Case Study on Patient with Depression

Abstract:
The need for effective population mental health promotion approaches is urgent as mental health concerns are
escalating globally and current allopathic treatment regimens are insufficient to bring people towards the state
of mental well-being. Successfully alleviating stress has the potential to promote wellbeing and prevent
illness. Worldwide, yoga is gaining popularity as an accessible, acceptable and cost-effective practice for
mind and body. People are turning to yoga for mental health improvement because of preferences for: self-
treatment as opposed to clinical intervention; perceived greater efficacy than medication; fewer side effects;
lack of response to medication. Yoga has minimal side effects and is cost-effective in comparison with
pharmacological treatments and psychotherapy. Yoga’s added benefit is that it improves physical fitness and
encourages self-reliance. Yoga could be considered an ancillary treatment option for patients with depressive
disorders and individuals with elevated levels of depression. In this case study we discuss the evidence for
yoga, naturopathy, diet& acupressure as a form of mental health promotion, illness prevention and treatment
for depression.

Introduction:
Depression is a state of mental illness. It is
characterized by deep, long-lasting feelings of
sadness or despair. Depression can change an
individual’s thinking/feelings and also affects his/her
social behavior and sense of physical well-being. It
can affect people of any age group, including young
Children and teens. It can run in families and usually
starts between the ages of 15 and 30 years. Women
And elderly people are more commonly affected than
men. There are several types of depression such as
Major depression it is a change in mood that lasts for
weeks or months. It is one of the most severe types of
depression. Dysthymia (chronic depression) is a less

severe form of depression but usually lasts for several
years. Psychotic depression a severe form of
depression associated with hallucinations and
delusions (feelings that are untrue or unsupported).
Seasonal depression, occurring only at certain time of
the year usually winter, also known as ‘winter blues’.

There are many types of Depression some are
mentioned below:
Major Depression (Major Depressive Disorder)

Psychotic Depression
Postpartum Depression (Peripartum/Perinatal
Depression)
Premenstrual Dysphoric Disorder

Substance Induced Mood Disorder
Depression due to Illness
Causes: Depression is thought to be caused by an
imbalance of certain brain chemicals called
‘neurotransmitters’ that carries signals in brain which
the body uses to control mood. Some of the common
factors that may cause depression are genetics
(hereditary), trauma and high levels of stress, mental
illnesses such as schizophrenia and substance abuse,
postpartum depression (women may develop
depression after the birth of the baby), serious
medical conditions such as heart disease, cancer and
HIV, use of certain medications, alcohol and drug
abuse, individuals with low self-esteem, trauma and
high levels of stress due to financial problems,
breakup of a relationship or loss of a loved one.
Signs and Symptoms: The signs and symptoms of
depression include feeling of sadness and loneliness,
loss of interest in activities once found enjoyable,
feeling of hopelessness, worthlessness or excessive
guilt, fatigue or loss of energy, sleeping too little or
too much, loss of appetite, restlessness and being
easily annoyed.
Diagnosis: The doctor may diagnose depression
based on the detailed history and sign and symptoms
of the individual. Many a times the individual is
asked a series of questions to help screen/check for
depression symptoms. Specific examinations include
physical examination of the individual such as height
and weight measurement. Examination of the vital
signs such as blood pressure, heart rate and
temperature. Laboratory tests such as blood tests to
screen for alcohol/drugs in blood. Psychological
evaluation of the individual’s thoughts, feelings and
behavior patterns.
Test: Psychotherapy - For mild to moderate
depression, psychotherapy may be the best treatment
option. There are two main types of psychotherapy
commonly used to treat depression. Cognitive-
behavioral therapy (CBT) helps change negative ways
of thinking and behaving. Interpersonal therapy (IPT)
helps people understand and work through troubled
personal relationships that may cause depression.
Complication: If left untreated, depression can lead
to some severe complications such as emotional,
behavioral, health and even legal/financial problems,
relationship difficulties, social isolation and even
suicide.
Case presentation: A 21 years old young Hindu male
student suffered from depression associated with
anxiety since two and half years visited our centre
with the complaints of disturbed sleep, pessimistic
behavior, reduction in weight, reduction in energy,
excessive guiltiness, indigestion, Unable to
Concentrate, Unable to Make Decisions and
Persistent Feeling of Regret from last six months and
even he was on medicine for depression. While
examine his weight is 63.4 kg and blood pressure is
130/70 mmHg. For examine severity of depression
we used Hamilton depression rating scale(HAM-D)-
in that scale he scored ranges between 19-22, that
means it has severe depression according to HAM-D
method.

Intervention: Patient practiced yoga therapy session
of one hour for 6 days per week for thirty days
including loosening exercises, Asanas, pranayama’s,
meditation and Kriyas and with that he followed
acupressure, proper diet and natural remedies as
prescribed.
Duration: 30 days
Frequency: One hour Yogic practices for 6 days a
week followed by proper Diet, acupressure and
natural remedies.
Yogic Intervention:
Kriyas: Kapalabhati – Kapalbhati (3 rounds 120
strokes each)
And other Yogic Kriyas weekly once.
Suryanamaskara- 6 Rounds
Sukshma Vyayama: Medha shakti vikasaka kriya
Sthula Vyayama: Rekhagati, Sarvangapushti
Yogasanas:
Tadasana
Urdhwa Hastottanasana
Ardha Chakrasana
Sarvangasana
Matsyasana
Bhujangasana
Setubandhasana
Pavanmuktasana
Ustrasana
Gomukhasana
Vakrasana
Pranayama:
Nadishodana
Bhramari pranayama
Meditation: Om chanting
Relaxation: Savasana (Deep Relaxation)
Naturopathy management at home:
Hot and cold fomentation over abdomen
Mud pack over abdomen
Abdomen pack
Dietary Regimen:
Advised to take foods rich in antioxidants, including:
● Beta-carotene: apricots, broccoli, carrots, peaches,
pumpkin, spinach, sweet potato
● Vitamin C: blueberries, grapefruit, kiwi, oranges,
peppers, potatoes, strawberries, tomato
● Vitamin E: margarine, nuts and seeds, vegetable
oils, wheat gram
- Carbohydrates rich foods such as Whole grains,
Fruits, vegetables, legumes and fiber.
-Protein rich foods such as beans, peas, low-fat
cheese, milk, soy products, and yogurt.
These foods which are rich in antioxidants,
carbohydrates and proteins helps to prevent cell
damage boost the energy and produce chemical
serotonin.
Acupressure Points given for the symptoms of
depression:
Conception Vessel 17 Point
Lungs 1 Point (LU1)
Three Mile Point (ST36)
Third Eye Point (GV24.5)
One Hundred Meeting Point (GV20)
Union Valley Point (LI4)
Gates of Consciousness Point (GB20)
Objective data:
Patient has followed prescribed yoga practice with
lifestyle modification for one month. He provided
the following information:
-gain weight and feeling energetic
-increased concentration
-reduced anxiety
-feeling more confident
-increase appetite
-sleep become sound
-relieves digestive problem
General examination results:
Weight – 65.5 kg, BP - 116/80, HR - 74/min
Post assessment of severity of depression after the
Yoga, Naturopathy ,Acupressure & Diet intervention-

As per HAM-D scale is 8-13 in range that confirms
the reduction in severity of depression from severe to
mild.
Discussion:
A 21 years old male student came with the complaints
of disturbed sleep, pessimistic behavior, reduction in
weight, reduction in energy, excessive guiltiness and
indigestion from last six months. While talking with
him he was little confused for answering the
questions related to him, even he was not responding
properly somewhat abnormal behavior, facial
expression was like angry mood ,not much confident,
in that condition he followed one month regular yogic
exercises, acupressure, healthy diet, natural remedies
at home and improves himself from state of
depression.
Conclusion:
Regular yogic practices with proper psychological
counseling and guidance helped patient regain mental
illness. In this case patient experienced the
satisfaction within one month of regular practice of
yoga followed by natural remedies, acupressure and
diet & his investigation reports supported the result.
Patient states that anxiety, excessive guiltiness,
indigestion has reduced remarkably and sleep, energy,
concentration improved. Certain poses helps to boost
circulation and improve blood flow to brain. Deep
breathing exercises relieves stress and anxiety by
balancing stress hormones, and eating whole grains
and fresh fruits and vegetables helps in reducing the
digestive problem and weakness.
