Abstract

Introduction: Preparation of the cervix and induction of labor is necessary before the onset of labor. In traditional medicine, rose has been used as an effective factor in the onset of labor contractions. However, clinical studies on its efficacy are very rare. This study was done to determine the effect of oral rose-water on cervical ripening in primiparous women. Methods: This study was a randomized clinical trial on 60 primiparous women with 40 weeks of gestation who were assigned to two groups of 30 people of intervention and control using method of intervention blocks. Intervention group consumed five ml of Rose water daily orally since 40 week of gestation for one week and the control group, was received routine care. One, three and seven days after beginning of the intervention and entering the labour, Bishop's score was measured. Data were gathered by a questionnaire form, physical examination, observing and counting fetal movement and check list of rose-water consumption. Data were analyzed using T-tests, analysis of variance and repeated values. Results: The mean age of patients in the intervention and control group was 26.53± 5.16, 26.53± 5.28 respectively. The onset of labor did not show a significant difference in two groups(p=0.303). Analysis of variance in duplicate measurements showed that the mean of Bishop's grade point average on the third and seventh days after beginning of intervention in the two groups did not differ significantly from each other (p =0/160). Also, the results of this test showed that the trend of bishop score changes in both groups was incremental. Conclusion: Consumption of 5 ml of rose-water daily, no significant effect on preparing cervix and was safe. It is recommended for future research to assess the effect of higher dose of rose-water on preparing cervix. and induce labor, compare it with the control group, for future research. Introduction: Alleviating labor pain has always been one of the most important issues in midwifery practice. Nowadays, non-pharmacological pain relief methods such as aromatherapy are considered due to low side effects. This study was performed with aim to review the studies which evaluated the effect of aromatherapy on relief of labor pain. Methods: In this review study, electronic databases including: SID, Iranmedex, Magiran, IranDoc, Cochrane Library, MEDLINE/PubMed, Scopus and Google Scholar were searched with the keywords of aromatherapy, labor pain and clinical trial up to November 9, 2016. Search was limited based on English or/and Persian articles. The quality of studies was assessed using the Cochrane "risk of bias" tool. The results were reported qualitatively. Results: Of 456 articles which were found in the initial search, 24 randomized trials (3414 participants) were selected for systematic review. The studies were conducted in Iran, India, Egypt, Korea, Italy and New Zealand. The used essence were lavender in 11 articles, rose in 6 articles, common sage in 3 articles, geranium in 3 articles, jasmine in 3 articles, chamomile in 2 articles, peppermint in 2 articles, bitter orange in 1 article, mandarin in 1 article, orange in 1 article, pink in 1 article, ginger in 1 article, lemongrass in 1 article and frankincense in 1 article. Most of the studies showed the positive effects of aromatherapy on labor pain relief. Conclusion: Available data demonstrate that aromatherapy can be an effective intervention for alleviation of labor pain, but more studies with strong methodology are needed to achieve more rigorous evidence.

Introduction: Normal physiologic childbirth program was implemented in mother-friendly hospitals of Iran after 2008. The aim of present study is to assess the effect of normal physiologic childbirth program in a mother-friendly hospital on duration of active phase and second stage of labor. Methods: This study was a clinical trial that was conducted at the Sina and Ommolbanin mother-friendly hospitals in Ahvaz, Mashhad, Iran, in 2016. The intervention group of 77 women was offered the childbirth preparation classes during pregnancy and normal physiologic childbirth program during labor and the control group of 77 women received routine care. Results: The results showed that after controlling the confounding factors, the active phase and second stage of labor were significantly shorter in the intervention group (p<0.001). Conclusion: Complete implementation of normal physiologic childbirth program can reduce the duration of labor.

Introduction: Today, Cesarean section is the most common obstetric surgery. In the past 20 years, its rate has been increased from 5% to 20%. Unintended consequences in cesarean section are more than in vaginal delivery; especially for mother. The experiences have shown that the women who had consumed sisymbrium during their gestational had more successful vaginal delivery. The goal of this study is to determine the effect of sisymbrium seeds consumption at the end of pregnancy on the rate of cesarean delivery and Apgar score. Methods: This double-blind clinical trial was held on 68 eligible nulliparous women who referred to the Health Care Centers of the Rafsanjan city. Individuals were randomly located into case and control groups. 28 packets which contained 2mesghal (equal to about 5 grams) of sisymbrium - seeds were given to the case group from 37th week. Two-sectional questionnaire was used for data collection. Some parts were filled in seven Health Centers of Rafsanjan City and the rest was completed in the maternity by the trained midwives. Data were analyzed by SPSS statistical software version 16, independent T and chi-square tests. P value less than 0.05 was considered statistically significant. Results: According to the results, duration of first stage in labor (p=0.022) and cesarean rate (p=0.032) was significantly less in intervention group while spontaneous labor was more. But this difference was not statistically significant (p=0.29). Apgar score in the first (p=0.39) and the fifth minute (p=1) and intrauterine passage of meconium (p=0.53) didn’t have any significant difference in two groups. Conclusion: Sisymbrium- seeds affect the parturition process by ripening cervix without increasing cesarean delivery or decrease of neonatal Apgar.

Background and Aim: Mutans streptococci are the main cause of tooth decay. Application of natural materials as mouthwash has been effective in reducing the bacterial count. This study aimed to assess the antimicrobial effects of rose water and pomegranate vinegar in comparison with Persica mouthwash on two oral bacteria responsible for tooth decay. Materials and Methods: Strongly adherent strains of Streptococcus mutans and Streptococcus sobrinus were selected for this in vitro study. Antimicrobial effects of pomegranate vinegar and rose water on microbial count in the biofilm and adhesion potential of bacteria were evaluated by microtiter plate method. Also, the well-plate technique was used to assess the effect of rose water and pomegranate vinegar in comparison with Persica mouthwash on bacterial growth and proliferation. The obtained resulted were analyzed by one-way ANOVA and Tukey's post hoc test using Graph Pad Prism version 5 software. Level of significance was set at P=0.05 with 95% confidence interval. Results: Pomegranate vinegar, rose water and Persica decreased plaque formation by S. mutans by 93%, 80% and 68%, respectively. These values for the S. sobrinus were 92%, 57% and 48%, respectively (all Ps<0.001). Pomegranate vinegar was more effective than the other two materials (P<0.001). However, none of these materials eliminated the biofilm. Pomegranate vinegar and Persica mouthwash inhibited the growth of the afore-mentioned bacteria. Conclusion: Within the limitations of this study, it seems that pomegranate vinegar and rose water have the potential to prevent or control the proliferation of S. mutans and S. sobrinus.

Rosa damascena Mill. is one of the most famous ornamental plants cultivated all over the world mostly for perfumery industries. Traditionally it has been used as an astringent, analgesic, cardiac and intestinal tonic.The paucity ofauthoritative monographs urged usto summarize its clinical effectiveness and safety with acomprehensive review of the literature. “PUBMED”, “SCOPUS”, “WEBOF SCIENCE” were searched up to April 30, 2017 with search terms:(“Rosa damascena” OR “Damask Rose”). All human studies with any mono-preparation were included. In vitro and animal studies from “PUBMED”were also reviewed and outlined. Of “1000” identified publications, twelveeligibleclinical trials were retrieved. Antimicrobial, anti-inflammatory, antioxidant, anticancer, protective neuronal, cardiac, gastrointestinal and hepatic effectsin 30 in vitro and 21 animal studies were also shown. there are promising evidences for the effectiveness and safety of Rosa damascena Mill in pain relief, but confirmatory studies withstandardized products is suggested.

Since ancient times, cervical assessment for predicting timing of delivery has relied primarily on digital (subjective) assessment of dilatation, softening, and length. To date, transvaginal ultrasound cervical length is the only one of these parameters that meets criteria for a biomarker; no objective, quantitative measure of cervical dilatation or softening has gained clinical acceptance. This review discusses how the cervix has been assessed from ancient times to the present day and how a precision medicine approach could improve understanding of not only the cervix, but also parturition in general.

Background: When delivery is indicated prior to 34 weeks, many providers perform a cesarean delivery (CD) rather than induce labor based on perceptions of a high failure rate. Given the morbidity of CD, an accurate estimate of the success rate and factors associated with success in preterm induction of labor (IOL) is important in management decisions. Objective: We sought to develop a prediction model for successful induction of labor in preterm patients using factors known at the time the decision is made to deliver. Study design: A retrospective cohort study of all live singletons undergoing an indicated IOL between 23-34 0/7 weeks from 2011-2015. Pregnancies with major fetal anomalies or no intrapartum fetal monitoring were excluded. Successful IOL was defined as vaginal delivery (VD). The cohort was randomly split into a training cohort to develop a prediction model for VD and a validation cohort to test the model. Factors significantly associated with VD were identified using univariate analyses, and candidate factors were used in the multivariate logistic regression model. Only factors known at the start of the IOL were used in the model. Receiver operating characteristic (ROC) curves were created to estimate the predictive value of the model. Sensitivity and specificity of the model was assessed. Results: Of 331 patients who underwent IOL, 208 (62.8%) delivered vaginally and 123 (37.1%) by CD. Of the factors significantly associated with CD, the final model included gestational age, simplified Bishop score, suspected IUGR, chronic hypertension, and body mass index. In the training cohort, the model correctly classified 72.3% of subjects with a sensitivity (CD predicted/CD performed) of 56.7% and a specificity (VD predicted/VD performed) of 84.1%. When applied to the validation cohort, 73.9% of subjects were correctly classified with a sensitivity of 44.6% and specificity of 89.0%. ROC curves had an area under the curve of 0.75 for the training cohort and 0.77 for the validation cohort. Conclusions: Over 60% of women undergoing IOL at <34 0/7 weeks deliver vaginally. For women undergoing IOL<34 0/7 weeks, this prediction model rarely classifies individuals who can have a VD as needing a cesarean. This model may provide an accurate assessment tool to evaluate which patients will likely deliver vaginally to avoid the morbidity of CD, while conversely identifying subjects at high risk of CD <34 0/7 weeks.

Objective: The objective of this study is to analyze the association of diabetes mellitus with progress and outcomes of prostaglandin (PG) labor induction using a retrievable vaginal insert. Study design: This is a secondary analysis of data collected during the Misoprostol Vaginal Insert Trial (Miso-Obs-004), a multicenter, double-blind, randomized controlled trial of women undergoing induction of labor with PGs. The duration, characteristics and outcomes of labor were compared in women with and without diabetes. Multivariable regression analysis was performed on all outcomes of interest, adjusting for differences in baseline characteristics. Results: There were 122 women with diabetes within the sample of 1275 women who delivered during their first admission. The time to reach active labor was significantly prolonged among women with diabetes compared with those without (22.0±13.0 vs 18.5±11.1, P=0.008) as was the time to delivery (30.2±15.0 vs 26.0±12.6, P=0.004). Fewer women with diabetes delivered within 36 h (adjusted odds ratio: 0.41, 95% confidence interval: 0.26 to 0.66, P=0.0003) and 48 h (adjusted odds ratio: 0.36, 95% confidence interval: 0.19 to 0.71, P=0.004). These relationships were significant after a multivariate regression analysis of baseline characteristics that adjusted for age, race, parity, body mass index, baseline modified Bishop Score, gestational age at induction and treatment group allocation. Conclusion: After PG labor induction, women with diabetes took longer to reach active labor and to deliver. Objective The objective of this study was to evaluate the relationship between the simplified Bishop score (SBS) on admission for labor and subsequent labor outcomes to identify women at higher risk for cesareans. Study Design This was a secondary analysis of a prospective cohort study of 4,733 singleton pregnancies. Adjusted odds ratios (aOR) were calculated comparing outcomes in women with an unfavorable SBS ≤ 5 to women with a favorable SBS > 5. A favorable SBS was compared with the individual parameters of dilation, effacement, and station. The primary outcome was vaginal delivery. Secondary outcomes were prolonged first stage, completion of first stage, oxytocin augmentation, and prolonged second stage. Results 47.8% of the patients admitted in labor had an unfavorable SBS. Nulliparous and multiparous patients with a favorable SBS were more likely to have a vaginal delivery (aOR 1.96, 95% confidence intervals [CI] 1.49–2.57; aOR 1.91, 95% CI 1.44–2.53) and less likely to require oxytocin augmentation (aOR 0.34, 95% CI 0.28–0.42; aOR 0.26, 95% CI 0.22–0.30. Compared with dilation alone, the SBS in its entirety was associated with a higher likelihood of vaginal delivery in nulliparous. Conclusion An unfavorable SBS on admission for labor is associated with a decreased likelihood of having a vaginal delivery.

Objective: Our objectives were to confirm the predictiveness of parity for successful labor induction and propose an improvement in the Bishop's score to take parity into account and simultaneously simplify the original Bishop score. Study design: Retrospective study of 326 deliveries induced by oxytocin and amniotomy before prostaglandins between January 1, 1987, and June 30, 1988. We conducted a univariate and then a multivariate analysis of the relation between successful labor induction - defined by vaginal delivery- and the components of Bishop's score and parity. Results: Nulliparous accounted for 38% of the studied population. The mean Bishop at induction was 5.75±1.4. Fetal station, cervical effacement, and parity were the only factors associated with the success of induction in this study. Removing the cervical position and consistency from the score as well as adding parity significantly improved the prediction of success (ROC curves, AUC 0.88 vs 0.68, p<0.001). By taking 5% as the maximum risk of induction failure, a cutoff point of 4 for the new score makes it possible to induce labor in 90% of the women that were considered in the study (vs 26% or 60%, according to whether the cutoff point of the original Bishop's score is set, respectively, at 7 or 6, p<0.001). Conclusion: Cervical position and consistency are not necessary for predicting successful labor induction by oxytocin and amniotomy. We confirmed the usefulness of a simplified Bishop score that considers parity.
