica comunitaria y de esta manera evaluar realmente la calidad del futuro profesional. Es en la comunidad donde nuestros estudiantes deben demostrar mediante la aplicación de protocolos de atención basados en la investigación, que han adquirido los conocimientos cognitivos y procedimentales y su aplicación con un sentido ético basado en las normas morales que se observan en las comunidades objeto de estudio y servicio en el campo de la salud visual. One of the pressing responsibilities of education in Mexico is to provide quality response to the needs in training of competent professionals to solve problems that arise in different areas of social life. Hence a permanent concern of educational institutions is to establish evaluation systems that demonstrate that the objective is achieved in vocational training. In the Interdisciplinary Center for Health Sciences, Unit Milpa Alta, the National Polytechnic Institute (CICS-UMA. IPN) is applying a learning system based on research with social responsibility, it has allowed us to operate the learning units integrated in the terminal stage of the Academic Program Optometry, apply them to solving social problems through community practice and thus really evaluate the quality of professional future. It is in the community where our students must demonstrate through application of treatment protocols based on research, that have acquired cognitive and procedural knowledge and its application with an ethical sense based on moral standards observed in the communities under study and service in the field of visual health.

Vivimos unos tiempos condicionados por aceleradas transformaciones que suceden en distintos ámbitos: sociales, tecnológicos, educativos… Más que en una época de cambios –como se viene repitiendo–, parece que estamos inmersos en un cambio de época que nos impulsa a preguntarnos cómo será la escuela del mañana. Pensar en el futuro es apasionante, porque nos presenta la posibilidad de alcanzar aquellos destinos que en tantas ocasiones se nos habían escapado. Este libro, destinado tanto a la comunidad docente como a la investigadora, se ocupa de la escuela y de los retos de la sociedad del futuro, incidiendo en la necesidad de incluir la multidimensionalidad en el campo de la educación con el propósito de impulsar vínculos entre los conocimientos, las nuevas tecnologías y la convivencia de nuestro alumnado, abriendo caminos que potencien la generación de redes de saberes que faciliten la comprensión de una realidad cada vez más interconectada y compleja. De esta forma, el libro presenta diferentes elementos que interaccionan entre sí para que el profesorado pueda apostar por unas determinadas finalidades educativas pensando en el mejor futuro posible.

Introduction: There are numerous factors and barriers that hinder the use of telemedicine, among which some aspects of the organisation stand out. Thus, it would be interesting to determine its intention to use it in a health institution. The objective of this paper was to identify the factors that influence the intent to use telemedicine by the group of professionals of the Andalusian Health Service and the type of professionals based on the use and expectations of (information and communications technology (ICT). Methods: A specific questionnaire was designed based on an extended Model of Technological Acceptance that was distributed to a sample of 2,847 professionals of all the categories of this institution. A univariate analysis, exploratory factor analysis and logistic regression were carried out to determine those explanatory variables that are related to the use of telemedicine. Results: A total of 424 responses were obtained. The study determined that the institution's support, perceived usefulness and ease of use (P<.05) were factors that influence the use of telemedicine. The support of the workers and the technological profile of the professionals (P<.05) were significant. Conclusions: Due to there being a high number of factors in health services, it is possible to find differences between the explanatory models in each specific organisation. For this reason, studies need to be carried out before the implantation and use of telemedicine systems in order to identify which of the possible variables influence favourable adoption as a prerequisite for the generalised use of telemedicine.

Background Although teleconsultations have been used for many years in Australia, there has been a recent increase following new government incentives. There is a paucity of literature on enabling medical students to acquire the relevant skills. With a focus on equipping students for practice in rural and remote areas, our medical school has developed an innovative clinical skills lesson to prepare our students for their rural practice placements.Methods This lesson was delivered to all students in their third year of training in small groups to enable interactive learning. The objectives of the lesson were to familiarise students with: the various methods of conducting teleconsultations currently in use; the legal and ethical considerations; the technical and procedural issues; and the barriers and benefits for patients and doctors. Students rotated through four different stations over 2 hours and the lesson was evaluated using a student survey.ResultsMedical students self-reported statistically significant improvements in understanding the issues and procedures, and in confidence in conducting a telehealth consultation.DiscussionAnalysis of the results and student comments demonstrated that students recognise the value of telemedicine learning, and benefit from formal teaching on all aspects of telemedicine, including technology, ethics and protocols. Interestingly, the students found the opportunity to discuss areas such as the ethics of, and barriers to, the use of teleconsultations to be the most challenging and helpful of all of the stations.There is a paucity of literature on enabling medical students to acquire the relevant [teleconsultation] skills

Telemedicine is a remote medical practice, which utilizes advanced telecommunications and information technologies for the delivery of healthcare and the exchange of heath information across distances. The development of telemedicine has significantly changed the medical collaborative decision making and doctor-patient relationships and has an impact on the responsibilities of physicians to patients and how to treat them. Moreover, in the chain of medical care information exchanged between a requesting physician and medical expert should assist in decision making. In this regard, we propose means for the formalization of exchanges, which is very important because it facilitates a transparent and traceable understanding of the remote process. So, we engage knowledge-based modeling for supporting collaborative activities in telemedicine. This includes the engagement of formal ontologies to ensure structuration of terminology and identification across all entities in a domain so that multiple sources of data can be aggregated through comparable reference terms.

Nationwide, as physicians and health care systems adopt electronic health records, health information technology is becoming integral to the practice of medicine. But current medical education and professional development curricula do not systematically prepare physicians to use electronic health records and the data these systems collect. We detail how training in meaningful use of electronic health records could be incorporated into physician training, from medical school, through licensure and board certification, to continuing medical education and the maintenance of licensure and board certification. We identify six near-term opportunities for professional organizations to accelerate the integration of health information technology into their requirements.
