Entgleisungskorrekturregeln für Typ-I-Diabetes: Das A, B, C der Blutzuckerkorrektur

Abstract

Für intensiviert behandelte Typ-I-Diabetiker wiederholt sich täglich die folgende Situation: Nach jeder Blutzuckermessung stellt sich die Frage, ob eine korrekturbedürftige, eventuell erst beginnende Stoffwechselentgleisung vorliegt. Falls ja, muß abgeschätzt werden, wieviel Insulin oder Broteinheiten hierfür zusätzlich eingesetzt und ob weitere absichernde Maßnahmen ergriffen werden sollen. Oft fallen ihnen diese Festlegungen schwer, weil sie sich trotz größter Mühe schon öfter irrten. Unter scheinbar gleichen Voraussetzungen haben sie erlebt, daß ein Blutzuckerwert von z. B. 238mg-% sich einmal mit 3 Einheiten Normalinsulin zu einem kräftigen Unterzucker auswuchs und ein anderes Mal nach 6 Einheiten Normalinsulin weiter anstieg. Eine Diabetikerin sagte mir einmal scherzend nach einer korrekturbedingten Unterzuckerung mit Bewußtseinsverlust, daß das Auswürfeln der Korrekturdosis zu keinem schlechteren Ergebnis führen könnte, ihr aber wenigstens den Stress der Entscheidung abnehmen würde. A diabetic once jokingly told me after a correction-related hypoglycaemia with loss of consciousness that throwing out the correction dose by a dice could not lead to a worse result, but would at least relieve her of the stress of the decision.
