Acne and One‘s Self Confidence: Cross-sectional study on Malaysian Student population

Abstract

Introduction: Self-confidence of an individual must be motivated by self without the inference of any internal or external consequences. Acne is a common skin defect among adolescents, and even in adults that can cause psychological weakness affecting one’s self-confidence level. The objective of this study is to investigate the effect of acne on the self-esteem of young adults of Malaysian student population. Materials and Method: A cross-sectional survey was done using a newly developed questionnaire having majorly close ended (in 2-point scale) questions. Total of 185 Malaysian MBBS students were randomly chosen, and only 171 valid responses from the victims of acne were analyzed. Results: Among the students with acne problems, 88% of them opine that they lose their self-confidence due to the effect of acne. Out of this, 78% of them were preferred to associated with the people where as 22% of the students do not prefer to be involved with the individuals. Conclusion: From the results, it was found that acne significantly reduces self-confidence of a majority of students whereas a small group of participants does believe that acne does not describe them. Students with acne problems are more self-conscious and perform poorly in social life. It is regardless of the clinical severity of their acne. Introduction
Acneis acommon skindefect amongadolescents,
andeveninadults.Itis caused by inammation of
theskinglandsandhairfollicleswiththeincreased
sebumsecretion.Thepresenceofacnehastheimpact
of being associated with several psychological
distresses, including anxiety, depression, self-
consciousness, lowered self-concept and lack of
self-condence. Scientic literature have described
the negative impact of acne upon one’s mood self
–esteem and interpersonal relationships. Severity
ofacne evencould lead to depression and suicidal
ideation1,2,3
Self-condencerelatestoafeelingorconsciousness
ofone’spowerorofrelianceonone’scircumstances.
Aperson’sself-assurednessisnecessaryforcurbing
depression and depicts how personal achievements
arevalued.Self-condenceisofmuchimportancein
a young adult’s life.
The psychological perspectives of skin diseases
are well studied. However, disease-specic
questionnairesasusedintheformofclinicaltrialsare

scanty. In this regard, survey-based studies to assess
the impact of facial acne on health-related quality
of life could help in the evaluation of therapeutic
changes4.
The present study was undertaken to understand
the attitude of Malaysian undergraduate students
concerned with the facial acne. The primary
parameters evaluated through this study were the
self-condence pattern of students with acne and
thesocial behaviorof bothacne aectedand non-
aected students. The responses given by these
students might be helpful in drafting a suitable
methodto handleany problemsassociated withit,
for instance, depression and inferiority complex
among students.
Materials and Methods
Sample size:This is a cross-sectional studybased
onrandomsamplingmethod.Atotalof185medical
undergraduate students, comprising of 102 male
students and 83 female students aged 17-19 years
participatedinthis studyandtheresponseratewas
100%. The students belonged to three dierent
Malaysian ethnic races i.e. Malays, Chinese and
Indians(MalaysianIndians)werechosenrandomly.
Data collection:Selfdesignedquestionnaireswere
usedinthestudy.Thequestionsweremostlyclosed-
ended (in 2-point scale) with few optional open-
ended. The questionnaire validated by the expert
faculties of Melaka Manipal Medical College,
ManipalUniversity.Thequestionsweredesignedin
thesimpleformanswerablebythestudentsaccording
to our objectives, and it included questions which
assessthestudent’sperceptionsregardingtheeect
ofacneontheirself-condence.Thisquestionnaire
constitutedof11closed-endedquestionswhichwere
validated by academic faculty.
Data analysis: All the data that obtained after
evaluating the questionnaires was analyzed
percentage wise. The results obtained were
representedastablesandpiechartsusingMicrosoft
Excel2007.
Ethical Clearance: Ethicalclearancewasobtained
from the institutional research committee, before the
study and the students gave their informed consent
before taking part in this study. Participation was
purelyonvoluntarybasis.
Results:
Outof185 studentswhowerevoluntarilyinvolved
in the study through the random selection, only 171
students (92%) responses were validated as they
haveself-declaredthateither they had experienced
the acne related complications or presently being
experiencing.Theremaining14students(8%)ruled
out that they have not been victims of facial acne in
their life (Figure 1).

Discussion
Acne is a universal problem seen amongst young
adults. More than an apparent skin disease, it can
result in psychosocial distress. It has been widely
experiencedthat,individualwithacnerelatedissues
showlowerself-condencethantheirpeerswithout
acne and multiple studies exhibit a correlation
betweenacneanddiminishedqualityoflife,anxiety,
and suicidality. Based on our assessment of the
subjects,a majority state that acneinuences their
self-consciousness, present-ability, and level of
inferiorityto communicate withothers.Amongthe
subjectswhosuerfromacne,manyofthemresort
toapplyingmethodstoovercometheirproblem.Itis
presupposedthatsubjectsviewacneasanintolerable
entity. According to the outcome, the subject’s
perceptionoftheirconditionisnotparallelwiththe
clinicalseverityof theiracne.Thus,anindividual’s
personal assessment is the crucial determinant of
one’spsychosocialproblems.
Ina previous study by Hassan et alexamined 132
patientsfromaspecialistacneclinicandequatedtheir
response to questionnaires including the Derriford
appearancescaleandself-ratedacnescales5.Subjects
above20yearsofageweremorepronetosuerfrom
appearancedistress due to acne compared to those
between16to19yearsold.Theauthorsnotedthata
personalevaluationoffacialacneseverityincreased
the likeliness of self-consciousness in women, but
not in men.
Uslu et al., done a cross-sectional study with a
populationof563highschoolstudents,agedbetween
13 to 19 years of age. The report documented an
acne prevalence of 63.6% with more lesions seen
inmalesthan females. Responsesassessed from a
questionnaire about acne, involving both, General
Health Questionnaire (GHQ), and the Rosenberg
Self-EsteemScale(RSES)showed that adolescents
with the improvement of acne due to treatment
subsequentlyhadhigherlevelsofself-esteem6.
An interview-based study by Magin et al., was
conductedfrom randomly selectedpatients visiting
general practices and specialty dermatology
practices in Australia. The study group involved
26patientssueringfromacne.Authorsconcluded
that,withrangingagesandacnestages,participants
had considerable issues with impaired self-image,
embarrassment, frustration, and low self-esteem7.
Smithardetal,conductedacross-sectionalstudywith
317adolescents usingthe Strengthand Diculties
Questionnaire (SDQ) and the Acne Management
Questionnaire (AMQ) to measure psychological
status, acne knowledge, and help-seeking behavior
of the participants. According to their perceptions,
higherSDQscorespresumedtolowself-condence.
Theauthorshavealsoreportedthat,theparticipants
withdenitelesions (morethan12lesions)ofacne
provedtohavehigherlevelsofemotionaldiculties,
and the level of understanding about the cause of
acne was low. However, a prevalence of 77% of
participants had normal SDQ scores8. However,
Srivastavaetal.,reportednocorrelationbetweenacne
andqualityoflifeofpatientseventhoughpresence
ofhigherpsychiatricmorbidityinthe patients with
acne vulgaris9.
Basedontheresultsobtained,despitebeingmedical
students, they not condent with themselves when
they have acne.  However, they do not have a
problem in association with an individual who are
having acne. Some dierent opinions were given
by the participants of these questionnaires. The
majority of them have an opinion that acne does
aect self-condence in one or another way. Most
ofthemgiven theirviewsthattheytendtohave an
uncomfortable or inferior feeling when they have
acneontheirface.Seriousproblemsofbeingvictims
ofacnereducetheirself-condencelevelsasothers
paymoreattentiontotheacneontheface.
Accordingto somestudents, acne reects hygiene.
So,theythinkthatpeoplewithacnedonotmaintain
their cleanliness. Moreover, acne victims assume that
itmakestheirappearanceunaesthetic.Afewofthem
sharetheopinionthathavingacneonthefaceisnot
presentableandmakespeoplejudgethem wrongly.
All these reasons contribute to the fact that acne
reducesanindividual’sself-esteeminaway.
Thepresentstudyinvolvingtheteens,andthefactis
teensgainindependenceduringadolescence.Atthis
stage,theirattitudetowardstheacnecouldadversely
eectontheiracademicactivities and hamper their
socialbehaviors.So,itisimperativetoconsiderthe
presenceofco-morbidpsychologicaldisordersinthe
treatment of acne related issues to attain a successful
impact of therapy on psychological outcomes10.
Timelyguidanceeitherfromtheirparentsorfriends
could overcome these complications, as there is a
positive correlation between treatment adherence
andimprovedqualityoflife11.
Conclusion:
Based on the data collected, the majority of the
studentsofourinvestigationshareasimilaropinion
thatacnediminishestheirself-condencelevel.From
the results, Students with acne problems are more
self-conscious and perform poorly in social life. A

Backgorund: Acne vulgaris (AV) is a skin disorder commonly found on late teens, young adults and most patients are females. The classic pathophysiology circulates the relationship between inflammation, bacterial colonization, sebum excretion and follicular epidermal hyperproliferation. Other factors may include diet, stress level, self-hygiene, and hormones especially in female patients. A proper diagnosis and the right treatment approach will help treat patient significantly. Sufficient knowledge on acne vulgaris and its treatment is important for a person to self-medicate or a visit to the dermatologist can be another option. Commonly used self-medication for acne are clindamycin and benzoyl peroxide (BPO). Treatment can either be single or combined therapy to achieve better results. Acne treatments might take some time to show its effects and it may also relapse. Objectives: The study aims to see the level of knowledge on acne vulgaris by the respondents. Methods: This cross-sectional study was conducted to medical undergraduates at Universitas Sumatera Utara. Results: 85.2% from total respondents have had acne before. 142 (55.5%) students chose to seek treatment with a dermatologist which majority of them are females (34.4%). Female students contribute the most with 160 responses. Most respondents are from age category of 16 to 21 (75.4%). Conclusion: Students do have the knowledge on acne vulgaris and its treatment hence almost half of them chose to self-medicate. Students are also aware that other treatment options are available. Diagnosis yang tepat dan pendekatan pengobatan yang tepat akan membantu merawat pasien secara signifikan. Pengetahuan yang cukup tentang akne vulgaris dan pengobatannya penting bagi seseorang untuk mengobati sendiri atau kunjungan ke dokter kulit bisa menjadi pilihan lain. Pengobatan sendiri yang umum digunakan untuk jerawat adalah klindamisin dan benzoil peroksida (BPO). Perawatan dapat berupa terapi tunggal atau kombinasi untuk mencapai hasil yang lebih baik. Perawatan jerawat mungkin membutuhkan waktu untuk menunjukkan efeknya dan mungkin juga kambuh. Tujuan: Mengetahui gambaran pengetahuan dan pemilihan pengobatan akne pada mahasiswa Fakultas Kedokteran Universitas Sumatera Utara. Metode: Menggunakan metode desain penelitian potong lintang (cross sectional) dengan membagikan kuesioner kepada mahasiswa/i Fakultas Kedokteran Universitas Sumatera Utara. Acne is one of the most prevalent skin conditions affecting teenagers. It is a disease of the pilosebaceous unit. Blockage of sebaceous glands and colonisation with Proionobacterium acnes leads to acne. Grading the severity of acne helps to determine the appropriate treatment. Treatment of acne should be started as early as possible to minimise the risk of scarring and adverse psychological effects. It should be tailored to the individual patient, the type of acne, its severity, the patient's ability to use the treatment, and the psychological state. Topical agents are the mainstay for treatment of mild acne. Moderate acne is treated with oral antibiotics. Resistance to antibiotics may be reduced by subsequent use of non-antibiotic topical medications. Severe acne is treated with isotretinoin, and this can lead to permanent remission. With better education and care given by medical profession, acne treatment could be significantly improved.

To investigate the psychological sequelae of acne vulgaris. Qualitative study using a grounded-theory approach. General practices and specialty dermatology practices in Newcastle, Australia. Patients with current acne recruited from the practices. We used semistructured interviews and recorded participants' comments verbatim. Data analysis was cumulative and concurrent throughout the data-collection period. Coding and analysis was done in the inductive tradition. Interviews were conducted with 26 subjects who represented a range of ages and acne severity. Psychological morbidity was considerable. Though participants had mood and anxiety symptoms, these symptoms tended to be subsyndromal and evanescent. More prominent symptoms were embarrassment, impaired self-image, low self-esteem, self-consciousness, frustration, and anger. Some subjects thought that acne had affected their personalities permanently and adversely. Psychological sequelae were attributed to the effects of facial acne on appearance. The psychological effects of acne can be considerable. The psychological morbidity is complex and often does not conform to standard psychiatric disease criteria. Recognition and management of the psychological sequelae of acne by general practitioners is of considerable importance.

Acne vulgaris is a common disease among adolescents and known to have adverse effects on psychological status. A cross-sectional study was conducted on 600 participants by means of a questionnaire designed for this study in high school students. In addition, an objective evaluation of acne in participants was undertaken. The questionnaire consisted of questions about acne, General Health Questionnaire (GHQ) and Rosenberg Self-Esteem Scale (RSES). Five hundred and sixty-three questionnaires out of 600 were answered and 550 adolescents who gave permission for examination were evaluated. The study population consisted of 303 girls and 260 boys between the ages 13 and 19, and the mean age was 15.24 +/- 1.05 years. Acne prevalence was 63.6% with 29.2% non-inflammatory and 34.4% inflammatory acne. It was more prevalent and severe in boys than in girls. Not the objective but the subjective severity of acne and opinion that one could benefit from acne treatment was found to be related to anxiety, depression and self-esteem. Factors implicated among causes of acne were food, bad skin hygiene and hormones in decreasing frequency. Forty-eight per cent of adolescents expect a maximum duration of 4 weeks for treatment. Despite the high prevalence of acne, there is still much deficiency of knowledge and wrong beliefs about acne. This indicates that there is an urgent need for education about etiopathogenesis, potential complications and importance of effective treatment for acne. Effective treatment may make significant contributions for the mental health of adolescent and as well as adult populations.

Acne is a significant adolescent problem and may precipitate emotional and psychological effects. The impact of acne on psychological parameters and implications for acne treatment are not fully understood. We performed a MEDLINE search using the terms "acne" and "adolescent" along with "psychological," "depression," or "psychiatric," which yielded 16 reviewed studies. Qualitative review of the selected articles revealed that the presence of acne has a significant impact on self-esteem and quality of life. Depression and other psychological disorders are more prevalent in acne patients and acne treatment may improve symptoms of these disorders. The reviewed studies were semi-quantitative analyses utilizing various standardized surveys or questionnaires. Therefore, quantitative analysis of selected studies was not possible. The presence of co-morbid psychological disorders should be considered in the treatment of acne patients and future prospective trials are needed to assess the impact of treatment on psychological outcomes.

This study examined appearance-related distress in patients with acne. One hundred and thirty-two people attending a specialist acne clinic completed questionnaires including the Derriford Appearance Scale and three self-rated acne scales covering more (facial) and less visible (chest, back) acne sites. Women with acne demonstrated greater self-consciousness of appearance and negative self-concept than men. Subjective rating of severity of facial acne was significantly associated with increased social self-consciousness in women, but not in men. Back acne was significantly associated with sexual and bodily self-consciousness of appearance in both men and women. Patients 20 years and above were significantly more likely to suffer appearance-related distress than 16-19-year-olds. Although not a key focus of this study, ethnicity was also implicated in appearance concern arising from acne.

Although psychosocial aspects of skin diseases are well known, disease-specific questionnaires validated for use in clinical trials are not available to assess the impact of facial acne on health-related quality of life or to evaluate therapeutic change. Development of such an instrument was undertaken and included item generation, reduction and pilot-testing phases. By interviewing acne subjects and dermatologists and literature review, 168 possible items were identified. Next, 165 acne subjects identified which items affected them and rated importance on a 5-point scale. Reduction to a brief questionnaire was performed by evaluating patient-perceived importance and factor analysis; four domains were identified (self-perception, role-emotional, role-social, acne symptoms). After pilot-testing for comprehension in acne subjects, further revisions were made to improve clarity and applicability. The resulting instrument takes 10 minutes to complete, and consists of 24 questions assessing how acne affected certain aspects of patients' lives during the past week on a 7-point scale. Thus, an instrument with excellent content validity was developed to assess health-related quality of life in patients with facial acne, and is comprised of statistically meaningful items of importance to patients. Other measurement characteristics are being assessed in a recently initiated study to evaluate test-retest reliability and responsiveness to therapy.

Acne vulgaris is a distressing condition that affects the majority of adolescents, but its impact on mental health in this age group is poorly understood. To determine the prevalence of acne, knowledge about acne and rates of help-seeking behaviour in English teenagers. It was hypothesized that presence of acne would be associated with higher rates of emotional and behavioural difficulties. Three hundred and seventeen pupils (80% response rate) aged 14-16 years participated from a comprehensive school in Nottingham. An age-appropriate, validated measure of emotional well-being, the Strengths and Difficulties Questionnaire (SDQ), and an Acne Management Questionnaire were used to assess participants' psychological health, level of acne knowledge and help-seeking behaviour. Acne severity was by graded by visual facial examination using an adaptation of the Leeds Acne Grading Technique. There was a prevalence of acne in 50% of the study sample, with 11% of participants having moderate to severe acne (> 20 inflammatory lesions). Participants with definite acne (12+ lesions) (P < 0.01) and girls (P < 0.05) had higher levels of emotional and behavioural difficulties. Participants with acne were nearly twice as likely as those without acne to score in the abnormal/borderline range of the SDQ (32% vs. 20%; odds ratio 1.86, 95% confidence interval 1.03-3.34). Knowledge about the causes of acne was low (mean 45%), and was unrelated to acne status. Fewer than a third of participants with definite acne had sought help from a doctor. Acne is a common disorder in English adolescents and appears to have a considerable impact on emotional health in this age group. Low levels of acne knowledge and poor acne management are concerns that could be amenable to a school-based education programme.

To assess patient compliance with psoriasis treatment. Open prospective study. Patients with psoriasis were examined and completed a brief medical and social history, a compliance assessment sheet, and the Dermatology Life Quality Index (DLQI). Patients were reexamined at 3 months, and their actual treatment use was assessed and compared with the expected use. Medication adherence was assessed by direct questioning. Dermatology outpatient clinic. Two hundred ninety-four patients fulfilled the inclusion criteria, and 201 completed the study. Adherence with topical and oral therapies, using objective and self-reporting methods and description of factors affecting compliance. The overall mean +/- SD medication adherence was 60.6% +/- 33.0% (range, 0%-169%). The mean +/- SD DLQI in the study was 17.4 +/- 8.9 (range, 0-30). There was a highly significant negative correlation (r = -0.92) between these variables. Being female, married, employed, and not paying for prescriptions were characteristics associated with increased medication adherence and a lower DLQI. Medication adherence was greater for topical or combined therapy, for once-daily treatment, and for first-time use of treatment. Adverse effects reduced compliance. The major reasons for missing treatment were drinking alcohol, being fed up, forgetfulness, and being too busy. Patients with facial disease and with more extensive disease had lower medication adherence. A range of disease-related and social factors may affect compliance with psoriasis treatment. The inversely proportional relationship between DLQI and medication adherence reflects the interaction of physical and psychological factors, as well as perceived treatment failure.

The occurrence of depression in association with dermatologic disease is common. Psychiatric disturbance is reported in approximately 30% of dermatology patients. Depression can have varied presentations and is more relevant clinically in dermatology patients during critical psychosocial periods of development. Early recognition and treatment of depression associated with skin disorders can lead to improved therapeutic outcomes and may avert disastrous outcomes, including suicide.

Acne is a ubiquitous affliction that can leave physical and emotional scars that can persist throughout the life of the affected individual. Recent studies have substantiated the psychological impact and support a causal, and at times reciprocal, link between acne and the emotional and functional status of the patient. The present article will examine the recent data substantiating the emotional and functional impact of acne on the affected individual. Criteria to identify high-risk patients are provided. High-risk patients are those at increased risk for psychological and functional impairment or self-injurious behavior.
