Systematic review and meta-analysis regarding the effectiveness of online/remote interventions for mental health in children, adolescents, and young adults after the onset of the COVID-19 pandemic (Preprint)

Abstract

BACKGROUND The prevalence of mental illness has increased in children, adolescents, and young adults during the COVID-19 pandemic, while at the same time, access to treatment facilities has been restricted, resulting in a need for the quick implementation of remote/online interventions. OBJECTIVE To give an overview of randomized-control studies examining remote/online interventions for mental health in children, adolescents, and young adults and to explore the overall effectiveness of these interventions regarding different symptoms. METHODS A systematic literature search was conducted according to PRISMA guidelines using PubMed and Google Scholar. A meta-analysis was applied using a random effects model to calculate overall effect sizes for interventions using standardized mean differences for post-intervention scores. RESULTS Twelve articles with n=3833 participants could be included in the final sample, and nine were included in the quantitative analysis. The studies examined different digital interventions for several different outcomes, showing better outcomes than controls in some studies. Meta-analyses revealed significant medium overall effects for anxiety (SMD = 0.35, 95% CI: 0.06, 0.64) and social functioning (SMD = 0.42, 95% CI: -0.68, -0.17) and a small significant effect for depression (SMD = 0.21, 95% CI: 0.13, 0.30). In contrast, no significant overall treatment effect could be found for well-being, psychological distress, disordered eating, and COVID-19-related symptoms. CONCLUSIONS The qualitative and quantitative analyses of the included studies show some promising results regarding the effectiveness of online interventions, especially for symptoms of anxiety and depression and for training in social functioning. However, for other symptom groups, effectiveness is not apparent. All in all, more research with high-quality studies is needed.

Abstract
Background: The prevalence of mental illness has increased in children, adolescents, and young adults during the COVID-19
pandemic, while at the same time, access to treatment facilities has been restricted, resulting in a need for the quick
implementation of remote/online interventions.

Background: This rapid review examines changes in the mental health of the German child and adolescent population during the COVID-19 pandemic. Methods: The basis are 39 publications, which were identified by means of systematic literature search (until 19.11.2021) and manual search. The databases of the included publications were systematized with regard to their representativeness for the general population, and the indicators used were categorized with regard to the depicted constructs and their reliability. Results: The large majority of the studies took place at the beginning of the pandemic until the summer plateau 2020. Representative studies mainly reported high levels of pandemic-related stress, increases in mental health problems, and negative impacts on the quality of life. Non-representative studies showed mixed results. Vulnerable groups could only be identified to a limited extent. Both routine and care-related data showed declines in the outpatient and inpatient service utilisation during the various waves of the pandemic followed by catch-up effects. Children and adolescents turned out to be more vulnerable during the pandemic compared to adults, but their stress levels varied with the waves of the pandemic and the related containment measures. This article provides a review of the magnitude of mental disorders in children and adolescents from recent community surveys across the world. Although there is substantial variation in the results depending upon the methodological characteristics of the studies, the findings converge in demonstrating that approximately one fourth of youth experience a mental disorder during the past year, and about one third across their lifetimes. Anxiety disorders are the most frequent conditions in children, followed by behavior disorders, mood disorders, and substance use disorders. Fewer than half of youth with current mental disorders receive mental health specialty treatment. However, those with the most severe disorders tend to receive mental health services. Current issues that are now being identified in the field of child psychiatric epidemiology include: refinement of classification and assessment, inclusion of young children in epidemiologic surveys, integration of child and adult psychiatric epidemiology, and evaluation of both mental and physical disorders in children.

English Abstract: Austrian adolescents often suffer from psychological stress and mental illness-a condition that was exacerbated by the CoVid-pandemic. Accessing support services such as therapies and counselling is difficult for many young people due to a lack of personal and structural resources; stigma and lack of information also mean that professional services are often not accessed by those who would profit from such services. What is more, the increased use of digital media among young people leads to additional stressors that require new coping strategies. The open2chat project addresses these difficulties and combines low-threshold support in the form of peer-to-peer guidance with a medium that is familiar and attractive to young people: the internet. While trained individuals between the ages of 16 and 21 support peers in a hybrid chat/e-mail format, open2chat is continuously evaluated scientifically and consequently improved based on feedback from users on both sides of the communication process. Deutsche Zusammenfassung: Österreichische Jugendliche litten schon vor, doch besonders auch während der CoVid-Pandemie häufig unter psychischen Belastungen und Erkrankungen. Auf Unterstützungsangebote wie Therapien und Beratungen zuzugreifen, gestaltet sich für viele junge Menschen aufgrund von persönlicher und struktureller Ressourcenknappheit als schwierig; auch Stigma und mangelnde Informationen führen dazu, dass oftmals nicht auf professionelle Angebote zugegriffen wird. Zusätzlich führt die vermehrte Nutzung digitaler Räume unter Jugendlichen zu zusätzlichen Stressoren, die neue Bewältigungsstrategien erfordern. Das open2chat-Projekt greift diese Schwierigkeiten auf und verbindet niederschwellige Unterstützung in Form einer Peer-to-Peer-Begleitung mit einem für Jugendliche vertrauten und attraktivem Medium: dem Internet. Während so ausgebildete Personen zwischen 16 und 21 Jahren Gleichaltrige in einem hybriden Chat/E-Mail-Format begleiten, wird open2chat laufend wissenschaftlich evaluiert und basierend auf den Wünschen von Nutzer*innen auf beiden Seiten des Kommunikationsprozesses verbessert.

Background Suicidal ideation is a major risk for a suicide attempt in younger people, such that reducing severity of ideation is an important target for suicide prevention. Smartphone applications present a new opportunity for managing ideation in young adults; however, confirmatory evidence for efficacy from randomized trials is lacking. The objective of this study was to assess whether a therapeutic smartphone application (“LifeBuoy”) was superior to an attention-matched control application at reducing the severity of suicidal ideation. Methods and findings In this 2-arm parallel, double-blind, randomized controlled trial, 455 young adults from Australia experiencing recent suicidal ideation and aged 18 to 25 years were randomly assigned in a 2:2 ratio to use a smartphone application for 6 weeks in May 2020, with the final follow-up in October 2020. The primary outcome was change in suicidal ideation symptom severity scores from baseline (T0) to postintervention (T1) and 3-month postintervention follow-up (T2), measured using the Suicidal Ideation Attributes Scale (SIDAS). Secondary outcomes were symptom changes in depression (Patient Health Questionnaire-9, PHQ-9), generalized anxiety (Generalized Anxiety Disorder-7, GAD-7), distress (Distress Questionnaire-5, DQ5), and well-being (Short Warwick–Edinburgh Mental Well-Being Scale, SWEMWBS). This trial was conducted online, using a targeted social media recruitment strategy. The intervention groups were provided with a self-guided smartphone application based on dialectical behavior therapy (DBT; “LifeBuoy”) to improve emotion regulation and distress tolerance. The control group were provided a smartphone application that looked like LifeBuoy (“LifeBuoy-C”), but delivered general (nontherapeutic) information on a range of health and lifestyle topics. Among 228 participants randomized to LifeBuoy, 110 did not complete the final survey; among 227 participants randomized to the control condition, 91 did not complete the final survey. All randomized participants were included in the intent-to-treat analysis for the primary and secondary outcomes. There was a significant time × condition effect for suicidal ideation scores in favor of LifeBuoy at T1 (p < 0.001, d = 0.45) and T2 (p = 0.007, d = 0.34). There were no superior intervention effects for LifeBuoy on any secondary mental health outcomes from baseline to T1 or T2 [p-values: 0.069 to 0.896]. No serious adverse events (suicide attempts requiring medical care) were reported. The main limitations of the study are the lack of sample size calculations supporting the study to be powered to detect changes in secondary outcomes and a high attrition rate at T2, which may lead efficacy to be overestimated. Conclusions LifeBuoy was associated with superior improvements in suicidal ideation severity, but not secondary mental health outcomes, compared to the control application, LifeBuoy-C. Digital therapeutics may need to be purposefully designed to target a specific health outcome to have efficacy. Trial registration Australian New Zealand Clinical Trials Registry ACTRN12619001671156

Objective To evaluate the clinical effectiveness of online remote behavior therapy, compared with face-to-face therapy in reducing tics and co-occurring disorders associated with the tics in a sample of youths with Tourette Syndrome. Design A randomized controlled trial. TS patients were randomized to receive face-to-face or online remote behavior therapy. Participants 40 children aged between 9 and 16 years affected by Tourette Syndrome. Results Online remote and face-to-face behavior therapy are equally effective in the treatment of tics and co-occurring disorders in children and adolescents affected by Tourette Syndrome. Both groups showed an improvement in the severity of tics, obsessive-compulsive symptoms, and anxiety symptoms, as assessed by neuropsychological findings. Online remote behavior therapy was more effective for reducing depressive symptoms than face-to-face behavior therapy. Conclusions Online remote behavior therapy is a promising tool for behavioral therapies for patients with Tourette Syndrome and may represents an alternative treatment option.

Objectives Our objective was to assess the effects of mental health interventions for children, adolescents, and adults not quarantined or undergoing treatment due to COVID-19 infection. Methods We searched 9 databases (2 Chinese-language) from December 31, 2019, to March 22, 2021. We included randomised controlled trials of interventions to address COVID-19 mental health challenges among people not hospitalised or quarantined due to COVID-19 infection. We synthesized results descriptively due to substantial heterogeneity of populations and interventions and risk of bias concerns. Results We identified 9 eligible trials, including 3 well-conducted, well-reported trials that tested interventions designed specifically for COVID-19 mental health challenges, plus 6 other trials with high risk of bias and reporting concerns, all of which tested standard interventions (e.g., individual or group therapy, expressive writing, mindfulness recordings) minimally adapted or not specifically adapted for COVID-19. Among the 3 well-conducted and reported trials, 1 ( N = 670) found that a self-guided, internet-based cognitive-behavioural intervention targeting dysfunctional COVID-19 worry significantly reduced COVID-19 anxiety (standardized mean difference [SMD] 0.74, 95% confidence interval [CI], 0.58 to 0.90) and depression symptoms (SMD 0.38, 95% CI, 0.22 to 0.55) in Swedish general population participants. A lay-delivered telephone intervention for homebound older adults in the United States ( N = 240) and a peer-moderated education and support intervention for people with a rare autoimmune condition from 12 countries ( N = 172) significantly improved anxiety (SMD 0.35, 95% CI, 0.09 to 0.60; SMD 0.31, 95% CI, 0.03 to 0.58) and depressive symptoms (SMD 0.31, 95% CI, 0.05 to 0.56; SMD 0.31, 95% CI, 0.07 to 0.55) 6-week post-intervention, but these were not significant immediately post-intervention. No trials in children or adolescents were identified. Conclusions Interventions that adapt evidence-based strategies for feasible delivery may be effective to address mental health in COVID-19. More well-conducted trials, including for children and adolescents, are needed.

Adolescents often look to their peers for emotional support, so it is critical that they are prepared to take on a supportive role, especially during a health crisis. Using a randomised controlled trial (ISRCTN99248812, 28/05/2020), we tested the short-term efficacy of an online training programme to equip young people with skills to support to their peers’ mental wellbeing during the COVID-19 pandemic. In June 2020, one-hundred UK adolescents (aged 16–18) recruited through social media were randomly allocated (1:1) to immediate 5-day peer support training or a wait-list, via an independently generated allocation sequence. Primary outcomes were indicators of ability to help others (motivation, perceived skills, frequency of help provided, compassion to others and connectedness to peers). Secondary outcomes included emotional symptoms, mental wellbeing, and indicators of agency (civic engagement and self-efficacy). We also collected qualitative reports of participants’ experience. Assessments were completed at baseline and 1 week post randomisation (primary endpoint), and up to 4 weeks post randomisation (training group only). The training increased support-giving skills, frequency of providing support, compassion and peer connectedness (medium–large-effect sizes), but not motivation to provide support, 1 week post randomisation, compared to controls. Gains in the training group were maintained 4 weeks post randomisation. Training also improved adolescents’ mental health and agency, and qualitative reports revealed further positive outcomes including increased self-care and empowerment. Leveraging digital platforms that are familiar to young people, peer support training has the potential to enable adolescents to support their own and their peers’ mental wellbeing during a health crisis.

Purpose: This study develops and investigates the changes in anxiety symptoms and quality of life (QoL) among participants of the online Strength-informed Acceptance and Commitment Therapy (SACT) across three tests. Methods: A small-scale, quasi-experiment with no control group was conducted. Repeated-measures analysis was employed to assess the changes of the three tests, which were the pre-experimental, post-test, and 3-month follow-up test. A total of 47 adolescents (ages 10–12) completed the 45-min intervention that lasted 10 weeks. Results: Compared with the pre-experimental, the post-test indicated significant anxiety symptoms reduction but not a statistically significant increase in QoL. The 3-month follow-up test indicated reduced anxiety symptoms and improved QoL compared with the pre-experimental. The with-in subject changes were substantial. Conclusions: The online SACT is a promising model to reduce anxiety symptoms and promote QoL among adolescents during the current COVID-19 pandemic, which show both short- and long-term benefit to the participants.

Background: Studies have shown a high prevalence of depression during pregnancy, and there is also evidence that cognitive behavioral therapy (CBT) is one of the most effective psychosocial interventions. Emerging evidence from randomized controlled trials (RCTs) has shown that technology has been successfully harnessed to provide CBT interventions for other populations. However, very few studies have focused on their use during pregnancy. This approach has become increasingly important in many clinical areas due to the COVID-19 pandemic, and our study aimed to expand the knowledge in this particular clinical area. Objective: Our systematic review aimed to bring together the available research-based evidence on digitalized CBT interventions for depression symptoms during pregnancy. Methods: A systematic review of the Web of Science, Cochrane Central Register of Controlled Trials, CINAHL, MEDLINE, Embase, PsycINFO, Scopus, ClinicalTrials.gov, and EBSCO Open Dissertations databases was carried out from the earliest available evidence to October 27, 2021. Only RCT studies published in English were considered. The PRISMA (Preferred Reporting Items of Systematic Reviews and Meta-analyses) guidelines were followed, and the protocol was registered on the Prospective Register of Systematic Reviews. The risk of bias was assessed using the revised Cochrane risk-of-bias tool for randomized trials. Results: The review identified 7 studies from 5 countries (the United States, China, Australia, Norway, and Sweden) published from 2015 to 2021. The sample sizes ranged from 25 to 1342 participants. The interventions used various technological elements, including text, images, videos, games, interactive features, and peer group discussions. They comprised 2 guided and 5 unguided approaches. Using digitalized CBT interventions for depression during pregnancy showed promising efficacy, with guided intervention showing higher effect sizes (Hedges g=1.21) than the unguided interventions (Hedges g=0.14-0.99). The acceptability of the digitalized CBT interventions was highly encouraging, based on user feedback. Attrition rates were low for the guided intervention (4.5%) but high for the unguided interventions (22.1%-46.5%). A high overall risk of bias was present for 6 of the 7 studies. Conclusions: Our search only identified a small number of digitalized CBT interventions for pregnant women, despite the potential of this approach. These showed promising evidence when it came to efficacy and positive outcomes for depression symptoms, and user feedback was positive. However, the overall risk of bias suggests that the efficacy of the interventions needs to be interpreted with caution. Future studies need to consider how to mitigate these sources of biases. Digitalized CBT interventions can provide prompt, effective, evidence-based interventions for pregnant women. This review increases our understanding of the importance of digitalized interventions during pregnancy, including during the COVID-19 pandemic.

Zusammenfassung Internetberatung ist in Österreich durch die Internetrichtlinie für Psychotherapeut_innen festgelegt, jedoch erlaubt diese nicht die psychotherapeutische Behandlung mittels Medien. Es wird darauf hingewiesen, dass derzeit keine Psychotherapie via Internet eines wissenschaftlich begründeten und evaluierten Vorgehens existiert. Interessen dies zu ändern, gibt es schon seit Jahren; die Herausforderung des Lockdown in der Covid19-Krise haben es – zumindest zeitbeschränkt – möglich gemacht, psychotherapeutische Behandlung mittels Medien durchzuführen. Die reflektierten Erfahrungen, die Psychotherapeut_innen und Klient_innen mit dieser ausnahmsweise gewährten Form von Psychotherapie gemacht haben, können ein kleiner Beitrag dazu sein, herauszufinden, ob Psychotherapie – weitere Forschung vorausgesetzt – auch mittels Internet oder Telefon stattfinden sollte, welche Faktoren zu beachten sind oder ob optimale Krankenbehandlung immer unmittelbar, im engeren Verständnis dieses Wortes, fachlich vertretbar ist.
