Die osteopathische Behandlung von intraossären Läsionen. Eine qualitative Inhaltsanalyse von Experteninterviews – Teil 1: Theorie und Forschungsstand

Diesem Artikel liegt eine Bachelorarbeit zugrunde, die sich mit der osteopathischen Behandlung der intraossären Läsion (IOL) befasst. Sie wird in zwei Artikeln publiziert, wovon dies der erste ist. Der derzeitige wissenschaftliche Forschungsstand der IOL umfasst die Pathologie, die Pathogenese, den Phänotyp, die Untersuchung und die Tragweite einer IOL. Erstmals wurde nun die Behandlung einer IOL wissenschaftlich erforscht. Dieser erste Artikel umfasst den theoretischen Hintergrund: eine Reflexion der osteopathischen Behandlung, des Knochens und der IOL.

Zusammenfassung
Im ersten Teil des Beitrags waren Theorie und Forschungsstand der Behandlung der intraossären Läsion (IOL) dargelegt worden [1].
Mithilfe von vier leitfadengestützen Interviews wurde die Behandlung der IOL wissenschaftlich analysiert. Die Auswertung der Daten erfolgte mit dem Datenverarbeitungsprogramm MAXQDA anhand der qualitativen Inhaltsanalyse nach Gläser & Laudel. Das Ergebnis dieser Studie, die ursprünglich im Rahmen einer Bachelorarbeit durchgeführt worden war, zeigt vielfältige Möglichkeiten der osteopathischen Behandlung einer IOL. Hierfür wird die Strukturierung einer Behandlung erläutert, die Behandlung beschrieben, beeinflussende Faktoren genannt und die Behandlung in Relation mit osteopathischen Behandlungsprinzipien gesetzt.
Abstract
While the first part of this article dealt with the osteopathic treatment of the interosseous lesion (IOL) [1], this contribution records the treatment of IOLs with the help of four guideline-supported interviews. The data were evaluated with the data processing program MAXQDA using the qualitative content analysis according to Gläser & Laudel. The result of this study, which was originally carried out as part of a bachelor thesis, shows a variety of possibilities for the osteopathic treatment of an IOL. For this purpose, the structure of a treatment is explained, the treatment is being described, influencing factors named and the treatment put in relation to osteopathic treatment principles.
Andrew Taylor Still, MD, DO, coined the original idea of lesion based on the obstruction of flow of body fluids, but primarily referring to bony structures and more precisely to the spine. Throughout the 20th century, this idea was shaped and developed into the concept of somatic dysfunction, a term that is familiar to both US-trained osteopathic physicians and foreign-trained osteopaths and has been an essential cornerstone of osteopathic practice and teaching. The present historical narrative review offers an overview of the evolution of Still's original lesion concept, major evidence-based models of somatic dysfunction that attempt to explain the clinical findings, and a critique of the concept.
