En 2005, todos los Estados Miembros de la OMS se comprometieron a luchar por una cobertura universal de salud, una expresión colectiva de la creencia de que todas las personas deben tener acceso a los servicios de salud que necesiten sin riesgo que ello les lleve a la ruina financiera o a un empobrecimiento. Trabajar por una cobertura universal de salud (CUS) es un mecanismo poderoso para lograr una salud y bienestar mejores, así como para promover el desarrollo humano. Esta, la tercera encuesta mundial sobre eSalud llevada a cabo por el Observatorio Mundial de eSalud de la OMS (GOe, por sus siglas en inglés) tiene un enfoque especial: el uso de la eSalud para apoyar la cobertura universal de salud. La eSalud –entendida aquí como el uso coste-efectivo y seguro de las Tecnologías de la Información y Comunicación (TIC) en apoyo de la salud y de los ámbitos relacionados con la salud, incluyendo los servicios de atención sanitaria, vigilancia de la salud, literatura y educación, conocimiento e investigación (2)–, desempeña un papel vital en la promoción de la cobertura universal de salud. Por ejemplo, ayuda a proporcionar servicios a poblaciones remotas y comunidades marginadas a través de la telesalud o la salud por dispositivos móviles (mSalud). Facilita la capacitación del personal sanitario a través del aprendizaje virtual, hace que la educación sea más accesible, especialmente para aquellos que están aislados, y mejora el diagnóstico y tratamiento del paciente al proporcionar información precisa y a tiempo del paciente por medio de los registros electrónicos de salud. A través del uso estratégico de las TIC, mejora las operaciones y la eficiencia financiera de los sistemas sanitarios... RESUMEN El ámbito de las tecnologías móviles aplicadas a la salud (mSalud) es una tendencia en auge a nivel mundial que ha generado enormes expectativas para paliar los proble-mas de prestación de servicios médicos y de salud pública ocasionados por la escasez de recursos y el reducido número de especialistas. Las numerosas posibilidades que aportan las tecnologías móviles, junto con su facilidad de uso, han atraído el interés tanto de los gobiernos políticos como el de las universidades. Es el caso de la Red Iberoamericana de Tecnologías Móviles y Salud (Red CYTED-RITMOS). Como resultado del primer año de actividad de la Red, en octubre de 2015 tuvo lugar en Barcelona el Internacional Workshop RITMOS con el objeto de presentar las áreas prioritarias de América Latina donde podrían desarrollarse proyectos de investi-gación, desarrollo e innovación (I+D+i) en salud móvil y posibles soluciones. El objetivo de este trabajo es mostrar las potencialidades y la aplicabilidad de la mSalud en la región de las Américas.

Objectives: While most studies have focused on analyzing the results of telemedicine use, it is crucial to consider the determinants of its use to fully understand the issue. This article aims to provide evidence on the determinants of telemedicine use in clinical practice. Methods: The survey targeted a total population of 398 medical professionals from a healthcare institution in Spain. The study sample was formed by the ninety-three primary care physicians who responded. Using an extended Technology Acceptance Model and microdata for the ninety-six physicians, binary logistic regression analysis was carried out. Results: The analysis performed confirmed the model’s goodness-of-fit, which allowed 48.1 percent of the dependent variable’s variance to be explained. The outcomes revealed that the physicians at the healthcare institution placed greater importance on telemedicine’s potential to reduce costs, and on its usefulness to the medical profession. The perception of medical information security and confidentiality and the patients’ predisposition toward telemedicine were the second explanatory factors in order of importance. A third set of moderating effects would appear to corroborate the importance of the physicians’ own opinions. Conclusions: These results have revealed the need for a dynamic approach to the design of telemedicine use, especially when it targets a variety of end-users. Hence, the importance of conducting studies before using telemedicine, and attempting to identify which of the above-mentioned predictors exert an influence and how.

Background: More advanced methods and models are needed to evaluate the participation of patients and citizens in the shared health care model that eHealth proposes. Objective: The goal of our study was to design and evaluate a predictive multidimensional model of eHealth usage. Methods: We used 2011 survey data from a sample of 13,000 European citizens aged 16-74 years who had used the Internet in the previous 3 months. We proposed and tested an eHealth usage composite indicator through 2-stage structural equation modelling with latent variables and measurement errors. Logistic regression (odds ratios, ORs) to model the predictors of eHealth usage was calculated using health status and sociodemographic independent variables. Results: The dimensions with more explanatory power of eHealth usage were health Internet attitudes, information health Internet usage, empowerment of health Internet users, and the usefulness of health Internet usage. Some 52.39% (6811/13,000) of European Internet users' eHealth usage was more intensive (greater than the mean). Users with long-term health problems or illnesses (OR 1.20, 95% CI 1.12-1.29) or receiving long-term treatment (OR 1.11, 95% CI 1.03-1.20), having family members with long-term health problems or illnesses (OR 1.44, 95% CI 1.34-1.55), or undertaking care activities for other people (OR 1.58, 95% CI 1.40-1.77) had a high propensity toward intensive eHealth usage. Sociodemographic predictors showed that Internet users who were female (OR 1.23, 95% CI 1.14-1.31), aged 25-54 years (OR 1.12, 95% CI 1.05-1.21), living in larger households (3 members: OR 1.25, 95% CI 1.15-1.36; 5 members: OR 1.13, 95% CI 0.97-1.28; ≥6 members: OR 1.31, 95% CI 1.10-1.57), had more children <16 years of age (1 child: OR 1.29, 95% CI 1.18-1.14; 2 children: OR 1.05, 95% CI 0.94-1.17; 4 children: OR 1.35, 95% CI 0.88-2.08), and had more family members >65 years of age (1 member: OR 1.33, 95% CI 1.18-1.50; ≥4 members: OR 1.82, 95% CI 0.54-6.03) had a greater propensity toward intensive eHealth usage. Likewise, users residing in densely populated areas, such as cities and large towns (OR 1.17, 95% CI 1.09-1.25), also had a greater propensity toward intensive eHealth usage. Educational levels presented an inverted U shape in relation to intensive eHealth usage, with greater propensities among those with a secondary education (OR 1.08, 95% CI 1.01-1.16). Finally, occupational categories and net monthly income data suggest a higher propensity among the employed or self-employed (OR 1.07, 95% CI 0.99-1.15) and among the minimum wage stratum, earning ≤€1000 per month (OR 1.66, 95% CI 1.48-1.87). Conclusions: We provide new evidence of inequalities that explain intensive eHealth usage. The results highlight the need to develop more specific eHealth practices to address different realities.

This document aims to provide solutions for the successful implementation of telemedicine services (and eHealth, in general) in the health care setting. Following a literature review that identifies key theories, models, and frameworks used in the science of implementation, we introduce a theoretical framework that addresses the obstacles to incorporating and standardizing the use of telemedicine in health care institutions. This narrative review gathers qualitative information from different sources on a particular topic, and summarizes and synthesizes the literature in order to determine “what is known” about a topic (3). This theoretical framework is based on understanding the context within its broader structure, together with existing challenges and opportunities. The document introduces the reader to the reality of implementing telemedicine services in actual health care settings; analyzes interactions that occur during the process of ICT implementation and changes that occur in organizations, management models, culture, and medical care services; and, finally, reflects on key aspects related to prioritization, design, deployment, integration, and assessment. Analysis of this theoretical framework should allow for the creation of a research program that focuses on major aspects in the successful development of telemedicine, and generate proposed actions to overcome difficulties. Bearing in mind its cross-cutting nature, this report is aimed at the community of professionals (in its broadest sense) willing to work in any field related to the advancement of the health care sector, both in health and social settings, with the implementation and intensive use of ICTs.

Background: Data routinely collected in electronic health records (EHRs) offer a unique opportunity to monitor chronic health conditions in real-time. Geographic information systems (GIS) may be an important complement in the analysis of those data. Objective: The aim of this study was to explore the feasibility of using primary care EHRs and GIS for population care management and public health surveillance of chronic conditions, in Portugal. Specifically, type 2 diabetes was chosen as a case study, and we aimed to map its prevalence and the presence of comorbidities, as well as to identify possible populations at risk for cardiovascular complications. Methods: Cross-sectional study using individual-level data from 514 primary care centers, collected from three different types of EHRs. Data were obtained on adult patients with type 2 diabetes (identified by the International Classification of Primary Care [ICPC-2] code, T90, in the problems list). GISs were used for mapping the prevalence of diabetes and comorbidities (hypertension, dyslipidemia, and obesity) by parish, in the region of Lisbon and Tagus Valley. Descriptive statistics and multivariate logistic regression were used for data analysis. Results: We identified 205,068 individuals with the diagnosis of type 2 diabetes, corresponding to a prevalence of 5.6% (205,068/3,659,868) in the study population. The mean age of these patients was 67.5 years, and hypertension was present in 71% (144,938/205,068) of all individuals. There was considerable variation in diagnosed comorbidities across parishes. Diabetes patients with concomitant hypertension or dyslipidemia showed higher odds of having been diagnosed with cardiovascular complications, when adjusting for age and gender (hypertension odds ratio [OR] 2.16, confidence interval [CI] 2.10-2.22; dyslipidemia OR 1.57, CI 1.54-1.60). Conclusions: Individual-level data from EHRs may play an important role in chronic disease surveillance, namely through the use of GIS. Promoting the quality and comprehensiveness of data, namely through patient involvement in their medical records, is crucial to enhance the feasibility and usefulness of this approach.
