Racism Fatigue

Racism and its concomitant circumstances have created atmospheric pressure that leave individuals feeling exhausted. This racism fatigue has become more apparent to me over the past few months, than ever before; and there was no “Aha” moment. It has been a long, slow evolution.

[Show full abstract] challenge of implementing and evaluating healthcare practices and approaches that effectively and respectfully meet the demands of diverse populations in need of serious illness care. Moreover, care for diverse patients and families with life-limiting illness is compromised when there is a lack of understanding about culturally bound treatment preferences and or distrust for the healthcare system. This chapter recommends cultural competence in advance care planning as a means for equipping social workers with the tools needed to engage individuals and family members in conversations early and increase the utilization of advance directives among diverse populations.

Promoting Advance Directives among African Americans: A Faith-Based Model

Studies show that African Americans are less likely than other ethnic groups to complete advance directives. However, what influences African Americans' decisions to complete or not complete advance directives is unclear. Using a faith-based promotion model, 102 African Americans aged 55 years or older were recruited from local churches and community-based agencies to participate in a pilot study ... [Show full abstract] to promote advance care planning. Focus groups were used to collect data on participants' preferences for care, desire to make personal choices, values and attitudes, beliefs about death and dying, and advance directives. A standardized interview was used in the focus groups, and the data were organized and analyzed using NUDIST 4 software (QRS Software, Victoria, Australia). Three fourths of the participants refused to complete advance directives. The following factors influenced the participants' decisions about end-of-life care and completion of an advance directive: spirituality; view of suffering, death, and dying; social support networks; barriers to utilization; and mistrust of the health care system. The dissemination of information apprises individuals of their right to self-determine about their care, but educational efforts may not produce a significant change in behavior toward completion of advance care planning. Thus, ongoing efforts are needed to improve the trust that African Americans have in medical and health care providers.

Data were gathered by semi-structured interviews in a rural ... [Show full abstract] community in southeastern North Carolina and analyzed using a qualitative content analysis mode. Twenty-six men, age 65+, who were responsible for the care of at least one grandchild, participated. Results. Eighty-one percent (N = 21) reported that their perception of powerlessness occurred either in the role transition or in the activities of daily parenting. Many expected to continue experiencing powerlessness throughout the parenting of the child. Discussion. Grandfathers experience powerlessness that has not been reported in the literature on grandmothers raising grandchildren. Results affirm the need for special attention to elderly men who take on the role of parent for their grandchildren. Key words: grandparents, rural men, grandfathers, parenting, caregivers
