Automatic washing machine injuries are more commonly associated with minor injuries in the pediatric population but may cause life and limb-threatening adult injuries in rare instances. This case describes a severe upper extremity injury after a schizophrenic patient placed her arm into a running machine. Herein, we describe the management, complex reconstruction, and repair of radial, ulnar, and metacarpal fractures in addition to transected tendons and vasculature. The patient had an excellent functional outcome with minor restrictions in motion and complete recovery of sensation.

Abstract

Introduction
Automatic washing machines have caused traumatic injuries since their production began in 1910 with the
introduction of electric Wringer washing machines [1]. 12.8% of automatic washing machine injuries
reported by the United States Consumer Product Safety Commission involved the placement of an upper or
lower extremity into a running machine [2]. And, 51.9% involved one or more bone fractures [2]. Herein, we
present a case of an adult female suffering from a mangled upper extremity after placing her arm into a
running washing machine. The patient had excellent functional outcome following extensive surgical
reconstruction. We describe the appropriate steps leading up to the assessment and repair of life and limb-
threatening injuries in a rare situation.
Case Presentation
A 59-year-old woman with a history of schizophrenia was brought to the trauma bay by ambulance after
injuring her right upper extremity by placing it into a running automatic washing machine. Extrication took
approximately 20 minutes and paramedics placed a tourniquet proximal to the elbow to stop the bleeding.
On arrival at the trauma bay, the patient was alert, tachycardic, tachypneic, and hypotensive. Right-hand
digits were mottled and cyanotic; and the orientation of the hand to the arm did not appear anatomic,
suggesting a displaced fracture. She was sedated and intubated. Upon removal of the dressing, an open
fracture of the distal radius was visible with active bleeding despite the tourniquet. The tourniquet was
briefly taken down, but no pulsatile bleeding was evident. A radiograph of the right forearm showed a
displaced comminuted radial fracture and markedly displaced oblique fracture of the distal ulna with
fragmentation. She was transported to the operating room for repair.
In the operating room, the wound was further explored (Figure 1), and additional radiographs were taken
(Figure 2). No major vascular injuries were identified and mottling improved when the tourniquet was
removed. She was found to have additional closed comminuted fractures of the right fourth and fifth
metacarpals as well as disruptions of the extensor digitorum communis tendon, extensor digiti minimi,
extensor pollicis longis, adductor pollicis longus, and flexor carpi ulnaris.

The wound was irrigated and debrided of devitalized tissue, and an external fixator was placed to reduce the
radial and ulnar fractures. The metacarpal fractures were fixated with pins. Tendons were primarily repaired,
except for the extensor digiti minimi tendon, which was transferred to the extensor digitorum communis
tendon. The 23cm wound was then closed around the fixator screws with staples, and a splint was applied
(Figure 3). She tolerated the procedure well and was transferred to the trauma intensive care unit for hourly
vascular checks.

The patient’s post-operative course was uneventful. She initially had paresthesia of the right first and
second digits, but sensation fully returned and all digits moved appropriately two days later. She was
discharged on post-operative day 10 with a peripherally inserted central catheter for two weeks of
antibiotics. The patient returned to the operating room two months later for corrective osteotomy of right
ulnar and radial shaft malunion, open reduction and internal fixation of ulnar and radial shafts,
reconstruction of the right radial shaft with bone autograft and demineralized bone matrix, and removal of
implanted hardware from the right hand. At her nine-month follow-up, the patient had recovered well with a
minor limited range of motion of the fingers, hand, and wrist for which she was working with specialized
physical therapists.
Discussion
Modern automatic washing machines have implemented numerous safety mechanisms to prevent injury
during operation, but there are older machines in use, especially among lower socioeconomic populations
[3]. Devastating injuries are uncommon but may require limb salvage techniques when they do occur.
When treating an injured limb, it is vital to treat life-threatening injuries first before addressing the limb,
unless the limb is the source of catastrophic bleeding [4,5] . Bleeding is frequently controlled with pressure;

but there are situations, such as this case, in which a tourniquet is appropriate [4]. Although tourniquets can
cause nerve injury as well as limb ischemia, they have been shown to have a very low rate of complications
when applied in the field prior to hospital admission [6,7]. After relative hemostasis, the wound must be
assessed next to determine the extent of the injury.
Initially, the status of limb perfusion must be determined, and vascular injuries should either be repaired or
temporized using a shunt if the vascular repair would hinder further reconstruction [4]. After the
administration of antibiotics, devitalized tissue should be debrided in the operating room. Timing for
debridement has been controversial, but multiple studies have shown that there is no difference in
outcomes if done within 24 hours [4,7]. In this case, the decision was made to debride immediately due to
mottled digits and bony instability.
Attention then turns to focus on any fractures. An external fixator should be considered when temporary
stabilization of the limb is necessary [8]. We applied an external fixator for stabilization to allow for the
repair of the extensive muscular and tendinous injuries. Bone shortening can be used for devitalized bone
and improved approximation [8]. Bone grafts can be used for defects less than 6cm, and vascularized fibular
bone or cement spacers with negative pressure dressings can be used for bone defects greater than 6cm
depending on the level of contamination [8].
Tendon contracture begins as early as the initial debridement, so tendinous injuries should be addressed as
soon as soft tissue coverage can be guaranteed [8]. Primary repair using nonabsorbable sutures can be
performed if there is no tension; but a tendon graft, commonly using a side-to-side or weave technique, may
be necessary to achieve a tension-free repair [9]. In this case, we were able to primarily repair multiple
tendons; however, we resorted to tendon transfer for the extensor digiti minimi. This is useful for more
extensive tendon injuries and allows for early active motion as the injured tendon is incorporated into a
different functional muscle unit [8]. Tenodesis is a last resort if there are no other options for repair [8].
Although no gross nerve injuries were visualized in this case, nerve injuries must be addressed to restore
function to the limb. Transections can be treated with autograft, most commonly using the sural nerve,
allografts, or conduits for small gaps [8,10]. Timing of nerve repair remains controversial with some sources
recommending repair within the first few days and others recommending a delay of up to three weeks to
allow for fibrosis, which may aid in suture repair [6].
Lastly, the closure must be considered for the wound to heal ultimately; but primary closure is not always
possible. In these cases, free flaps can be beneficial due to increased blood flow to the wounded area and
improved healing secondary to increased antimicrobial properties and nutritional resources [8,10]. Muscle
flaps may also be considered but are less advantageous as they require skin grafting [8,10].
Conclusions
Automatic washing machines are more commonly associated with minor extremity injuries in the pediatric
population; however, injuries can be limb and life-threatening in all patient populations. After ruling out
life-threatening injuries, attention should be turned to the perfusion of the limb followed by the assessment
and repair of bones, tendons, nerves, and skin. This case describes a severe upper extremity injury that
utilized complex reconstructive strategies to salvage a limb successfully with minimal loss of function.

Washing machines are common in most households and crucial for laundry management. However, they are associated with risks, especially in children. In this report, we present three cases of severe hand injuries in pediatric patients that resulted from attempts to unload laundry while the washing machine was still spinning. Fortunately, in all three cases, the injured hands were successfully treated with revascularization of the affected parts. Compared to revision amputation or other reconstructive techniques, finger replantation and revascularization offer the most favorable outcomes. Several preventive strategies, such as more parental supervision and thorough appliance safety training, can reduce the likelihood of such injuries. Increasing awareness and promoting safe practices can drastically reduce the frequency of potentially fatal accidents.

Mangled describes an injury caused by cutting, tearing, or crushing, which leads to the limb becoming unrecognizable; in essence, there are two treatment options for mangled upper extremities, amputation and salvage reconstruction. With advances in our understanding of human physiology and basic science, and with the development of new fixation devices, modern microsurgical techniques and the possibility of different types of bony and soft tissue reconstruction, the clinical and functional outcomes are often good, and certainly preferable to those of contemporary prosthetics. Early or even immediate (emergency) complete upper extremity reconstruction appears to give better results than delayed or late reconstruction and should be the treatment of choice where possible. Before any reconstruction is attempted, injuries to other organs must be excluded. Each step in the assessment and treatment of a mangled extremity is of utmost importance. These include radical tissue debridement, prophylactic antibiotics, copious irrigation with a lavage system, stable bone fixation, revascularization, nerve repair, and soft tissue coverage. Well-planned and early rehabilitation leads to a better functional outcome. Despite the use of scoring systems to help guide decisions and predict outcomes, the decision to reconstruct or to amputate still ultimately lies with the surgical judgment and experience of the treating surgeon.

Washing machines are part of every household and there are various reports of upper extremity injuries due to inadequate safety precautions while operating the machine. Most of the injuries occur when an attempt is made to remove the clothes from the machine and the hand gets caught in the spinning machine. The presentation can vary from minor soft tissue injuries to a mangled upper extremity. The chance of neurovascular damage resulting in compartment syndrome is very high. The author reports three cases of washing machine injuries to draw attention to this not so uncommon injury. The relevant literature is also considered.

To describe washing machine related injuries in children in the United States. Injury data for 496 washing machine related injuries documented by the Consumer Product Safety Commission's National Electronic Injury Surveillance System and death certificate data files were analyzed. Gender, age, diagnosis, body part injured, disposition, location and mechanism of injury were considered in the analysis of data. The upper extremities were most frequently injured in washing machine related injuries, especially with wringer machines. Fewer than 10% of patients required admission, but automatic washers accounted for most of these and for both of the deaths. Automatic washer injuries involved a wider range of injury mechanism, including 23 children who fell from the machines while in baby seats. Though most injuries associated with washing machines are minor, some are severe and devastating. Many of the injuries could be avoided with improvements in machine design while others suggest a need for increased education of potential dangers and better supervision of children if they are allowed access to areas where washing machines are operating. Furthermore, washing machines should only be used for their intended purpose. Given the limitations of educational efforts to prevent injuries, health professionals should have a major role in public education regarding these seemingly benign household appliances.

“ABC” has now been replaced by “ ABC”, where “ ” stands for “catastrophic haemorrhage” ABC has become established as the ubiquitous emergency care paradigm, reflected across the spectrum of advanced life support programmes. Military practitioners have been intuitively uncomfortable with this, as experience and evidence indicate that external peripheral haemorrhage is the leading cause of combat casualty death. In the UK military, ABC has now been replaced by ABC, where stands for catastrophic haemorrhage. The rationale for this change is explained in this commentary, together with its relevance to civilian practice. Military ballistic injury is different from civilian blunt trauma. The nature of ballistic injury varies according to the type of weapon system causing the injury, the type of ballistic protection worn by the casualty and the nature of the conflict (urban, maritime or jungle). Although some injuries are inevitably unsurvivable, death may be avoidable in many cases, with rapid and decisive care at the point of wounding. Mangled upper extremity, as a result of trauma, is a life-altering event requiring a multidisciplinary approach for a successful outcome. All attempts are made to salvage the extremity and preserve function, which may require multiple complex procedures. This paper discusses the importance of a systematic reconstructive sequence and provides a review of commonly utilized techniques, supported with illustrative cases.

Background: Data regarding outcomes after peripheral nerve injuries is limited, and the optimal management strategy for an acute injury is unclear. The aim of this study was to examine timing of repair and specific factors that impact motor-sensory outcomes after peripheral nerve injury. Methods: This was a single center, retrospective study. Patients with traumatic peripheral nerve injury 01/2010 - 06/2015 were included. Patients who died, required amputation, suffered brachial plexus injury, or had missing motor-sensory exams were excluded. Motor-sensory exams were graded 0-5 by the Modified British Medical Research Council system. Operative repair of peripheral nerves was analyzed for patient characteristics, anatomic nerve injured, level of injury, associated injuries, days until repair, and repair method. Results: 311 patients met inclusion criteria. 258 (83%) patients underwent operative management, and 53 (17%) underwent non-operative management. Those who required operative intervention had significantly more penetrating injuries 85.7% vs 64.2% (p<0.001), worse initial motor scores 1.19 vs 2.23 (p=0.004), and worse initial sensory exam scores 1.75 vs 2.28 (p=0.029). Predictors of improved operative motor outcomes on univariate analysis were Injury Severity Score (ISS) <15 (p=0.013) and male sex (p=0.006). Upper arm level of injury was a predictor of poor outcome (p=0.041). Multivariate analysis confirmed male sex as a predictor of good motor outcome (p=0.014, AOR=3.88[1.28-11.80]). Univariate analysis identified distal forearm level of injury (p=0.026) and autograft repair (p=0.048) as predictors of poor sensory outcome. Damage control surgery (DCS) for unstable patients undergoing laparotomy (p=0.257) and days to nerve repair (p=0.834) did not influence motor-sensory outcome. Outcomes did not differ significantly in patients who underwent repair ≤24 hours versus those who were repaired later. Conclusion: Outcomes were primarily influenced by patient characteristics and injury level rather than operative characteristics. Peripheral nerve injuries can be repaired after DCS without detriment to outcomes. Level of evidence: Prognostic Study, Level III.

Objectives: To evaluate the association between time to surgery, antibiotic administration, Gustilo grade, fracture location, and development of deep infection in open fractures. Design: Prospective cohort between 2001 and 2009. Setting: Three Level 1 Canadian trauma centers. Participants: A total of 736 (791 fractures) subjects were enrolled and 686 subjects (93%; 737 fractures) provided adequate follow-up data (1-year interview and/or clinical follow-up >90 days). Intervention: Demographics, injury information, time to surgery, and antibiotics were recorded. Subjects were evaluated using standardized data forms until the fracture(s) healed. Phone interviews were undertaken 1 year after the fracture. Main outcome measures: Infection requiring unplanned surgical debridement and/or sustained antibiotic therapy. Results: Tibia/fibula fractures were most common (n = 413, 52%), followed by upper extremity (UE) (n = 285, 36%), and femoral (n = 93, 12%) fractures. Infection developed in 46 fractures (6%). The median time to surgery was 9 hours 4 minutes (interquartile range, 6 hours 39 minutes to 12 hours 33 minutes) and 7 hours 39 minutes (interquartile range, 6 hours 10 minutes to 9 hours 54 minutes) for those without and with infection, respectively (P = 0.04). Gustilo grade 3B/3C fractures accounted for 17 of 46 infections (37%) (P < 0.001). Four UE (1.5%), 7 femoral (8%), and 35 tibia/fibula (9%) fractures developed infections (P = 0.001). Multivariate regression found no association between infection and time to surgery [odds ratio (OR), 0.97; 95% confidence interval (95% CI), 0.90-1.06] or antibiotics (OR, 1.0; 95% CI, 0.90-1.05). Grades 3A (OR, 6.37; 95% CI, 1.37-29.56) and 3B/3C (OR, 12.87; 95% CI, 2.72-60.95) relative to grade 1 injuries and tibia/fibula (OR, 3.91; 95% CI, 1.33-11.53) relative to UE fractures were significantly associated with infection. Conclusion: Infection after open fracture was associated with increasing Gustilo grade or tibia/fibula fractures but not time to surgery or antibiotics. Level of evidence: Prognostic level I. In cases of digital nerve injury in which nerve ends cannot be approximated without tension, autologous nerve grafts represent the most commonly used method for reconstruction. Recently, interest in synthetic nerve guides as an alternative to grafting has increased. Although several basic science studies have shown promise for collagen tubes, clinical studies of their success in humans are limited. The purpose of this study was to review our early clinical experience with collagen nerve tubes. The authors identified and followed all cases involving digital nerve repair at our institution over a 2-year period. Twelve patients had repair of a digital nerve with a collagen nerve tube during the study period. Two patients were lost to follow-up, and 1 patient had amputation of the grafted finger secondary to complications of other injuries. The primary outcome data points for the remaining 9 patients were the static 2-point discrimination (2PD), Semmes-Weinstein monofilament testing, and a Quick Disabilities of the Arm, Shoulder, and Hand (DASH) outcome survey at final follow-up. Nine patients had follow-up of at least 1 year, with an average follow-up time of 15 months (range 12-22 months). There were no intraoperative or postoperative complications related to the nerve tubes. Using modified American Society for Surgery of the Hand guidelines, 2PD results were good or excellent in 8 out of 9 of patients. Semmes-Weinstein testing results were full in 5 patients, diminished light touch in 2, diminished protective sensation in 1, and loss of protective sensation in 1. Average Quick DASH scores for the group were 10.86 overall, 4.86 for the work module, and 23.21 for the sports/performing arts module. Although the patients in this study are still within the early follow-up period, our initial results compare favorably with those reported in the existing literature for various types of nerve repair and reconstruction, suggesting that collagen nerve tubes might offer a clinically effective option for restoration of sensory function.
