Effect of Gastric Bypass and Sleeve Gastrectomy on Body Image

Abstract

Background: The aim of this study was to compare the body image in two groups of patients with obesity, undergoing gastric bypass surgery and Sleeve gastrectomy. Methods: This descriptive study was performed on 126 morbid obese patients (two groups of 63), who underwent gastric bypass and Sleeve gastrectomy procedures from January 2017 to October 2017 at Alzahra Medical Training Center in Isfahan, Iran. Body image of all patients was assessed preoperatively and 6 months after the surgery using Multidimensional Body Self-Relation Questionnaire. All data were analyzed using SPSS software. Findings: Paired t test indicated that the mean of total body image score increased significantly in both groups after the surgery (P < 0.01); but independent t test showed that there was no significant difference between the two groups at baseline (P = 0.62), was well as 6 months after the surgery (P = 0.62). Conclusion: The mean total body image score was significantly improved at 6 months postoperatively. But in long term, following more weight loss and flabby or loose skin, their body image would change or decrease consequently. Therefore, it is recommended to follow up these patients in a longer periods.

Purpose of review: Little is known about the psychological effects on life after bariatric surgery despite the high prevalence of psychological disorders in candidates seeking this procedure. Our review discusses the literature around the psychological impact of bariatric surgery, exploring whether the procedure addresses underlying psychological conditions that can lead to morbid obesity and the effect on eating behaviour postoperatively. Recent findings: Findings show that despite undisputed significant weight loss and improvements in comorbidities, current literature suggests some persisting disorder in psychological outcomes like depression and body image for patients at longer term follow-up, compared to control groups. Lack of postoperative psychological monitoring and theoretical mapping limits our understanding of reasons behind these findings. Reframing bariatric approaches to morbid obesity to incorporate psychological experience postoperatively would facilitate understanding of psychological aspects of bariatric surgery and how this surgical treatment maps onto the disease trajectory of obesity.

Morbid obesity as an entity that is associated to numerous clinical complications, an increased mortality and a deterioration of the quality of life, the World Health Organization (WHO) recommends surgical treatments as an alternative. In this sense, the physical and psychological benefits associated with bariatric surgery are well documented. However, the surgery can generate negative effects. The structural changes in the digestive system, coupled with the disruption of the body image especially patients who undergo bypass gastric, reinforce the idea that everything is not positive. Given this reality, this theoretical review is to emphasize the importance of studying the role that psychological variables in adapting to the new body image in these patients. keyword: Obesity, Gastric bypass, body image, psychological variables.

Atualmente, a cirurgia plástica vem se firmando cada vez mais como parte integrante do tratamento cirúrgico da obesidade mórbida, na medida em que visa a devolver as melhores condições de contorno corporal ao enorme contingente de pacientes submetidos a grandes perdas ponderais. Os aspectos peculiares que acompanham essa nova trajetória do paciente obeso exigem abordagem interdisciplinar, com cuidadoso acompanhamento psicológico, antes e depois da cirurgia bariátrica, que deverá prepará-lo continuamente para as grandes transformações impostas a sua imagem corporal. Com a popularização das gastroplastias e a crescente demanda por procedimentos de contorno corporal após grandes emagrecimentos, é cada vez mais comum a presença desses pacientes nos consultórios de cirurgiões que não estão ligados aos serviços multidisciplinares, e que, portanto, precisam conhecer, avaliar e lidar também com os aspectos psicológicos envolvendo candidatos a cirurgia plástica pós-bariátrica. Este trabalho estabelece uma revisão da literatura acerca do complexo ambiente psicológico na obesidade, voltada para o cirurgião plástico, com ênfase na identificação e no controle das condições psíquicas desfavoráveis, possibilitando o melhor planejamento operatório em pacientes com perda significativa de peso após cirurgia bariátrica.

Objective Since large weight losses are rarely achieved through any method except bariatric surgery, there have been no studies comparing individuals who initially lost large amounts of weight through bariatric surgery or non-surgical means. The National Weight Control Registry (NWCR) provides a resource for making such unique comparisons. This study compared amount of weight regain, behaviors, and psychological characteristics in NWCR participants who were equally successful in losing and maintaining large amounts of weight via either bariatric surgery or non-surgical methods. Design Surgical participants (n = 105) were matched with two non-surgical participants (n = 210) on gender, entry weight, maximum weight loss, and weight maintenance duration and compared prospectively over 1 year. Results Participants in the surgical and non-surgical groups reported having lost approximately 56 kg and keeping ≥ 13.6 kg off for 5.5 ± 7.1 years. Both groups gained small but significant amounts of weight from registry entry to 1 year (p = 0.034), but did not significantly differ in magnitude of weight regain (1.8 ± 7.5 kg and 1.7 ± 7.0 kg for surgical and non-surgical groups, respectively; p = 0.369). Surgical participants reported less physical activity, more fast food and fat consumption, less dietary restraint, and higher depression and stress at entry and 1 year. Higher levels of disinhibition at entry and increased disinhibition over 1 year were related to weight regain in both groups. Conclusions Despite marked behavioral differences between the groups, significant differences in weight regain were not observed. The findings suggest that weight loss maintenance comparable to that after bariatric surgery can be accomplished through non-surgical methods with more intensive behavioral efforts. Increased susceptibility to cues that trigger overeating may increase risk of weight regain regardless of initial weight loss method.

A high body-mass index (BMI, the weight in kilograms divided by the square of the height in meters) is associated with increased mortality from cardiovascular disease and certain cancers, but the precise relationship between BMI and all-cause mortality remains uncertain. We used Cox regression to estimate hazard ratios and 95% confidence intervals for an association between BMI and all-cause mortality, adjusting for age, study, physical activity, alcohol consumption, education, and marital status in pooled data from 19 prospective studies encompassing 1.46 million white adults, 19 to 84 years of age (median, 58). The median baseline BMI was 26.2. During a median follow-up period of 10 years (range, 5 to 28), 160,087 deaths were identified. Among healthy participants who never smoked, there was a J-shaped relationship between BMI and all-cause mortality. With a BMI of 22.5 to 24.9 as the reference category, hazard ratios among women were 1.47 (95 percent confidence interval [CI], 1.33 to 1.62) for a BMI of 15.0 to 18.4; 1.14 (95% CI, 1.07 to 1.22) for a BMI of 18.5 to 19.9; 1.00 (95% CI, 0.96 to 1.04) for a BMI of 20.0 to 22.4; 1.13 (95% CI, 1.09 to 1.17) for a BMI of 25.0 to 29.9; 1.44 (95% CI, 1.38 to 1.50) for a BMI of 30.0 to 34.9; 1.88 (95% CI, 1.77 to 2.00) for a BMI of 35.0 to 39.9; and 2.51 (95% CI, 2.30 to 2.73) for a BMI of 40.0 to 49.9. In general, the hazard ratios for the men were similar. Hazard ratios for a BMI below 20.0 were attenuated with longer-term follow-up. In white adults, overweight and obesity (and possibly underweight) are associated with increased all-cause mortality. All-cause mortality is generally lowest with a BMI of 20.0 to 24.9.

Background: This study was done to analyze the problems of post-bariatric patients with excess skin and to determine their interest in body contouring surgery. The self-administered Sahlgrenska Excess Skin Questionnaire (SESQ) was used together with a study-specific questionnaire. Methods: The patients who were operated with bariatric surgery at Sahlgrenska University Hospital between 1999 and 2008 were identified and sent the SESQ and a study-specific questionnaire. Results: The response rate was 65% (23% men). The most common problem in both men and women was the feeling of having an unattractive body (91 and 67%, respectively). The most frequently reported sites of excess skin were the upper arms in women (91%) and the abdomen in men (78%). In both women and men, excess skin on the abdomen was reported to cause the most discomfort (median 7 and 3, respectively, on a scale from 0 to 10). Women reported significantly more problems, discomfort, and amount of excess skin (p < 0.05) than men. There was a strong correlation between the amount of excess skin and the degree of discomfort for all body parts. Seventeen percent of the responders had been operated with body contouring surgery of one body part and 5% of two or more. Fourteen percent desired body contouring surgery of one body part and 61% of two or more. Conclusions: Most post-bariatric patients, but women in particular, experience significant problems of excess skin and request body contouring surgery.

Body contouring following the massive weight loss associated with bariatric surgery has grown in popularity. Little is known, however, about the psychosocial aspects of these procedures. This article discusses the psychological considerations of the postoperative bariatric surgery patient who undergoes body contouring surgery. A review of the psychosocial and physical characteristics of individuals with extreme obesity who undergo bariatric surgery, and the changes in these traits that typically occur postoperatively, is provided. The relationship of body image dissatisfaction to both bariatric and plastic surgery is discussed. The breast reduction literature is reviewed and used to provide a framework with which to understand the psychosocial issues of body contouring patients. These related literatures are used as a guide to provide suggestions for future research investigating the physical and psychosocial outcomes following body contouring surgery. Preliminary recommendations for the psychosocial assessment of body contouring patients are provided. Given the current popularity of both bariatric and plastic surgery, the number of individuals who present for body contouring following massive weight loss is likely to increase. Appropriate psychosocial assessment and management of these patients is believed to play an important role in successful postoperative outcomes.
