Чистая инфекция и инфекционные заболевания: соотношение понятий

Abstract

Уточнение понятий и терминов инфекционной экологии. Дефиниции ориентированы на новый цикл полевых и экспериментальных исследований. Стандарт инфекционной экологии связан с детальным исследованием территорий на основании полигонов и полу-полигонов. Первый уровень: чистая инфекция, нулевое время инфекции, эпигеосистема, естественная география микроорганизмов. Согласно S-Theory, чистая инфекция является результатом проявления физико-химических сигналов среды, ведущих к дисбалансу вызывающему адаптационные реакции экосистемы микроорганизма. Второй уровень: инфекционные заболевания особей различных биологических видов. На втором уровне представлены: единичные заболевания особей определенных видов, массовые заражения особей определённых биологических видов, инфекционное цунами. Причины массовой инфекционной заболеваемости носят полицентрический характер. Инфекционные заболевания различных биологических видов (второй уровень) рассматриваются нами как результат наложения первого уровня инфекции и ареалов распространения определенных биологических видов. Первый уровень проявления патогенности микроорганизмов + ареал распространения видов = конкретная география инфекционного заболевания. Нужно рассматривать полную связку «патогенный микроорганизм – протозоа – насекомые – теплокровные». Развитие инфекционного процесса определяется экологической организацией указанных уровней и спецификой физико-химических сигналов среды. Третий уровень: популяционная диффузия инфекционных заболеваний, диффузионная инфекционная уязвимость. Система терминов для описания феномена инфекции, громадного пласта информации по палеопатогенике, современным инфекционным заболеваниям и инфекционным процессам обязательно должна строиться на многопараметрической основе.

Z-culture has a wide variety of expressions. It is connected not only with the Russian Federation and its population. Its manifestations can be seen everywhere outside the Russian Federation. This is not only connected with the “Russian world”. Z culture speaks different languages. For her, some characteristic features can be noted. Z-culture creates its own reality. People of the Z-culture show some amazing determination and perseverance. There was a kind of "glassy" look in the eyes. Reminds me of Voodoo religion. There is something in common. Senseless and looking somewhere far away. A person can say completely crazy and obviously not acceptable words. Complete loss of habitual knowledge of causal relationships. In the Z-culture, the "brakes" have disappeared. This culture creates many problems in the economy. There was no such thing. Let us consider only one particular case. It is associated with international pharmaceutical companies operating in the post-USSR. The war in Ukraine in 2022 was a shock to everyone. Including for international business, one way or another connected with Russia and Ukraine. Things got really bad in 2014. Since February 24, 2022, the situation has deteriorated rapidly. The position of international companies in Russia has become even more difficult. Strictly speaking, a large international pharmaceutical company rarely "lives without war." There are armed conflicts in many places in the world. Often they are of a protracted nature. A particular difficulty is associated with latent regional conflicts. Crimea and Donbass were such latent conflicts. War is war, but are the pills on schedule? How to be pharmaceutical companies during such a war? For a long time, many billions of dollars have been invested in the economy of a particular country. A company cannot pack up and leave the country in 5 minutes. Who will compensate for the losses? Pharmaceutical companies are about drugs. Thousands of diseases have been described. Because of the dictator and a certain number of his Z-supporters, should the sick people of the Russian Federation be doomed to death? In the media, the performance of pharmaceutical companies is almost always evaluated only in terms of profit. It is not correct. The work of international pharmaceutical companies raises many issues. They need to be properly formulated and discussed.�

Conclusions are made on the "reformatory" activity within the framework of dogmatic epidemiology. A critical analysis of dogmatic epidemiology is essential. Underestimating the bioterrorist capabilities of nature and overestimating the bioterrorist capabilities of Homo sapiens lead to numerous cognitive limitations and failures. The COVID-19 will inevitably lead to attempts to reform dogmatic epidemiology. Many people understand the categorical inability of the dominant paradigm to explain massive infectious processes. The fact that, when explaining COVID-19, in dogmatic epidemiology, species were again declared as host is not at all accidental. There is no attempt to really understand what is happening. This is a simulated scientific explanation. Fundamental questions about the scientific explanation of the pandemic are not correctly formulated. As usual, it all comes down to a lot of particulars and clinical manifestations of a new infectious disease in humans. This has nothing to do with the study of the pandemic process. Including the study of its earliest manifestations. An attempt to replace vertebrates with invertebrates and protozoa, understood as a species - reservoir for pathogenic microorganisms, in principle, changes nothing in the dominant paradigm. Within the framework of infectious ecology, the idea of studying pathogenic viruses, in the context of invertebrates and vertebrates, is considered in completely different terms. The emphasis is on the fundamental idea of pathogenicity as an adaptive response of microorganisms to the physicochemical signals of their natural environment.

Dogmatic epidemiology and its expert community are not the kind of frozen in time phenomenon. The expert community is very large and well organized. It has military discipline. The dominant paradigm not only actively suppresses scientific alternatives in understanding pathogenicity and infectious diseases. It should itself demonstrate success in countering infectious diseases. Nature plays infectious jazz. Infectious novelties must be answered. The "Saviors of Humanity" must demonstrate their "superpower". This should be done within the framework of very strictly defined cognitive limitations of dogmatic epidemiology. The situation gives rise to numerous problems. For this reason, dogmatic epidemiology is periodically attempted to be reformed. The "reformist" activity of Dr (PhD), colonel M.V. Supotnitskiy is systematically analyzed. An authoritative military epidemiologist from the "27th Science Center". The contradictory nature of his approach to "reforming" is shown. It is perfectly normal practice for such "reforms" to obstruct alternative scientific points of view. They must be categorically ignored. In the “reformist” activity of M.V. Supotnitskiy, numerous cognitive limitations of dogmatic epidemiology are manifested. The result of the "reform" is frankly poor. However, it has an important meaning. This kind of activity is imitative. It is important in itself.

In pathogenicity survey the levels of a) microorganisms with pathogenic properties, b) types of terrain that contribute to the manifestation of pathogenic properties of microorganisms and the occurrence of infectious diseases, and c) infectious diseases themselves are often confused. Most often, an attempt is made to survey an infectious disease. To do this, it is associated with certain types of terrain. These types of terrain can be described in completely different terms (landscapes, ecotones, geosystems, or it is not clear what). Many pathogen experts consider this issue very unprofessional. It plays an "auxiliary" character and does not see any scientific difficulties in him. In infectious ecology, the geography of microorganisms with pathogenic properties is clearly defined as an independent level of organization of nature and the object of research. To develop the geography of pathogens as a new scientific direction, a new system of terms and concepts is being introduced. With all the simplicity of these theses, they can be correctly interpreted only under the condition of a strictly defined understanding of the levels of manifestation of pathogenicity in nature. Everything is determined by the fundamental understanding of the phenomenon of pathogenicity. There is pure infection, zero time of infection, EGS and much more. The terminology that speaks of infection in its pure form, and not only in the form of an infectious disease, must be mastered. This is a key point in understanding the new approach. I proceed from the S-Theory understanding of pathogenicity. The idea of an infectious "minefield" is being developed, which is activated and gives various outbreaks of pathogenicity, as well as periodic infectious diseases of one nature or another. The pathogenicity of microorganisms is a reaction to the physicochemical signals of their natural environment. When these signals go beyond a certain chemical norm and begin to bear an unusual character, they can cause adaptive reactions. This is the basis for the potential emergence of infectious diseases of various types.

The COVID-19 pandemic has spawned a vibrant scientific community. Hundreds of thousands of various publications reacted to it. New communities were quickly formed and engaged in active work. The pandemic was disassembled into many different and completely unrelated parts. The manifestation of a natural process is considered in dozens of fragments. This provides a very large body of knowledge regarding the shards and a disastrous lack of answers to fundamental questions. There is no way to withdraw the expert community from such a variant of the study of COVID-19. Everything is done by experts spontaneously and very professionally. They are based on their interests in maintaining and improving their own status in the expert community. The status of the organization is also important. A new round of usual battle of experts for resources begins. It was the resources generated by the pandemic that became the basis of violent activity, and not an attempt to understand the infectious novelty of nature. The vast majority of these studies are conducted from the standpoint of the dominant paradigm. It is not questioned in any degree. Alternative fundamental scientific approaches are naturally ignored. They are not recognized as scientific. They are neither seen nor heard. These approaches do not make commercial sense and it is natural to ignore them. It's not about understanding a pandemic. It's about the earnings of experts and their organizations on the pandemic. From the point of view of science about science, the current situation is not new. It has been described many times in the history of science. Probably, it is most correctly described in T. Kuhn's concept. This is very sad because a reasonable way out of this situation is highly questionable. We see it only in the creation of a new scientific discipline (infectious ecology). It is not customary to doubt the limitations of scientific knowledge. It is limitless. But there is good reason to doubt the infinity of the cognitive possibilities of paradigms. Modern research on the COVID-19 clearly demonstrates this thesis. The limitations of scientific knowledge must be sought in the specifics of the work of the expert community. It is the # 1 limiter. Experts from the Journal of «Environmental Epidemiology» take an infectious ecology-based approach. In particular, S-Theory and the adaptation hypothesis of the COVID-19, developed on its basis, are of great importance. This hypothesis can be developed and verified on the basis of experimental infectious ecology.

Nature plays an infectious jazz. The phenomenon of pathogenicity of microorganisms is variable in nature. In particular, the manifestation of infection changes significantly in geological time. Depending on the initial theoretical and methodological attitude of the experts, they see something significantly different in infectious processes. It is for this reason that there are many different concepts in the scientific understanding of infection. The parameters of a comparative analysis of some conceptual points of view on the study of the manifestation of the pathogenic properties of microorganisms in the natural environment have been determined. 53 parameters were taken into account. The comparison is carried out according to the following conceptual approaches: 1. Dogmatic version of landscape epidemiology E.N. Pavlovsky, 2. The apologetic version of landscape epidemiology K.N. Tokarevich / Leningrad group, 3. Reformed version of V.L. Adamovich / Bryansk group, 4. The concept of epidemic chains of I.S. Soldatkina / Saratov group, 5. Microelement (ecological) concept of E.V. Rothschild, 6. S-Theory D. Nikolaenko. The work is part of the solution of a more general problem of developing the theoretical and methodological foundations of infectious ecology. The text was written in 2010-2014. Minor editorial changes were made to the text in 2021. Ideas were discussed many times in the course of various scientific projects and in the process of communication with colleagues. The following expert reaction is typical: surprise at the abundance of concepts and a complete lack of interest in this diversity. Due to the duration of the comparative analysis (more than 10 years), some contradictions have accumulated in the text. They are not eliminated. It's too late to do this in 2021. The text is published for the first time.

The first systematic presentation of S-Theory. It was defined as the theory of an "infectious sandwich". Infection is treated as a discrete property. There is no constant presence of infection in nature. It is an optional property. Microorganisms with pathogenic properties react to certain signals. Signals can be different. For example, they may be associated with the dynamics of trace elements of soil environments. New concepts have been introduced. An example may be the concept of an epigeosystem (EGS). The phenomenon of infection is considered as an adaptive property of EGS. RUSSIAN VERSION. Первое систематическое изложение S-Theory. Она была определена как теория «инфекционного бутерброда». Инфекция рассматривается как дискретное свойство. Нет постоянного присутствия инфекции в природе. Она есть факультативное свойство. S-Theory about ten years. The current version is defined as 3.0. The theory, like computer programs, has its own versions. Full name: "Sandwich Theory of Infection". Short name: "S-Theory". "S" means "Sandwich". This is not "Super". This name may not be the most successful, but it reflects quite important features of the theoretical design. In nature, there are certain ingredients. They are periodically compiled by nature and give a new quality, which is called "infection." This is a certain attribute of nature. It is a temporary nature. In contrast to the microorganism, infection in nature is not always present. It is created by a certain natural situation. It is not an object. It is a short-term attribute. In theory, its own terminology. There are clear definitions of terms. An example may be the concept of an epigeosystem (EGS). Theory terminology clearly aimed at making a geostationary research. The collection of quantitative information was originally defined in theory. It's not just speculation. This is a theory for working with devices on micro-polygons. An important role is played by typological analysis. Nature plays an infectious jazz. We can understand it on the basis of typological analysis. The number of types of discrete activation of pathogens can be determined differently. Much depends on the criteria that are introduced by experts. A big topic in the theory is related to the description of diffusion infectious processes. In theory, the usual terms "epizootic" and "epidemic" are not used. It introduced its own terminology. Including it is associated with diffusions, which are relevant specifically in the population of people. An example is the spread of HIV/AIDS. The space and time of a given diffusion process are described. On the basis of S-Theory, consecutive interpretations of a number of infectious diseases are given. An example may be tularemia and anthrax. These are characteristic infectious diseases. There are many studies on them. Any expert can compare S-Theory approaches and those that are implemented on the basis of the dominant paradigm of modern epidemiology. Feel the difference. Sometimes this is helpful. S-Theory is the theoretical basis of Infectious Ecology. It is important to understand that infectious ecology may have other theoretical grounds. The object of it research is incredibly complex. This connection of the living and not live in nature, the result of which is the phenomenon of infection. Such a complex object cannot be correctly considered from one theoretical position only.

New fundamental knowledge on the manifestations of the pathogenicity of microorganisms cannot be bought for money. It is not a commodity by definition. No matter how rich the world's pharmaceutical company, or even a consortium of all the world's pharmaceutical companies, will not be able to generate new fundamental knowledge regarding pathogenicity. They are focused on the production of goods. The development of vaccines against new infectious diseases is a very important area of research. But we need to understand the fundamental limitations of this approach. This approach solves very specific and far from unambiguous tasks. It does not provide an understanding of the natural process of manifestation of pathogenicity. No matter how much money is invested in the dominant paradigm, it will give only those results that do not contradict its fundamental foundations. These can be impressive particulars. It is not the business of the dominant paradigmists to create killer alternatives for themselves. They produce only that knowledge that confirms the right to an infinite monopoly of their paradigm. Science and commercial interest in the study of the pathogenicity of microorganisms have come together. Fundamental knowledge can only emerge from cognitive activity. It is not inherently profitable. The situation is even worse. For a deeper fundamental understanding of the nature of pathogenicity of microorganisms, it is necessary to leave the platform of dogmatic epidemiology. We need to develop new scientific directions. An example would be Infectious Ecology. New fundamental knowledge, which may be the basis for a fundamentally new understanding of the pathogenicity of microorganisms, already exists. It is possible to indicate at least a dozen experts and research groups who have made more or less successful attempts to formulate a new understanding of pathogenicity. In all these cases, new fundamental knowledge is not a commodity. It is not for sale. If it is obtained as a result of projects, then it appeared contrary to what was written in their justification. In the vast majority of cases, such knowledge is obtained only in the "order of personal initiative." At the moment, the expert community ignores the available array of alternative fundamental knowledge on pathogenicity. A change in the situation in science can lead to a prompt change in the dominant ideas about pathogenicity. There are many examples of this kind of radical change.

Landscapes and manifestations of the pathogenic properties of microorganisms should be linked. This is the opinion of a large number of experts. There is a direction called "landscape epidemiology". It exists for about a hundred years. The appearance of the GIS gave an outburst of activity in the search for the connection of landscapes and the pathogenic activity of microorganisms. Based on the GIS, repeated attempts were made to search for these spatial regularities. Various databases were being processed. The results of these efforts are frankly not convincing. In the article the definitions of the concept of landscape are considered. There are a lot of them. A methodical demonstrative example of GIS for processing data on anthrax (example of the Crimea) is given. The theoretical and methodological issues of using GIS to formulate and solve problems of this kind are analyzed. The conclusion is very pessimistic. On the basis of the landscape approach, it is difficult to understand anything in the spatial & temporal dynamics of the manifestation of the pathogenic properties of microorganisms. It's not the lack of this dynamics and its patterns. The reason is in the most landscape approach. It cannot be an effective basis for investigating the geography of manifestation of the pathogenic properties of microorganisms. Semantics is a large and complex direction of scientific research. The more complex the object of research, the more meaningful can be the semantics of scientific research. Investigation of diffusion infectious processes makes it extremely accurate to work with semantics. The definition of what is semantics is considered. The main part of the article is devoted to cartosemantics. Its founder was Dmitry Nikolaenko. The first works were carried out on the basis of processing a huge array of data on anthrax in Russian empire. The role of an observer in GIS data processing is shown. Cartosemantics has excellent prospects in the context of infectious ecology. Its standard works much more carefully with empirical information on natural environments than it does in modern epidemiology. A correct explanation of the diffusion process for HIV/AIDS, tuberculosis and hepatitis C requires a system of concepts. The article gives an example of development such a system for describing the phenomenology of infectious changes. The system of concepts was first published by Dmitry Nikolaenko in 2005. Later, its modifications were made. Ten definitions are given. Provides examples of their use in the interpretation of the empirical data. The work is focused on preparation for the GIS project. In diffusion of HIV/AIDS process, tuberculosis and hepatitis C there is relative (characteristic) space and time. The development of this process is categorically not reduced to quantitative changes only. Understanding the "infectious counter" can be done on the basis of a description of the characteristic space-time-structure only. Phenomenology is the categorically necessary direction of these studies. Including, on GIS. The system of concepts developed by Dmitry Nikolaenko is analyzed. It allows us to describe the phenomenology of the diffusion infectious process. The new system of concepts gives completely new understanding of infectious changes. Ignoring the problem will lead to the fact that next GIS will reduce to a purely formal processing of medical indicators. This approach does not provide anything for a scientific understanding of diffusion infectious processes. It does not give anything and to develop of effective prevention. For a correct description of multiparametric infectious diffusion processes, it is necessary to introduce a system of coefficients. It is impossible to be limited to the processing of standard medical statistics only. We are dealing with a massive space-time process. It has his own patterns. To understand them, correct monitoring should be established. GIS description of the process is its component. Seven coefficients are considered. They help to describe the diffusion infectious process from different points of view. A case has been developed for the specification of urban areas. The most effective GIS will be in description such megacities as Chicago or Los Angeles (USA). This is due to many reasons. The case of the post-Soviet space is also considered. Something very unusual is happening here. The HIV prevalence of the population has already risen up to 9% of the total population. This is the case of Orsk (Russia). We are based on the morphological concept of Dmitry Nikolaenko. It is very successful as a basis for prediction of the mass space-time infectious changes.
