Mapping of immunization coverage using Geographic Information system (GIS) technology in urban slums of central Karnataka, India

Abstract

Introduction: Immunization is an essential component of public health programs and the most cost effective intervention. Despite having a robust and well-functioning health system in India some states face reduction in immunization coverage which needs to be improved. Objectives: In urban field practice area of a tertiary care teaching hospital - To assess the immunization coverage among children aged 6-23 months and To assess the use of Geographic Information system (GIS) technology as a tool for spatial mapping of the vaccine coverage Method: A cross sectional study was done among 6-23 months children residing in the urban slums in the field practice area of tertiary care teaching hospital. Data was collected by house to house visits with interview of mother/primary caregiver using pre-tested semi structured questionnaire. QGIS software was used for spatial mapping of the immunization coverage. Result: A total of 374 children aged 6–23 months were part of the study. It was found that the completely immunized, partially immunized, and unimmunized children were 78.3%, 16.9%, and 4.8%, respectively. The most common cause for partial immunization and non-immunization was child being ill and the lack of information, respectively. Spatial mapping of immunization coverage helped us in identifying the urban areas with low immunization coverage. Conclusions: Vaccine coverage in the area needs to be improved more which can be done by educating the mothers/primary caregivers and motivating them for better health of their children. GIS can be used as a useful tool in mapping immunization coverage.

Introduction: Immunization programme has been a major cost-effective public health intervention for child survival. Despite implementation of immunization programme since 1978 by Govt of India (GOI), the coverage of immunization is not satisfactory due to various socio economic factors influencing vaccination coverage. Aim: To determine the coverage and to identify the various factors influencing primary immunization in children. Methods: Hospital based cross sectional study was conducted from January 2012 to September 2013 by interviewing parents of 2000 children of 1-2 yr age attending OPD and Children`s ward in Govt General Hospital, Kakinada, AP. Socio demographic factors and details of immunization status of the children and the reasons for dropouts were noted. The child who received all primary doses of BCG, DPT, Hepatitis B, OPV and Measles mentioned in National immunization schedule is considered fully immunised. Child who missed even a single dose and who did not receive even a single dose of any vaccine is considered partially immunised and un-immunised respectively. Percentages and Chi square test were used for statistical analysis. Results: Overall, 1810 were fully immunised (90.5%), 186 were partially immunised (9.3%) and 4 were unimmunised. Type of family and education of parents had a significant role in the immunization coverage of children whereas gender, religion and residence of children were not found to have significant impact on immunization status. Unwell child, lack of knowledge and migration to other places were found to be major contributing factors for poor immunization. Conclusion: Regular health education of all sections of the community especially the mothers to increase awareness of childhood immunization and addressing the issues of dropouts and poor accessibility is urgently required for better and equitable coverage of across all regions of the country.

Socioeconomic scales are being used for determining the socio-economic status of study subjects which in turn is important indicator of health and nutritional status of an individual. Since BG Prasad socio-economic classification is an income-based scale and, therefore it demands constant update to take inflation and depreciation of rupee into account. Among all socio economic scales used in India BG Prasad socio-economic classification is one of most frequently used scale. BG Prasad scale was originally devised in 1961 and was later modified by himself in 1968 and 1970. These details of the calculations involved will help many researchers to calculate specific income categories for their ongoing and prospective research work in current calendar year. State-specific CPI (IW) should be used to determine more accurate income categories by researchers in community health-related studies. © 2019, Indian Association of Preventive and Social Medicine. Background & objectives: The global positioning system (GPS) technology along with Google Earth is used to measure (spatial map) the accurate distribution of morbidity, mortality and planning of interventions in the community. We used this technology to find out its role in the investigation of a cholera outbreak, and also to identify the cause of the outbreak. Methods: This study was conducted in a village near Bengaluru, Karnataka in June 2013 during a cholera outbreak. House-to-house survey was done to identify acute watery diarrhoea cases. A hand held GPS receiver was used to record north and east coordinates of the households of cases and these values were subsequently plotted on g0 oogle e0 arth map. Water samples were collected from suspected sources for microbiological analysis. Results: A total of 27 cases of acute watery diarrhoea were reported. Fifty per cent of cases were in the age group of 14-44 yr and one death was reported. GPS technology and Google Earth described the accurate location of household of cases and spot map generated showed clustering of cases around the suspected water sources. The attack rate was 6.92 per cent and case fatality rate was 3.7 per cent. Water samples collected from suspected sources showed the presence of Vibrio cholera O1 Ogawa. Interpretation & conclusions: GPS technology and Google Earth were easy to use, helpful to accurately pinpoint the location of household of cases, construction of spot map and follow up of cases. Outbreak was found to be due to contamination of drinking water sources.

Introduction: The government of Karnataka has introduced a new vaccine, i.e., pentavalent vaccine (PVV) in the Universal Immunization Programme (UIP) schedule from 1st April, 2013. There are no published studies on vaccination coverage of this new vaccine. Objectives: 1. To assess the coverage of PVV of infants in a rural area. 2. To find out the reasons for immunization failure. 3. To describe the use of Global Positioning System (GPS) technology and Google Earth as new tools for spatial mapping the vaccine coverage. Methodology: This exploratory study was conducted in September, 2014 by covering three rural Primary Health Centers near Bangalore by a team of field investigators. The population studied was children aged between 6 and 23 months at the time of the survey. WHO's standard EPI 30 cluster sampling technique was used for assessing the vaccine coverage. GarminGPS72H, a hand held GPS receiver, and Google Earth were used for spatial mapping the vaccination coverage. Results: A total of 210 children aged 6-23 months were included in the study. It was found that the completely immunized, partially immunized, and unimmunized children were 93.3%, 4.3%, and 2.4%, respectively. The most common cause for partial immunization and non-immunization was child being ill and the lack of information, respectively. Spatial mapping of vaccination coverage described the immunization coverage in the area and also gave insight into the probable reason for partial/non-immunized children. Conclusions: Coverage of PVV was very high and vaccine was well accepted by the community. Introduction: It is very important to analyze the factors which acts as obstacle in achieving 100% immunization among children. Lot Quality Assurance Sampling (LQAS) is one of the effective method to assess such barriers. Aim: To assess the full immunization coverage among 12 to 23-month old children of rural field practice area under Department of Community Medicine, Agartala Government Medical College and identify the factors for failure of full immunization. Materials and methods: A community based cross-sectional study was conducted from November 2013 to October 2014 on children aged 12 to 23 months old of area under Mohanpur Community health centre. Using LQAS technique 330 samples were selected with multi-stage sampling, each sub-centre being one lot and two calculated to be the decision value. Data was collected using pre-designed pre-tested questionnaire during home visit and verifying immunization card and analysed by computer software SPSS version 21.0. Results: The full immunization coverage among 12 to 23 months old children of Mohanpur area was found as 91.67%. Out of all the 22 sub-centres, 36.36% was found under performing as per pre-fixed criteria and the main reasons for failure of full immunization in those areas are unawareness of need of subsequent doses of vaccines and illness of the children. Conclusion: LQAS is an effective method to identify areas of under-performance even though overall full immunization coverage is high.

The application of geospatial data to public health problems has expanded significantly with increased access to low-cost handheld global positioning system (GPS) receivers and free programs for geographic information systems analysis. In January 2010, we piloted the application of geospatial analysis to polio supplementary immunization activities (SIAs) in northern Nigeria. SIA teams carried GPS receivers to compare hand-drawn catchment area route maps with GPS tracks of actual vaccination teams. Team tracks overlaid on satellite imagery revealed that teams commonly missed swaths of contiguous households and indicated that geospatial data can improve microplanning and provide nearly real-time monitoring of team performance.

The term vaccine hesitancy refers to delay in acceptance or refusal of vaccines despite the availability of vaccination services. Different factors influence vaccine hesitancy and these are context-specific, varying across time and place and with different vaccines. Factors such as complacency, convenience and confidence are involved. Acceptance of vaccines may be decreasing and several explanations for this trend have been proposed. The WHO Strategic Advisory Group of Experts (SAGE) on Immunization has recognized the global importance of vaccine hesitancy and recommended an interview study with immunization managers (IMs) to better understand the range of vaccine hesitancy determinants that are encountered in different settings. Interviews with IMs in 13 selected countries were conducted between September and December 2013 and various factors that discourage vaccine acceptance were identified. Vaccine hesitancy was not defined consistently by the IMs and most interpreted the term as meaning vaccine refusal. Although vaccine hesitancy existed in all 13 countries, some IMs considered its impact on immunization programmes to be a minor problem. The causes of vaccine hesitancy varied in the different countries and were context-specific, indicating a need to strengthen the capacity of national programmes to identify the locally relevant causal factors and to develop adapted strategies to address them.

Background: Immunization coverage is better in urban than rural areas. However, we anticipate and argue that within the urban areas disparities and inequities persist in immunization coverage and that the socioeconomically disadvantaged, particularly those who reside in slum areas are more vulnerable and may contribute to the lower uptake of immunization. The objectives of the study were to assess the under-five immunization coverage amongst the households in slums of Mangalore taluk and to identify determinants of full immunization uptake among under 5 in the slums population and to know the reasons for non-immunization or partial immunization of children.Methods: Community Based Cross sectional study was conducted in the notified slum areas of Mangalore during August-November 2015. Information was collected from the parents regarding vaccination of their children and sociodemographic variables using a semi-structured interview schedule.Results: 88 (57.7%) of under 5 children were fully immunized, 62 (41.3%) were partially immunized in the slum areas of Mangalore, Karnataka, India. Mean age of the children in our study was found to be 35.15 months. Immunization coverage was found to be associated with the presence of immunization card, occupation of mother, knowledge about universal immunization programme (UIP), knowledge about protection offered by vaccines.Conclusions: Unfortunately, the immunization coverage was found to be very bad in the slum areas of Mangalore city, Karnataka, India. There is an urgent need to ensure that regular health education sessions are conducted. Secondly, slum areas should be the target of RCH programme with special focus on immunization related activities e.g. ensuring that all households with children should have immunization cards, as the slum areas remain the neglected section of the society due to various reasons.
