Shifts of the center of pressure (CoP) through modulation of foot placement and ankle moments (CoP-mechanism) cause accelerations of the center of mass (CoM) that can be used to stabilize gait. An additional mechanism that can be used to stabilize gait, is the counter-rotation mechanism, i.e., changing the angular momentum of segments around the CoM to change the direction of the ground reaction force. The relative contribution of these mechanisms to the control of the CoM is unknown. Therefore, we aimed to determine the relative contribution of these mechanisms to control the CoM in the anteroposterior (AP) direction during a normal step and the first recovery step after perturbation in healthy adults. Nineteen healthy subjects walked on a split-belt treadmill and received unexpected belt acceleration perturbations of various magnitudes applied immediately after right heel-strike. Full-body kinematic and force plate data were obtained to calculate the contributions of the CoP-mechanism and the counter-rotation mechanism to control the CoM. We found that the CoP-mechanism contributed to corrections of the CoM acceleration after the AP perturbations, while the counter-rotation mechanism actually contributed to CoM acceleration in the direction of the perturbation, but only in the initial phases of the first step after the perturbation. The counter-rotation mechanism appeared to prevent interference with the gait pattern, rather than using it to control the CoM after the perturbation. Understanding the mechanisms used to stabilize gait may have implications for the design of therapeutic interventions that aim to decrease fall incidence. Summary statement Understanding the mechanisms used to stabilize gait during unperturbed and perturbed walking may have implications for the design of therapeutic interventions that aim to decrease fall incidence.

During human walking, the centre of mass (CoM) is outside the base of support for most of the time, which poses a challenge to stabilizing the gait pattern. Nevertheless, most of us are able to walk without substantial problems. In this review, we aim to provide an integrative overview of how humans cope with an underactuated gait pattern. A central idea that emerges from the literature is that foot placement is crucial in maintaining a stable gait pattern. In this review, we explore this idea; we first describe mechanical models and concepts that have been used to predict how foot placement can be used to control gait stability. These concepts, such as for instance the extrapolated CoM concept, the foot placement estimator concept and the capture point concept, provide explicit predictions on where to place the foot relative to the body at each step, such that gait is stabilized. Next, we describe empirical findings on foot placement during human gait in unperturbed and perturbed conditions. We conclude that humans show behaviour that is largely in accordance with the aforementioned concepts, with foot placement being actively coordinated to body CoM kinematics during the preceding step. In this section, we also address the requirements for such control in terms of the sensory information and the motor strategies that can implement such control, as well as the parts of the central nervous system that may be involved. We show that visual, vestibular and proprioceptive information contribute to estimation of the state of the CoM. Foot placement is adjusted to variations in CoM state mainly by modulation of hip abductor muscle activity during the swing phase of gait, and this process appears to be under spinal and supraspinal, including cortical, control. We conclude with a description of how control of foot placement can be impaired in humans, using ageing as a primary example and with some reference to pathology, and we address alternative strategies available to stabilize gait, which include modulation of ankle moments in the stance leg and changes in body angular momentum, such as rapid trunk tilts. Finally, for future research, we believe that especially the integration of consideration of environmental constraints on foot placement with balance control deserves attention.

During human walking, perturbations to the upper body can be partly corrected by placing the foot appropriately on the next step. Here, we infer aspects of such foot placement dynamics using step-to-step variability over hundreds of steps of steady-state walking data. In particular, we infer dependence of the 'next' foot position on upper body state at different phases during the 'current' step. We show that a linear function of the hip position and velocity state (approximating the body center of mass state) during mid-stance explains over 80% of the next lateral foot position variance, consistent with (but not proving) lateral stabilization using foot placement. This linear function implies that a rightward pelvic deviation during a left stance results in a larger step width and smaller step length than average on the next foot placement. The absolute position on the treadmill does not add significant information about the next foot relative to current stance foot over that already available in the pelvis position and velocity. Such walking dynamics inference with steady-state data may allow diagnostics of stability and inform biomimetic exoskeleton or robot design.

Understanding balance during human gait is complicated by the abundance of recovery options. Among all possible recovery options, three main strategies are often considered for human balance control, being the ankle, hip, and foot placement strategies. All can be addressed when balance is threatened during walking, but their relative importance remains uncertain. We have previously shown that healthy human subjects did not significantly adjust their foot placement relative to the body's center of mass (COM) in the first recovery step following anteroposterior pelvis perturbations, as compared to unperturbed walking. An ankle strategy could have contributed to the recovery instead. Here the goal is to further elucidate balance strategy preferences by investigating the stepping and hip strategies following these anteroposterior perturbations, but with an ankle strategy made ineffective. This was achieved by physically blocking each ankle and minimizing the support area of each foot through a pair of modified ankle-foot orthoses. These “pin-shoes” enabled stilt-like walking and ensured that foot placement adjustment was the only way to modulate the center of pressure location, comparable to “footless” inverted pendulum models of walking. Despite the pin-shoes, subjects did not additionally address a hip strategy compared to normal walking, but relied on foot placement adjustments instead. The observed foot placement adjustments were furthermore in line with concepts derived from a linear inverted pendulum model of walking. These results suggest low hip strategy priority when a foot placement strategy is available, while the latter can be predicted with concepts derived from a simple walking model.

During walking our balance is maintained by muscle action. In part these muscle actions automatically respond to the imbalance. This paper considers responses to balance perturbations in muscles around the ankle, peroneus longus (PL), tibialis anterior (TA) and soleus (SO). It is investigated if their action is related to previously observed balance mechanisms: the ‘braking reaction’ and the mediolateral ankle strategy. Subjects walked on a treadmill and received pushes to the left and pulls to the right in various phases of the gait cycle. Muscle actions were divided into medium latency R1 (100–150 ms), long latency R2 (170–250 ms), and late action R3 (270–350 ms). Short latency responses, before 100 ms, were not observed but later responses were prominent. With inward perturbations (e.g. pushes to the left shortly before or during stance of the right foot) responses in RPL were seen. The forward roll-over of the CoP was briefly stalled in mid stance, so that the heel was not lifted. Stance was shortened. With outward perturbations, pushes to the left shortly before or during stance of the left foot, responses in all three muscles, LTA, LSO, and LPL were seen. Our interpretation is that these muscle activations induce a ‘braking reaction’ but could also contribute to the ‘mediolateral ankle strategy’. The resultant balance correction is small but fast, and so diminishes the need for later corrections by the stepping strategy.

Background: Treadmills are used frequently in rehabilitation enabling neurologically impaired subjects to train walking while being assisted by therapists. Numerous studies compared walking on treadmill and overground for unperturbed but not also perturbed conditions. Objective: The objective of this study was to compare stepping responses (step length, step width and step time) during overground and treadmill walking in a group of healthy subjects where balance assessment robots applied perturbing pushes to the subject's pelvis in sagittal and frontal planes. Methods: During walking in both balance assessment robots (overground and treadmill-based) with applied perturbations the stepping responses of a group of seven healthy subjects were assessed with a motion tracking camera. Results: The results show high degree of similarity of stepping responses between overground and treadmill walking for all perturbation directions. Both devices reproduced similar experimental conditions with relatively small standard deviations in the unperturbed walking as well as in perturbed walking. Conclusions: Based on these results we may conclude that stepping responses following perturbations can be studied on an instrumented treadmill where ground reaction forces can be readily assessed which is not the case during perturbed overground walking.

In many simple walking models foot placement dictates the center of pressure location and ground reaction force components, whereas humans can modulate these aspects after foot contact. Because of the differences, it is unclear to what extend predictions made by models are valid for human walking. Yet, both model simulations and human experimental data have previously indicated that the center of mass (COM) velocity plays an important role in regulating stable walking. Here, perturbed human walking was studied for the relation of the horizontal COM velocity at heel strike and toe-off with the foot placement location relative to the COM, the forthcoming center of pressure location relative to the COM, and the ground reaction forces. Ten healthy subjects received various magnitude mediolateral and anteroposterior pelvis perturbations at toe-off, during 0.63 and 1.25 m s−1 treadmill walking. At heel strike after the perturbation, recovery from mediolateral perturbations involved mediolateral foot placement adjustments proportional to the mediolateral COM velocity. In contrast, for anteroposterior perturbations no significant anteroposterior foot placement adjustment occurred at this heel strike. However, in both directions the COM velocity at heel strike related linearly to the center of pressure location at the subsequent toe-off. This relation was affected by the walking speed and was, for the slow speed, in line with a COM velocity based control strategy previously applied by others in a linear inverted pendulum model. Finally, changes in gait phase durations suggest that the timing of actions could play an important role during the perturbation recovery.

Over 50% of individuals with lower limb amputation fall at least once each year. These individuals also exhibit reduced ability to effectively respond to challenges to frontal plane stability. The range of whole body angular momentum has been correlated with stability and fall risk. This study determined how lateral walking surface perturbations affected the regulation of whole body and individual leg angular momentum in able-bodied controls and individuals with unilateral transtibial amputation. Participants walked at fixed speed in a Computer Assisted Rehabilitation Environment with no perturbations and continuous, pseudo-random, mediolateral platform oscillations. Both the ranges and variability of angular momentum for both the whole body and both legs were significantly greater (p<0.001) during platform oscillations. There were no significant differences between groups in whole body angular momentum range or variability during unperturbed walking. The range of frontal plane angular momentum was significantly greater for those with amputation than for controls for all segments (p<0.05). For the whole body and intact leg, angular momentum ranges were greater for patients with amputation. However, for the prosthetic leg, angular momentum ranges were less for patients than controls. Patients with amputation were significantly more affected by the perturbations. Though patients with amputation were able to maintain similar patterns of whole body angular momentum during unperturbed walking, they were more highly destabilized by the walking surface perturbations. Individuals with transtibial amputation appear to predominantly use altered motion of the intact limb to maintain mediolateral stability. Copyright © 2015 Elsevier B.V. All rights reserved.

Tripping over obstacles is one of the main causes of falls. One potential hazard to actually fall when tripped is inadequate foot landing. Adequate landing is required to control the body's angular momentum, while avoiding dangerous surfaces (slippery patch, uneven ground). To avoid such dangers, foot trajectory needs to be controlled by inhibiting and adjusting the initiated recovery foot path during a tripping reaction. We investigated whether such adjustments can be made without jeopardizing balance recovery. Sixteen healthy young adults (25.1 ± 3.2 years) walked at their comfortable speed over a walkway equipped with 14 hidden obstacles. Participants were tripped 10 times in between a random number of normal walking trials; five trips included a projection of a forbidden zone (FZ, 30 × 50 cm) at the subject's preferred landing position. Participants were instructed to land their recovery foot outside the FZ, if the FZ was presented. Responses were evaluated in terms of foot position and body angular momentum at and following recovery foot landing. Participants successfully landed their recovery foot outside the FZ in 80 % of trials, using strategies of either shortening their recovery steps (84 %) or side stepping (16 %). Their performance improved over trials, and some participants switched strategies. Angular momenta of the adjusted steps remained small at and following recovery foot landing. Young adults can quickly change foot trajectory after tripping by using different strategies, and without detrimental consequences on balance recovery, in terms of the angular momentum. These results open possibilities for training of tripping reactions.
