Características de la suspensión de cirugías electivas en un hospital de la Seguridad Social en Chiclayo, Perú. 2016

Abstract

Objetivo: Determinar las características de la suspensión de cirugías electivas en un hospital de la seguridad social de Chiclayo, Perú en el periodo enero- diciembre 2016. Material y métodos: Se realizó un estudio descriptivo, observacional, transversal y retrospectivo, en el cual se recolectó información de cada una de las fichas de suspensión de cirugía electiva encontradas en sala de operaciones correspondientes al periodo enero-diciembre 2016 que cumplieron los criterios de inclusión. Resultados: Los servicios con mayor frecuencia de suspensión de cirugías electivas fueron: traumatología (16,3%), oftalmología (13,7%) y cirugía general (11,9%), siendo además la tasa de suspensión de cirugías electivas del 10%. Las cirugías de catarata (facoemulsificación) fueron individualmente los procedimientos más suspendidos (13,2%), seguidos de otros procedimientos (9,2%) y osteosíntesis (6,9%). Las causas de suspensión administrativas fueron las más frecuentes constituyendo el 57,1%, seguidos de las causas clínicas (23,7%) y las quirúrgicas (8,5%); siendo específicamente la prolongación del tiempo operatorio la causa con mayor frecuencia (27,3%), seguida de las malas condiciones clínicas del paciente (13,7%) y la falta de insumos (10,5%). Conclusión: Las causas de suspensión de cirugías electivas halladas en el presente estudio fueron, en su mayoría, de causa administrativa, dentro de la cual destaca la prolongación de tiempo operatorio como causa más importante.

Objetivo: Caracterizar a prevalência e as causas de suspensão das cirurgias eletivas considerando determinantes clínicos e não clínicos.Método: Estudo exploratório, retrospectivo, quantitativo. A coleta de dados foi realizada no banco eletrônico de um hospital público do Sul do Brasil,no período entre janeiro de 2018 e dezembro de 2019. Consideraram-se as variáveis sexo, faixa etária, quantitativo de cirurgias, especialidade médica emotivos da suspensão das cirurgias e realizou-se análise estatística descritiva. Resultados: Das 11.792 (100,0%) cirurgias programadas, 2.170 (18,4%)foram suspensas. As especialidades cirúrgicas com maior taxa de suspensão foram ortopedia (32,6%) e cirurgia geral (19,0%), tendo como principaismotivos não clínicos: prioridade para urgência (21,5%), falta de vaga em unidade terapia intensiva (15,6%) e não comparecimento do paciente(13,6%). O principal motivo clínico de suspensão cirúrgica foi condição clínica desfavorável (8,7%). Conclusão: O estudo contribuiu para a avaliaçãodos indicadores de qualidade relacionados aos agendamentos e às suspensões cirúrgicas no Centro Cirúrgico, além de oportunizar a criação deestratégias de melhoria contínua.

Objective: To describe the frequency of gynecological surgery cancellation as well as the factors leading up to cancellation, and to examine how patients perceive this situation. Methods: Case series including patients over 18 years of age scheduled for gynecological surgery due to benign conditions whose surgery was cancelled in a general referral hospital in Lima, Peru, between June 2019 and March 2020. Baseline characteristics of the patients and the reasons for cancellation were assessed, and the Patient Satisfaction Questionnaire Short Form (PSQ-18) tool was administered. Descriptive analysis. Results: The study included 84 patients. During the study period, the frequency of surgery cancellations was 11 %. Medical reasons were the main factor for surgery suspension, at 46.5 %; administrative causes accounted for 36.6 % of the cancellations, due mainly to lack of operation time in 25.4 %. In terms of satisfaction, 21,4 % of the patients were satisfied with the cancellation. Conclusions: Good communication with patients whose surgery needs to be cancelled is essential. Further studies that look into this problem in our region are required.

Objetivo Analizar la suspensión de cirugías en el Hospital de Trauma Dr. Federico Abete en el año 2014. Método Se analizaron datos de 441 cirugías suspendidas en la Unidad de Quirófano del Hospital de Trauma Dr. Federico Abete entre el 1 de enero y el 31 de diciembre de 2014. Se definió como suspensión de cirugía programada toda aquella programación de procedimientos quirúrgicos que por cualquier razón no se realizó en el día y la hora programados en el centro quirúrgico del hospital. Resultados La tasa de suspensión de cirugías fue del 7.6%. La tasa más baja fue registrada en el mes de diciembre (4.3%), mientras que noviembre registró la mayor tasa (11.1%). Las causas relacionadas con la logística o las de tipo administrativo correspondieron al 44.2% de las causas de suspensión, mientras que las causas médicas (no quirúrgicas) tuvieron una frecuencia del 40.8%. Las causas relacionadas con la anestesia representaron el 5.4% del total de las suspensiones de cirugías. Conclusiones Estos hallazgos permiten disponer de un insumo importante para gestionar los recursos de manera eficiente.

Cancellation of operations in hospitals is a significant problem with far reaching consequences. This study was planned to evaluate reasons for cancellation of elective surgical operation on the day of surgery in a 500 bedded Government hospital. The medical records of all the patients, from December 2009 to November 2010, who had their operations cancelled on the day of surgery in all surgical units of the hospital, were audited prospectively. The number of operation cancelled and reasons for cancellation were documented. 7272 patients were scheduled for elective surgical procedures during study period; 1286 (17.6 %) of these were cancelled on the day of surgery. The highest number of cancellation occurred in the discipline of general surgery (7.1%) and the least (0.35%) occurred in Ear-Nose-Throat surgery. The most common cause of cancellation was the lack of availability of theater time 809 (63%) and patients not turning up 244 (19%) patients. 149 cancellations (11.6%) were because of medical reasons; 16 (1.2%) were cancelled by the surgeon due to a change in the surgical plan; 28 (2.1%) were cancelled as patients were not ready for surgery; and 40 (3.1%) were cancelled due to equipment failure.]. Most causes of cancellations of operations are preventable.

This study discusses the problem of surgery cancellation on the economic-financial perspective. It was carried out in the Surgical Center Unit of a school hospital with the objective to identify and analyze the direct costs (human resources, medications and materials) and the opportunity costs that result from the cancellation of elective surgeries. Data were collected during three consecutive months through institutional documents and a form elaborated by the researchers. Only 58 (23.3%) of the 249 cancelled scheduled surgeries represented costs for the institution. The cancellations direct total cost was R1.713.66(averagecostperpatientR 1.713.66 (average cost per patient R 29.54); distributed as follows: expenses with consumption materials R333.05;sterilizationprocessR 333.05; sterilization process R201.22; medications R149.77andhumanresourcesR149.77 and human resources R1,029.62. The human resources costs represented the greatest percentile in relation to the total cost (60.40%). It was observed that most of the cancellations could be partially avoided. Planning on management; redesigning work processes, training the staff and making early clinical evaluation can be strategies to minimize this occurrence.

In the search for better quality indices, the suspensions of surgeries have been examined by and received attention from researchers in the field of health. High cancellation rates reveal an inadequate management that impacts negatively on the health team iself and the patient's life and and that of his or her family. This retrospective descriptive study aim to identify the rate of surgical suspension and the reasons for this procedure in a public teaching hospital. Data were collected from the Surgical Center Archive and Medical Archive Service Statistics. There was a rate of 14.14% surgical cancellations and in 1,402 (63.78%) occasions no records were found that justify the suspension of the surgeries. The results showed the following justifications for the suspensions of the surgery: 194 (8.82%) were non hospitalized patients; 182 (8.30%) for inadequate clinical conditions;, 179 (8.14%) due to administrative problems;, 94 (4.27%) for welfare issues;, 61 (2.77%) for structural issues; 40 (1.82%) for lack of patients and 46 (2.10%) for other reasons. These findings point to the need to adopt measures that will ensure patient safety and quality of service offered.

To model the frequency of 'last minute' cancellations of planned elective procedures in the English NHS with respect to the patient and provider factors that led to these cancellations. A dataset of 5,288,604 elective patients spell in the English NHS from January 1st, 2007 to December 31st, 2007 was extracted from the Hospital Episode Statistics. A binary dependent variable indicating whether or not a patient had a Health Resource Group coded as S22--'Planned elective procedure not carried out'--was modeled using a probit regression estimated via maximum likelihood including patient, case and hospital level covariates. Longer waiting times and being admitted on a Monday were associated with a greater rate of cancelled procedures. Male patients, patients from lower socio-economic groups and older patients had higher rates of cancelled procedures. There was significant variation in cancellation rates between hospitals; Foundation Trusts and private facilities had the lowest cancellation rates. Further research is needed on why Foundation Trusts exhibit lower cancellation rates. Hospitals with relatively high cancellation rates should be encouraged to tackle this problem. Further evidence is needed on whether hospitals are more likely to cancel operations where the procedure tariff is lower than the S22 tariff as this creates a perverse incentive to cancel. Understanding the underlying causes of why male, older and patients from lower socio-economic groups are more likely to have their operations cancelled is important to inform the appropriate policy response. This research suggests that interventions designed to reduce cancellation rates should be targeted to high-cancellation groups.
