Wearing old complete dentures can be the cause of various disorders of the stomatognathic system in many patients, including, among others: pain, chewing difficulties, prosthetic instability and aesthetic problems. On examination, the most frequent problems turn out to be related to lesions mainly affecting the fibromucosa, which leads the practitioner to first carry out a preliminary treatment at this level before making a new prosthesis. Thus, this work will describe the most frequent lesions, their etiologies, and the most adapted therapeutic solutions.

[Show full abstract] prosthodontists, a nutrologist, a speech therapist, and a psychotherapist were involved in the treatment of this case. A 52-year-old female patient, Angle Class III malocclusion, with few teeth and extensive maxillary bone loss, attended the dental clinic of the Brazilian Association of Dentistry in Uberlândia. The treatment involved reverse planning, extraction of the dental remnants, calvarial bone grafting, placement of 6 titanium implants (Neodent) in the maxilla (upper jaw) and 5 in the mandible (lower jaw), orthognathic surgery, and installation of implant-supported fixed complete dentures in both jaws. Furthermore, psychotherapeutic and nutrologist’s interventions were necessary during the dental treatment, concluding the treatment with speech therapy. Within the limitations of this case, the multidisciplinary approach proved to be efficient. It promoted the reestablishment of the stomatognathic system functions without compromising nutrition during the periods when it was impossible to wear prostheses for better healing of the tissues.

Various techniques are available to record the condylar guidance; These are axiographic technique and panoramic radiograph followed by programming the semi adjustable articulators. Aim: The aim of the present study was to evaluate and compare the condylar guidance using the conventional technique the gothic arch tracing with that of axiographic technique and the panoramic radiographs. Materials and Methods: The observational study with the clinical set up was carried out over a period between October 2018- March 2019. Twenty completely edentulous patients were selected for the study. The comparative evaluation of Axiographic tracing technique, Panoramic radiographic technique and the Extraoral gothic arch tracing technique was conducted. The horizontal condylar guidance values obtained with these techniques were assessed. The data obtained was statistically analysed using Karl Pearson correlation test to determine the correlation between the values obtained from all the three techniques on the right side and the left side of the subjects. Paired t-test and Bonferroni Post-Hoc test were used to compare the Extraoral gothic arch tracing values and Orthopantomogram values and axiographic tracing values on right side and left side of the subjects. Results: A statistically significant correlation was observed between the extraoral Gothic arch tracing technique and the Panoramic radiographic technique on right (r=0.047, p-value 0.04) and left sides (r=0.55, p-value 0.01) of the subjects. The axiographic tracing showed lower values when compared with gothic arch tracing technique and the Panoramic radiographic technique (95%CI of difference was -0.49 lower). When an immediate complete denture is fabricated, esthetics plays a fundamental role and thus the assembly of artificial teeth can be performed maintaining the same position, alignment and arrangement of the remaining anterior teeth, providing a natural and esthetic appearance to the denture, thus the transition from the dentate to the edentulous state is less noticeable. This paper reports the case of a patient who needed oral rehabilitation with an immediate upper complete denture and presented favorable smile esthetics of the anterior teeth, which allowed the preservation of alignment, position and arrangement of natural teeth during the assembly of artificial teeth, maintaining and preserving the esthetic individuality and facial harmonization, meeting the patient’s desire and expectations.
