Minimally invasive treatment approaches in oral implantology using a dental clinical microscope for magnification

Abstract

Operating on fine anatomical structures without using optical magnification is simply unthinkable ever since the development of the surgical microscope by Dr Littmann and Prof Wullstein in 1953. The combined use of the dental microscope and micro-instrumentation provides the basis for microsurgical approaches in dentistry (Velvart 1997). Even though dental use of the surgical microscope has previously been limited for the most part to endodontics and periodontics (Baumann 2000), the following article should provide a sound argument for use of optical magnification also in minimally invasive implantology procedures.

This prospective clinical study evaluated the 5-year survival and success rates of 66 titanium implants placed in bone that had been previously augmented with autografts and nonresorbable barrier membranes. During the observation period, three patients with five implants dropped out of the study. None of the remaining 61 implants were lost during the follow-up period (implant survival rate of 100%). One implant exhibited a periimplant infection, whereas 60 implants were considered clinically successful at the 5-year examination, resulting in a 5-year success rate of 98.3%. It can be concluded that the clinical results of implants in regenerated bone are comparable to those of implants in nonregenerated bone.

The purpose of this article is to describe the method for reconstructing patients with severe anterior maxillary defects, using a combination of distraction osteogenesis, onlay bone grafts, and implants to support either fixed or fixed/removable prostheses. The surgical technique involves placement of an extraosseous distraction device followed by greater than 10 mm of distraction of the alveolus. A corticocancellous graft is harvested from the hip to establish sufficient horizontal bone for implant placement. Implants are placed after graft consolidation followed by fabrication of a fixed or fixed/removable prosthesis depending on the needs of the individual patient. Five patients have been prospectively followed using this technique, with completion of their prosthetic reconstruction, with follow-up ranging from 1 to 4 years. The combination of distraction, onlay bone grafting, and implant placement has been able to reconstruct patients with severe maxillary defects.
