U.S. Deaths: A Look at Monthly Data from 1999 to 2021.
Follow-up to COVID-19 Deaths: A Look at U.S. Data.
Abstract
This paper presents plots of monthly U.S. deaths, from 1999 to 2021, using publicly available CDC data.
Variables considered include underlying cause of death, age, and state of residence of the deceased at the time
of death. Distributions of deaths across months, years, causes, age groups, and states, are reviewed.
Few observations are made about the plots presented. The main goals of this working paper are (1) to provide
an update to data presented in Briand’s Feb. 2021 working paper and (2) to provide plots of publicly available
data to readers, to complement their own private observations (personal or professional).

On Graph 1c above, for all plots, the vertical axis was scaled with minimum bound of 0 and maximum bound of
50,000. Although this doesn’t allow us to distinguish variations of deaths across months within a state with
lower population, it is a great way to get an overall picture of the variation of monthly deaths across states, as
well as to get a quick glance at which states have grossly departed from their usual pattern this past two years.
New York and New Jersey (to a lesser extent) are behind the April 2020 U.S. spike in deaths and California is
behind the January 2021 one. Florida and Texas dominate the September 2021 U.S. deaths spike, in numbers.

Months of yearly deaths peaks at the national level are outlined above: They usually occur in December-
January, but have also occurred in March. The April 2020 deaths peak is also outlined. The September
2021deaths peak is seen taking shape for age groups 65-74 and younger.

The April 2020 deaths for the 75-84 and 85 years and older are best compared to the 2000, 2018 and 2020-2021
highs. The highest monthly deaths were observed Dec. 2020-Jan. 2021, and the most recent lowest one, June
2021. Note, that low is lower than the 2020 one, for both the 75-84 and 85 years and older age groups, despite
their general upper trend in deaths.

Both age groups (55-64 and 65-74) show a noticeable change in their pattern of monthly deaths for years 2020-
21, with (1) deaths in Dec. 2020-Jan. 2021 higher than in April 2020, or any other prior peak of monthly deaths,
and (2) deaths in Aug.-Sept. 2021, comparable to April 2020 level for the 65-74 years old and nearly as high as
their Dec. 2020-Jan. 2021 peak for the 55-64 years old.

Peaks and troughs are reversed in the time of the year they occur for age groups 15-24 and 25-34 compared to
older ones: Peaks occur in July (instead of January) and troughs occurs in February (instead of June-
September). The low in February 2021 was the highest ever experienced by both of these age groups over years
1999-2021. The Aug-Sept 2021 deaths peak was the highest ever experienced for the 25-34 years old over years
1999-2021.

3.2 Leading causes

The highest peak, over the period reviewed here, occurred January 2000 for deaths due to heart attacks and
January 2018 for deaths due to diseases of the respiratory system (COVID-19 excluded). The peaks of deaths due
to heart attacks were lower in April 2020 and January 2021 than they were in January 2000 and January 2018.
The peak of deaths due to diseases of the respiratory system (COVID-19 excluded) was lower in April 2020 than
it was in January 2018 and, in January 2021, it became lower yet.

Monthly COVID-19 deaths show a pattern unlike any other cause. U.S. COVID-19 deaths reached a low of 6,498
in June 2021, after a high of 96,024 in January 2021. COVID-19 deaths of 58,305 in September 2021 were close
to their April 2020 number of 61,824.

Jan.-July 2021 influenza and pneumonia deaths are clearly lower than they have never been before, during those
months, over the period reviewed here (it is true for Dec. 2021 deaths, too); this points to the reclassification of
some of the deaths to the newly created COVID -19 category.

The pattern of deaths due to Cerebrovascular diseases mirrors that of monthly deaths due to heart diseases. The
higher deaths due to Alzheimer from 2015 on, points to a structural change most likely due to the introduction
of improved tests or methodology of detection. This structural change is not unlike the one observed since
March 2020 for U.S. deaths (see Graphs 1a-b).

Deaths due to diabetes were higher in 2020-21 than in previous years, for most months—this was true for
deaths due to Alzheimer in 2020.

California is the most populated state in the United States (2). It does not show any spike in deaths in April
2020 (See Appendix Graph A.5), but it does in Dec. 2020-Jan. 2021: See Graphs 17a-c for differences in deaths
between age groups within the state of California.

Wyoming, the least populated U.S. state (2), does not show any deaths spike in April 2020 (See Appendix
Graph A.50), but it does in Dec. 2020 and Oct. 2021. These Dec. 2020 and Oct. 2021 deaths spikes can be seen
for oldest age groups on Graph 18a.

The Oct. 2021 deaths spike is also discernible for the 55-64 years old on Graph 18b.
The number of deaths for younger age groups are too few (less than 10) to be publicly available for most
months.

5 Conclusion
The U.S. population did not decrease in 2020. The percentage death rate experienced in 2021 is the same as it
was in 2020. U.S. monthly deaths have exhibited a consistent seasonal pattern year after year, with peaks
occurring in December-January, or March. This changed in 2020-21.
The three months with the highest deaths, at the U.S. level, over years 1999-2021, were April 2020, January
2021 and September 2021. A structural change in monthly deaths is observed February 2020 on, as indicated by
troughs of monthly deaths estimated higher by 20,000 (or 240,000 on a yearly basis) than in previous years.
December 2020-January 2021 U.S. deaths are higher than in April 2020 and September 2021 for 65-74, 75-84,
and 85 years and older age groups, while August-September 2021 U.S. deaths are higher than in April 2020 and
January 2021 for 25-34, 35-44, and 45-54 years age groups.
Deaths due to accidental poisoning and exposure to noxious substances, homicides, and chronic liver disease
and cirrhosis, increased in 2020-21, with poisoning peaking in April 2020, homicides peaking in July 2020 and
deaths due liver diseases peaking in January 2021. Surprisingly, suicides do not show an increase in 2020-21.
Increases in deaths due to diabetes and Alzheimer are also observed in 2020-2021.
January 2021 deaths due to heart diseases are lower than their January 2018 number. Deaths due to diseases of
the respiratory system reached a peak level in January 2021, that hasn’t been that low since 2012—this points to
the reclassification of some of the deaths to the newly created COVID-19 category.
While April 2020 U.S. deaths peak was driven by New York state’s number, the higher January 2021 peak was
driven by California. The September 2021 U.S. deaths peak was driven by Florida and Texas’ numbers. At the
states level, the September (Aug-Oct) 2021 deaths peaks were higher than their Dec. 2020-Jan. 2021 ones for
Alaska, Florida, Hawaii, Idaho, Louisiana, Montana, Oregon, Vermont, Washington and Wyoming.
