Sailan-ur-Rahem (Abnormal Vaginal Discharge) in Greco-Arabic Medicine: A Review

Abstract

Greco-Arab medicine also known as Unani System of Medicine (USM) is well known based on the Hippocratic humoral theory. According to USM, every person is supposed to have a unique humoral constitution which represents his healthy state and any change in this state causes illness of the said person. The severity of the disease depends directly upon the change in equilibrium from mizaj (Temperament). In Greco-Arab medicine, Sailan-ur-Rahem is a diseased condition which occurs due to change in the quality and quantity of Balgham (phlegm) and because of excess useless matter present in the uterus, which is accumulated due to Zoef-e-quwat-e-dafea. The aim of the present review is to ascertain the Unani concept of Sailan-ur-Rahem as described in Unani classical literature, and hence to spread the knowledge for preventive measures and the management of Sailan-ur-Rahem by Unani single and compound drug formulations which are cost effective and easily available and devoid of side effects.

Leucorrhoea is a broad term commonly used for a condition where woman suffer from uterine, cervical or vaginal discharge other than blood due to genital tract infection or alteration in genital physiology and is highly prevalent (31-70%). In Unani medical literature it is described as Sayalān al-Rahim. According to Avicenna, uterus contains excessive waste products and hence prone to infection which leads to weakening of digestive faculty of uterine vessels resulting in discharge. The purpose of the present study was to validate the safety and efficacy of Unani pharmacopoeial formulation Majoon Suparipak in leucorrhoea. This open label validation study conducted on 103 eligible women were randomly enrolled and given Majoon Suparipak at a dosage of 7 g twice a day for four weeks. The levels of improvement were recorded at 14 th day and at the end of the study, in terms of percentage efficacy as calculated from improvement in the sign and symptoms in the visual analogue scale. The baseline and follow up values were statistically analyzed using Friedman post-hoc test and Student's paired 't' test. The result was expressed as Mean ± SEM. The amount of vaginal discharge, backache, general weakness and anaemia were significantly improved (P<0.01). The present study gives the impression that a Unani pharmacopoeial formulation Majoon Suparipak improves the general health, targets the causative agents of the disease and helps in effective management of leucorrhoea.

The Unani Medicine (Greek medicine) is a broad system of medicine, which specifically deals with a variety of states of wellbeing and disease. It deals in prevention of disease so that it can be checked in early stages and also helps to restore the function of body. This System of medicine is one of the oldest conventional systems of medicine, which has its root from the time of Hippocrates (460-377 BC). In modern medicine, Leucorrhoea is described as a thick discharge from vagina which is whitish, or yellowish in colour. It is commonly a non-pathological symptom which occurs after inflammation of the vagina or cervix. In Unani system of medicine, the modern counterpart name for leucorrhoea is Sayalan al-Rahim. A huge number of drugs, either single or compound formulations, have been described for the management of Sayalan al-Rahim. Majoon Supari Paak is a polyherbal Unani formulation that contains twenty-five herbal ingredients, of which Foful (Areca catechu) is the main ingredient along with Zafran (Crocus sativus), Darchini (Cin-namomum zeylenicum), Zanjabeel (Zingiber officinalis), Qaranfal (Cinnamomum verum) and many more. The medicine has been used to treat leucorrhoea, as a uterine tonic and also to treat infertility along with different gynaecological disorders. The ingredients have been considered to perform as an antioxidant, anti-inflammatory, and boost fertility. Scientific researches of the above-mentioned constituents have also reinforced their beneficial anti-inflammatory, astringent, and antiseptic properties.

The great Greek Philosopher & Physician Hippocrates (460 -377 B.C.) is the founder of Unani Medicine. Unani medicine is based on the humoral theory (Nazriya Akhlat) postulated by Hippocrates. According to this theory, the health of a person depends on the four basic fluids (Akhlat) viz. Dam (blood), Balgham (phelgm), Safra (yellow bile) and Sauda (black bile) of the body. Every person is supposed to have a unique humoral constitution which represents his healthy state. And to maintain the correct humoral balance there is a power of self-preservation or adjustment called Quwwat-e-Mudabbira Badan or simply Tabiat (Homeostasis) in the body. [1, 7-9] When this proportion is disturbed it manifests in various pathologies. These pathologies are predisposed by the six essential factors of life i.e. environment; foods and drinks; rest and body functions; emotions and psychological rest; sleep and wakefulness and retention and excretion. The medicines and other therapeutic managements used in this system, in fact, help the body to regain this power to an optimum level and thereby restore humoral balance, thus retaining health.[1-3, 6, 8, 10] ABSTRACT Coronary heart disease (CHD) is the leading cause of death in India and the leading cause of death worldwide. One of the important cause of CHD is change in lifestyle and hence obesity and its related consequences. Today, more than 1.1 billion adults worldwide are overweight, and 312 million of them are obese. In addition, at least 155 million children worldwide are overweight or obese. Unani System of Medicine is a well known for its own way of treatment by various methods. The present paper describes the detailed concept of obesity (Samane Mufrat) and its related consequences and management with reference to the Unani concept.
