The Effectiveness of Foot Reflexology Massage in Reducing Cortisol Saliva Levels as a Biomarker of Stress for Patients with Cancer

Authors:

Abstract and Figures

Patients with cancer will experience limitations in meeting their daily needs, including psychological problems such as stress. Stress can be assessed through cortisol saliva as a biomarker of stress. Foot Reflexology will help restore the body's energy balance, stimulate relaxation and endorphins, and reduce stress. To determine the effect of foot reflexology to reduce salivary cortisol levels as a biomarker of anxiety for patients with cancer. Adult patients with cancer in stage II and III hospitalized in a Public hospital in Central Java, Indonesia, were invited. Foot reflexology was given once (40 minutes) a day. Saliva collection was carried out two times a day (30 minutes before intervention) and 15 minutes after intervention. Salivary cortisol levels at baseline and post-intervention were measured and compared to determine the effect of foot reflexology. Data were analyzed using the Wilcox on signed-rank and effect size (ES) test. Fifty-five patients (93.2%) participated in this study. We found that the effect size of foot reflexology was 0.82 (ρ = 0.001) with a mean and standard deviation of salivary cortisol levels at baseline and post-intervention were (9.31 ± 5.45 and 6.17 ± 4.55) respectively. These findings indicated the significant effect of giving Foot Reflexology Massage in reducing salivary cortisol levels. Foot Reflexology massage is proven to have a substantial impact on reducing the Salivary cortisol level as a stress biomarker.

Some studies show that dhikr therapy can improve relaxation and can reduce anxiety, but research on dhikr therapy in cancer patients is still not widely done. This study aims to determine the effect of dhikr therapy on reducing anxiety in cancer patients. Methods: This study used a quantitative method with a quasi-experimental-nonequivalent control group design. The study was conducted in April-May 2018, with respondents 20 people in the intervention group and 20 people in the control group. Dhikr therapy is given once a day in the intervention group with a minimum time of 10 min. Anxiety was measured using the Visual Analog Scale for Anxiety. The effect of dhikr therapy was measured using the Mann-Whitney and effect size (ES). Results: There were a statistically significant differences in the anxiety level of patients in the intervention group and the control group, with a value of P = 0.001 (P < 0.05) and ES = 0.87 (ES > 0.5) indicating the influence of dhikr therapy in reducing patient anxiety. Conclusions: Dhikr therapy has a great effect on reducing anxiety in cancer patients.

Mental disorders occur in people in all countries, societies and in all ethnic groups, regardless socio-economic order with more frequent anxiety disorders. Through the process of time many treatment have been applied in order to address this complex mental issue. People with anxiety disorders can benefit from a variety of treatments and services. Following an accurate diagnosis, possible treatments include psychological treatments and mediation. Complementary and alternative medicine (CAM) plays a significant role in health care systems. Patients with chronic pain conditions, including arthritis, chronic neck and backache, headache, digestive problems and mental health conditions (including insomnia, depression, and anxiety) were high users of CAM therapies. Aromatherapy is a holistic method of treatment, using essential oils. There are several essential oils that can help in reducing anxiety disorders and as a result the embodied events that they may cause.

Cancer is becoming an increasingly important health problem in the low- and middle-income countries in the Asia Pacific region, as well as in high-income countries because of ageing populations and changes in lifestyle associated with economic development and epidemiologic transition. This paper reviews the cancer burden and control in the Asia Pacific region (limited to East Asia, South Eastern Asia and Pacific Islands countries, territories and other areas), with relevant information primarily extracted from the GLOBOCAN 2008, Cancer Incidence in Five Continents series and WHO websites. Most low- and middle-income countries have a cancer control strategy and/or an action plan; however, coverage of cancer registration is still very low and does not meet the international standard in terms of quality. Therefore, only limited data were available for the recent global estimation of cancer burden. Large variations, in both cancer incidence and mortality, were observed in the populations in the different sub-regions of Asia. The most common cancer in males is lung cancer in the Eastern and South Eastern sub-regions, while prostate cancer comes close to lung cancer in the Pacific Island countries. In females, breast cancer is the most common in all three regions. The predominance of lung, stomach, colorectal, prostate, breast and cervical cancers makes cancer control more amenable in the Asia Pacific region. Up-to-date statistics on cancer occurrence and outcome are essential for the planning and evaluation of cancer control programmes. Priority can be given to population-based cancer registration, risk reduction, especially tobacco control, and primary health care based enhancement of health care systems to diagnose and manage cancer specifically in low- and middle-income countries.

This narrative review provides an overview of the scope of psychosocial distress and stress in cancer patients and survivors and the potential negative consequences of untreated symptoms. Evidence-based interventions to treat these symptoms are reviewed, beginning with a summary of published clinical practice guidelines, followed by more detailed reviews of the specific integrative interventions with the largest empirical support: cognitive-behavioral stress management, yoga, mindfulness-based interventions, and massage. We also comment on use of natural health products because of their popularity. Finally, we conclude with recommendations to improve the quality of research in integrative interventions for stress management.

Pain management for cancer patients should include pharmacologic and nonpharmacologic interventions. Integrative medicine therapies, such as mind-body practice, acupuncture, massage therapy, and music therapy, have been studied for their roles in pain management. Data from randomized controlled trials support the effect of hypnosis, acupuncture, and music therapy in reduction of pain. Mindfulness meditation, yoga, qigong, and massage therapy, although may not reduce pain per se, can relieve anxiety and mood changes, which are commonly associated with pain. In clinical practice, one should also consider burdens and risks to patients, patient preference, and the presence or absence of better alternatives when making decisions on whether an integrative medicine therapy is of clinical value.

Patients with advanced cancer experience problems and unmet needs. However, we assume that patients with advanced cancer will have more problems and unmet needs in a country with a lower economic status than in an economically stronger country. We studied whether patients with advanced cancer in Indonesia have more problems and unmet needs than a similar group of patients in the Netherlands. We performed a cross-sectional survey. We compared the data for 180 Indonesian and 94 Dutch patients relating to 24 items of the Problems and Needs in Palliative Care-short version questionnaire. We performed descriptive and χ(2) analysis with Bonferroni correction. The prevalence of most physical problems, including pain, was similar in the 2 groups. In Indonesia, financial problems were the most common: 70 to 80% vs. 30 to 42% in the Netherlands. In Indonesia, 25 to 50% of the patients reported psychological and autonomy problems versus 55 to 86% in the Netherlands. The Indonesian group had many more unmet needs for each problem (> 54%) than the Dutch group (< 35%). Apparently, economic and cultural differences hardly influence physical problems. Nonetheless, fewer Indonesian patients reported psychological and autonomy problems than Dutch patients. This difference contradicts our hypothesis. However, we found more unmet needs for professional attention in Indonesia than in the Netherlands, which is compatible with our hypothesis. These simple comparative data provide interesting insights into problems and unmet needs and give rise to our new hypothesis about cultural influences. This hypothesis should be studied in more depth.

Unlabelled: Multiple physiological and psychological regulatory domains may contribute to the pathophysiology of pain in temporomandibular disorder (TMD) and other bodily pain conditions. The purpose of this study was to evaluate the relationship between multisystem dysregulation and the presence of TMD pain, as well as the presence of different numbers of comorbid pain conditions in TMD. Secondary data analysis was conducted in 131 non-TMD (without comorbid pain) controls, 14 TMD subjects without comorbid pain, 78 TMD subjects with 1 comorbid pain, and 67 TMD subjects with multiple comorbid pain conditions who participated in a TMD genetic study. Twenty markers from sensory, autonomic, inflammatory, and psychological domains were evaluated. The results revealed that 1) overall dysregulation in multiple system domains (OR [odds ratio] = 1.6, 95% confidence interval [CI] = 1.4-1.8), particularly in the sensory (OR = 1.9, 95% CI = 1.3-2.9) and the psychological (OR = 2.1, 95% CI = 2.1-2.7) domains, were associated with increased likelihood of being a painful TMD case; and 2) dysregulations in individual system domains were selectively associated with the increased odds of being a TMD case with different levels of comorbid persistent pain conditions. These outcomes indicate that heterogeneous multisystem dysregulations may exist in painful TMD subgroups, and multidimensional physiological and psychological assessments can provide important information regarding pathophysiology, diagnosis, and management of pain in TMD patients. Perspective: The concurrent assessment of multiple physiological and psychological systems is critical to our understanding of the pathophysiological processes that contribute to painful TMD and associated comorbid conditions, which will ultimately guide and inform appropriate treatment strategies that address the multisystem dysregulation associated with complex and common persistent pain conditions.

Compared to the knowledge of physiological processes associated with stress, ill-health, and disease, the understanding of physiological processes associated with positive psychological functioning is lagging behind. The present study aimed to examine the relationships between psychological well-being and physiological indicators, including blood pressure, catecholamines, and cortisol. Initial questionnaires concerning demographic information, work characteristics (decision authority, job demands, and skill discretion), physical symptoms, nicotine use, and Ryff's Psychological Well-Being scales (RPWB) which cover self-acceptance, environmental mastery, positive relations with others, personal growth, purpose in life and autonomy, were completed by a group of highly educated white-collar workers. Subsequent assessments of blood pressure, urinary catecholamines, and salivary cortisol were performed during two separate workdays. Analyses of data from 12 women and 11 men revealed that individuals with high psychological well-being had significantly lower total cortisol output than individuals with low psychological well-being (p < 0.01), while no significant differences emerged for blood pressure and catecholamines. Further, individuals with high psychological well-being had significantly lower levels of general (p < 0.01) as well as musculoskeletal symptoms (p < 0.01). In conclusion, the findings suggest a link between positive psychological functioning and lowered cortisol release. Copyright © 2002 John Wiley & Sons, Ltd.

The diagnosis and treatment of breast cancer can adversely affect quality of life. Here the aim was to determine the effects of reflexology on host defences and endocrine function in women with early breast cancer. Six weeks after surgery for early breast cancer, 183 women were randomly assigned to self-initiated support (SIS), SIS plus foot reflexology, or SIS plus scalp massage. Peripheral blood mononuclear cells and serum were isolated at T1 (6 weeks post surgery; baseline), T2 and T3 (4 and 10 weeks post completion of intervention, respectively). Lymphocyte phenotyping found that CD25+ cells were significantly higher in the massage group compared with the SIS group at T3. The percentage of T cells, and more specifically the T helper subset expressing IL4, decreased significantly in the massage group compared with the SIS group at T3. This change was accompanied by an increase in the percentage of CD8+ T cytotoxic cells expressing IFNγ in the massage group. Natural killer and lymphokine activated killer cell cytotoxicity measurements, serum levels of cortisol, prolactin and growth hormone, and flow cytometric assessment of their corresponding receptors all revealed no significant differences between the three groups of patients. This study provides evidence that the immunological balance of patients can be altered in a potentially beneficial manner by massage. The original trial was registered with the International Standard Randomised Controlled Trial Registry (ISRCTN87652313).
