Ilizarov Fixator in the Management of Benign, Malignant and Complicated Cases of Bone Tumours

Objective: evaluate the Ilizarov fixator in the management of bone tumours regarding the duration and the ease of the procedure, the outcome and the complications. Methods: This study was done at the Department of Orthopedic surgery in Ain Shams University Hospitals. 15 patients with benign or malignant tumours were treated by resection followed by bone and soft tissue reconstruction and ... [Show full abstract] Ilizarov circular fixator in January 2012-April 2013 and followed up till July 2014. Results: Follow-up was done with a mean period of 21.7 (14-28) months. Adjustment of the frame was done as required. As regards malignant tumours, the disease free survival has a mean of 22.8 (12-30) months. 2 patients developed local recurrence. The overall survival was a mean of 25.8 (13-30) months. 1 patient died as a result of chemotherapy. The MSTS score was 50% to 100% with a mean value of 84.6%. ASAMI-bone score was excellent in 53% of the cases, good in 2 cases, fair in 1 case but poor in 4 cases. ASAMI-function score was excellent in 53% of cases, good in 33% of cases, fair in 1 case and poor in 1 case. The adverse effects were 7 problems, 5 obstacles and 6 complications according to Paley’s classification. Conclusion: Biological reconstruction using Ilizarov circular fixator should be considered safe and effective in the management of benign and low grade malignant tumours, and a salvage treatment in case of failure of other methods of limb salvage.

Platelet rich plasma in treatment of plantar fascitis

Plantar Fasciitis is a very common problem in the field of orthopaedics and are very difficult to treat and a challenge to orthopaedicians. The aim of our study was to find out the efficacy of autologous PRP ... [Show full abstract] in relieving pain in patients with plantar fasciitis. Methods: In this study, 25 patients with plantar fasciitis (age above 18 years) were selected. The pain intensity was assessed with visual analogue score initially and during follow up. All subjects were given single autologous intra-lesional PRP injection and the results were assessed using difference in VAS . Results: The mean pain score at presentation was 8.08. The mean pain scores at subsequent intervals of 1,2,4,6 months after injection were 4.32, 2.60, 1.88 & 2.00 respectively. (P<0.05 which is statistically significant). The maximum pain relief was seen at 4^th month post PRP injection and the pain relief was sustained till the last follow up. Conclusions: From our study we have found that Intra-lesional autologous PRP is effective in relieving pain in patients with plantar fasciitis. PRP is biological and can be a safe adjuvant in the treatment of Plantar Fasciitis. </p

Durch die Anwendung neuer Prinzipien in der Osteosynthesetechnik (v.a. winkelstabile Verfahren) lässt sich dieses Fast-Track-Vorgehen nun auch bei der Therapie fragiler (osteoporotischer) und komplexer oder Defektfrakturen anwenden. The concept of „fast track“ has not yet been established in orthopaedic trauma surgery. Principles such as those used in the „fast track” procedure for abdominal surgery have been employed in orthopaedic surgery for a long time. The best results can be achieved by early operative treatment, stable osteosynthesis and, if the soft tissues allow, an early initiation of mobilization under optimal pain management. Based on new techniques in osteosynthesis, in particular locked-screw techniques, „fast track“ is also applicable for fragility fractures (osteoporosis), complex shattered bone and bone defect situations.

MRI features of low-grade and high-grade chondrosarcoma in enchondromatosis

The pre-biopsy MRI study was assessed by two experienced musculoskeletal radiologists for tumour origin (intramedullary or surface), cortical expansion, cortical destruction, bone marrow oedema, periosteal reaction, soft tissue mass and soft tissue oedema. MRI features were compared with the final histopathological diagnosis.ResultsThe study group comprised 25 males and 16 females, with a mean age of 34.9 years (range 6–81 years). Fifty-nine lesions were assessed (12 patients had > 1 tumour treated), including 43 LGCT and 16 HGCT. Significant MRI features suggesting malignant transformation to HGCT for both observers included bone oedema (p = < 0.001 and 0.002), periosteal reaction (p = 0.01) and soft tissue oedema (p = 0.001 and 0.05). Cortical destruction and soft tissue mass were predictors of HGCT in major long bones, but no significant differentiating features were identified in the hands and feet.Conclusion The presence of bone oedema, periosteal reaction and soft tissue oedema on MRI may indicate a high-grade malignant transformation of chondral tumours in patients with enchondromatosis.

Herein, we report on a combination of these two methods: circular external fixator with low-profile mini-fragment plate fixation. At 18 months postoperatively, the American Orthopaedic Foot & Ankle Society Ankle-Hindfoot score was a perfect score of 100. Radiographs taken in the loading position showed no narrowing of the joint fissure. There were no soft tissue infections, no plate breakage, no bone fusion, no symptoms of plate irritation, and no need for nail extraction.
