Vitiligo is chronic idiopathic disorder of skin pigmentation that plays havocs
with the social livings of patients. Its major symptom is the occurrence of white
patches/lesions on human skin1. It affects 1-2 % of general population, being
more prominent in darker skin and less visible in lighter skin 2. Amongst total
prevalence, 25% cases were affected below the age of 14 years 3. Vitiligo is the
disease of ancient ages as mentioned in Ebes Papyrus (2500BC) 1. Psychiatric
symptoms badly devastate the personality of the effected. Lower self esteem,
lack of confidence, social phobia, anxiety and depressive episodes prevail if the
ailment is not managed properly. These conditions are negative feedback for the
patches to multiply and their spread on different body organs 4. There are
different goals to manage the Vitiligo. Multiple regimens are used therapeutically
such as corticosteroids, u.v light, psoralins etc. Effected lesions especially those
on face can be camouflaged with the use of cosmetics. Surgery is also one of
the used remedy 5.
Present study enlightens the use of natural formulation to fight Vitiligo. Milk

(also called complete diet) is used as solvent and Honey
(the natural syrup) as solute to formulate the solution of
this natural formulation. Honey has been mentioned as the
cure of diseases in revolutionary book of skies i.e. Al Quran.
“Then eat from all the fruits and follow the ways of your
Lord laid down [for you].” There emerges from their bellies
a drink, varying in colors, in which there is healing for
people. Indeed in that is a sign for a people who give
thought” 6.

A 15 years juvenile male patient (AB +Ve blood group)
was affected by generalized Vitiligo since the age of 6
years with no familial history of the ailment. No disease
was diagnosed other than Vitiligo since then. He has been
continuing his studies in class 9th and reported the lack of
real interests in studies and felting the sense of inferiority
due to being stigmatic by Vitiligo particularly on face. His
face was near to be totally de-pigmented before the start of
natural remedy (Fig-1, Pictue-A). Other effected body parts
were forearms, elbows, chest and genitalia etc. There was
history of taking Unani, Homeopathic and allopathic treatment
but none was successful in stopping the progression of
Vitiligo. Ointment containing powder form of Psoralia
corylifolia was started but patches testing failed due to the
development of contact dermatitis at day 2. Considering
the profile of the case, it was recommended to keep on
using the natural formulation that contains the mixture of
cow’s milk and honey. Patient was asked to take two table
spoonful of honey per oral mixed with one cup of boiled
milk of cow early in the morning (once a day). Photographs
of the patient were taken at day 1st. Patient continued using
the natural formulation. He was observed and photographs
taken at interval of 60 days initially and 30 days thereafter.
Dramatic improvement and clear observation of re-
pigmentation on face and elbow in 1st follow up meeting
was very encouraging for the patient and the investigator,
therefore the treatment protocol was adopted for next two
follow up with the interval of 30 days each.
White de-pigmented skin was observed to be partially re-
pigmented after 4 months with the reversal of disease
progression (Fig-1) and side effects were not reported with
this natural formulation as elucidated in hemato-biochemical
and serological laboratory investigations at the completion
of trial (Table-1). Measureable change observed from picture
A to B after completion of 1st two months of therapy while
healing was not observed to be accelerated in the same rate
in preceding pictures (Picture-C and Picture D) of the Fig-
1. This might be either due to the change of source of
honey or left over of volunteer from school resulting negative
daily routine of subject that aggravated anxiety symptoms.

The patient in the current study was juvenile male, therefore
the risk factor was more pronounced in experimental
medicated/herbal trial either oral or topical. However
considering the consisting will of patient and his guardian
after written informed consent, ointment containing powder
form of Psoralia corylifolia was tried and discontinued

immediately due to failure of patch testing. Profile of the
patient let the researcher to think about natural and safer
remedy that based the idea of using the mixture of honey
and milk.
Honey is a natural combination (super saturated solution of
sugar) of vitamins, enzymes, minerals, amino acids, organic
acids 7. Honey contains wide variety of components depending
on the source on which the bees forage. Climatic factors
and vegetation environment can influence on the properties
of Honey. Honey contains proteins, fats, carbohydrates,
polyphenoles and vitamin C etc 8 Honey encompasses highly
complex antibacterial spectrum due to the components of
sugar, low pH, hydrogen per oxide, peptides, there are large
variations in concentrations of these multiple compounds.
For mild to moderate burns, the topical application of honey
potentiates skin growth and healing of wounds 9. Contents
of honey accelerate pigmentation 10. In addition to
carbohydrate, fat, glucose, sucrose and water honey contains
more than 180 substances that depicts its complex nature,
to be used as antibacterial, anti-parasitory, antiviral, anti-
inflammatory, antioxidant, antitumor and anti-mutagenic
effects. Facial Vitiligo has been successfully treated with
the combination of honey, Allium cepa and Avena sativa in
a case study1.
In brief, current formulation is rich in antioxidant, anti-
inflammatory components coupled with abundant nutrients
to nourish the skin into normal position.

White patches of Vitiligo were re-pigmented partially and
disease progression was halted within 4 months with the
natural and safer remedy comprised of honey and milk.
Current formulation is rich in antioxidant, anti-inflammatory
components coupled with abundant nutrients to nourish the
de-pigmented skin into normal. Further studies, involving
the adult cases and large number of patients are suggested
with same formulation to confirm melanogenic effects of
mixture of milk and honey in generalised Vitiligo.

Objectives: To evaluate efficacy and safety of a natural formulation, completed by sun light exposure, in the management of localized vitiligo. Case summary: A 34 –year- old man was affected by facial localized vitiligo with no family history of this disease. After 17 months of treatment with conventional drugs with unsatisfactory results, the patient has adopted a new therapeutic protocol including topical application of a formulation of honey bee, Allium cepa L. juice and Avena sativa L. stem decoction. Also, a daily sun light exposure of 15-20mn was practiced. Results and discussion: A complete re-pigmentation of white patches was obtained in a period of 11 months with no adverse effects. The prolactin value was 7.06 ng/ml and cortisol was 12.36 μg/dl with 17.14 g/l of sericIgG which reflect a stabilization of stress state of the patient. Conclusions: The tested complementary therapy was safe and has successfully re-pigmented the different vitiligo lesions. Further studies are needed to confirm this stimulation of melanocyte’s proliferative activity by the present formulation.

To investigate topical honey in superficial burns and wounds though a systematic review of randomised controlled trials. Cochrane Library, MEDLINE, EMBASE, PubMed, reference lists and databases were used to seek randomised controlled trials. Seven randomised trials involved superficial burns, partial thickness burns, moderate to severe burns that included full thickness injury, and infected postoperative wounds. Studies were randomised trials using honey, published papers, with a comparator. Main outcomes were relative benefit and number-needed-to-treat to prevent an outcome relating to wound healing time or infection rate. One study in infected postoperative wounds compared honey with antiseptics plus systemic antibiotics. The number needed to treat with honey for good wound healing compared with antiseptic was 2.9 (95% confidence interval 1.7 to 9.7). Five studies in patients with partial thickness or superficial burns involved less than 40% of the body surface. Comparators were polyurethane film, amniotic membrane, potato peel and silver sulphadiazine. The number needed to treat for seven days with honey to produce one patient with a healed burn was 2.6 (2.1 to 3.4) compared with any other treatment and 2.7 (2.0 to 4.1) compared with potato and amniotic membrane. For some or all outcomes honey was superior to all these treatments. Time for healing was significantly shorter for honey than all these treatments. The quality of studies was low. Confidence in a conclusion that honey is a useful treatment for superficial wounds or burns is low. There is biological plausibility.

Natural honey is one of the most widely sought products due to its unique properties, which are attributed to the influence of the different groups of substances it contains. Honey is used for nutritional, medicinal and industrial purposes and it is an important commodity in the international market; serving as foreign exchange earner for many countries. In Nigeria, honey production (beekeeping) has the potential to develop as a prime agro-horticultural and forest-based industry which can be a major foreign exchange earner if international standards are met. The precise chemical composition and physical properties of natural honeys differ according to the plant species on which the bees forage. Differences in climatic conditions and vegetations are also important factors that can affect the various properties of honey. North-eastern Nigeria consists of humid, semi arid and arid climates with varying agricultural activities and blossoms from different types of vegetations, which can influence the natural composition and properties of honey. Thus, analysis of the biochemical composition of 18 honey samples obtained from different locations in the northeast sub-region of Nigeria was carried out to ascertain their qualities. Moisture and ash contents of the samples had average values of 16.00 ± 2.19 g/100 g and 0.47 ± 0.09 g/100 g, respectively. The protein contents ranged between 0.35 and 1.08g/100 g with a mean of 0.67 ± 0.25 g/100 g while fat content lied between 0.10 and 0.50 g/100 g with a mean of 0.29 ± 0.11 g/100 g. Total carbohydrate contents and Energy values showed average values of 82.30 ± 2.03 g/100 g and 1,401.33 ± 33.71 KJ/100 g, respectively. Fructose contents gave an average of 38.94 ± 0.90 g/100 g, while glucose contents had a mean value of 31.65 ± 2.79 g/100 g. The sucrose contents of the honey samples had a mean value of 1.84 ± 0.79 g/100 g. Total polyphenol and vitamin C contents showed mean values of 65.31 ± 19.50 mg Gallic Acid Equivalent (GAE)/100 g and 21.15 ± 3.99 mg/100 g, respectively. The results of this study indicate that the samples compare favorably with samples in many parts of the world and also fall within the limits of international standards.

Almost all honey are derived from nectar. They are about 83 degrees Brix and has very low moisture content. A honey's carbodyfrates is mostly monosaccharides with more fructose than glucose. It is also composed of sucrose, other disacchardies and maltose, isomaltose, nigerose, turanose and maltulose. Honey is acidic but is not obvious since the high sugar content tends to mask the acidity in the taste. Average pH is 3.9, has a 50-fold range of values and has trace elements including chromium, lithium, nickel, lead, tin and zinc. There are also enzymes found in honey, including catalase and an acid phosphatase, also a small quantity of protein in finished honey. There are also vitamins found in honey albeit small quantities. Honey features a high refractive index and viscosity while is specific gravity is 1.4 while specific heat is 40% less than that of pure water.

Vitiligo is an acquired depigmentary disorder affecting around 1% of the world's population. In 25% of cases it has its onset prior to the age of 14 years. Articles on vitiligo in children published after 1995 were retrieved from PubMed. The prevalence, etiology, clinical presentation, differential diagnosis, treatment and management of vitiligo in children were summarized. Vitiligo is characterized by acquired, sharply demarcated depigmented macules or patches on the skin, the mucous membranes and/or white hair and it is mainly differentiated from congenital achromic skin lesions. It is frequently associated with various autoimmune diseases. Hashimoto's thyroiditis is the most common association in children. Information on the nature, possible causes and course of the disease leads to acceptance of the disorder and higher compliance with the treatment. The choice of medical treatment depends on the type, location and duration of lesions as well as the eagerness of the child and his/her parents to pursue therapy. The management of childhood vitiligo includes information and reassurance of young patients and their parents on the disease, thyroid investigation, avoidance of trigger factors, topical treatment and proper follow-up.
