LDL cholesterol estimation in patients with the metabolic syndrome

The Friedewald formula (LDL-F) for the estimation of low-density lipoprotein (LDL) cholesterol concentrations is the most often used formula in clinical trials and clinical practice. However, much concern has been raised as to whether this formula is applicable in all patient populations such as the presence of chylomicronaemia and/or hypertriglyceridaemia. [Show full abstract] evaluate various LDL cholesterol calculation formulas as well as LDL cholesterol levels provided by the Lipoprint LDL System (LDL-L) in patients with the metabolic syndrome (MetSyn). LDL-F showed significant differences from other formulas in the total cohort, as well as in MetSyn individuals. This was not the case in nonMetSyn subjects, where LDL-F did not differ with other formulas, with the exception of one formula (LDL by Planella, LDL-P). The bias between LDL-F and other LDL estimation formulas were significantly higher in MetSyn subjects compared to nonMetSyn individuals, except for LDL-L which produced similar bias with LDL-F in both study groups. LDL-F seems to exhibit some limitations as far as the calculation of LDL-C levels in patients with the MetSyn is concerned. LDL-L might be more accurate in MetSyn subjects, but so far its use is limited for the estimation of small, dense LDL (sdLDL) cholesterol levels and mean LDL particle size for research purposes only.

To evaluate the effects of gemfibrozil and pravastatin in coronary artery disease patients with HDL-cholesterol (HDL-C) < 35 mg/dl). Twenty-nine patients (20 males, 60 +/- 9) were divided in a gemfibrozil group (G) (1200 mg/day n = 15) and a pravastatin group (P) (10 or 20 mg n = 10 and 4, respectively). HDL-C was not affected in P, TC and LDL-cholesterol (LDL-C) reductions were superior to those in G (31.3% vs 13.4% and 38.7 and 11.5%, p < 0.05 and < 0.01 respectively). In G HDL-C raised by 50% (12th week p < 0.01). Gemfibrozil reduced TG levels in 44.7% while in P it varied -32.2% (12th week p < 0.01 and < 0.05 respectively). Gemfibrozil is more effective in reducing TG and raising HDL-C than pravastatin. On the other hand, pravastatin was more potent in reducing LDL-C levels.

The effect of adopting an uncooked extreme vegan diet lacking in cholesterol and readopting conventional diet on serum lipids and apolipoprotein was studied. Total serum cholesterol, triglyceride, LDL, HDL cholesterol and apolipoprotein AI as well as B decreased significantly within two weeks of beginning the new diet. The alleviated levels remained throughout the period of consuming vegan diet. The lipid and apolipoprotein concentrations returned to normal level within one month of resuming the conventional diet. The HDL-cholesterol: LDL-cholesterol ratio was elevated in the test group after one month of consuming the new diet. Serum apolipoprotein AI level was less decreased by the vegan diet compared to that of apolipoprotein B. In the control group, no changes were observed during the study. The results suggest that the uncooked extreme vegan diet causes a significant decrease of the atherosclerosis risk factor.

Comparison of simvastatin and ciprofibrate in the treatment of hypercholesterolemia—a French multice...

The efficacy and safety of simvastatin, 10–40 mg/day, were compared with ciprofibrate, 100 mg/day, in patients with primary hypercholesterolaemia. The study consisted of a 4-week placebo period followed by an 18-week double-blind treatment period (initial total plasma cholesterol levels ≥ 6.45 mmol/l (250 mg/dl)). In patients whose total cholesterol was greater than 5.2 mmol/1 (200 mg/dl) at the end of 6 weeks, the dose of simvastatin was increased to 20 mg for another 6-week period. After 12 weeks' treatment, the dose was further increased to 40 mg/day in patients whose total cholesterol remained above 5.2 mmol/l. By week 18, simvastatin had reduced total cholesterol, low density lipoprotein (LDL)-cholesterol, apolipoprotein (apo) B and triglycerides by 29.5%, 40.7%, 26.8% and 11.4%, respectively. With ciprofibrate, the corresponding reductions were 15.5%, 19.8%, 16.9% and 33.9%, respectively. The mean percentage increases in high density lipoprotein (HDL)-cholesterol were significant and similar in each treatment group after 18 weeks (8.2% with simvastatin, 9.7% with ciprofibrate). However, the mean percentage reductions from baseline in total cholesterol and LDL-cholesterol were significantly larger in the simvastatin group than in the ciprofibrate group at weeks 6, 12 and 18 (P ⩽ 0.01). By contrast, the reduction in total triglycerides was smaller with simvastatin than with ciprofibrate. Overall, the frequency of adverse events was similar in both treatment groups. Moreover, the effects of simvastatin on total and LDL-cholesterol are greater than those of ciprofibrate, regardless of the levels of total cholesterol and triglycerides prior to treatment.

[Show full abstract] lesions.Methods: This study was a single center retrospective study involving patients below 40 years who underwent diagnostic coronary angiography. Demographic variables, lipid profile and FR index were estimated. Patients were divided into four groups based on the FR index scores of 0, 1–4, 5–10 and 11-15, respectively.Results: A total of 158 patients (Mean ± SD of age; 35.65 ± 3.81 years) were included in the study. Among demographic variables, gender (P = 0.03) and body mass index (BMI) (P < 0.001) were found to be associated with FR index. In addition, total cholesterol (P < 0.001), low density cholesterol (LDL) cholesterol (P < 0.001), non-high density cholesterol (non-HDL) (P < 0.001) and ratio of triglycerides (TG) /non-HDL cholesterol (P = 0.004) showed significant differences between the FR groups. Logistic regression analysis showed that only diabetes (P = 0.02) and BMI (P = 0.004) were significant predictors of the extent and severity of coronary artery lesions in terms of FR index.Conclusions: A strong direct relationship was observed between total cholesterol, LDL and non HDL cholesterol while a negative correlation with the TG/non HDL ratio. Diabetes and BMI also play a very significant role.

Main results: Compared to patients without CAD (n = 99), patients with CAD (n = 235) had significantly higher homocysteine concentrations (10.4 ± 3.2 vs. 9.9 ± 4.2 μmol/1, p < 0.05). Furthermore, patients with CAD also had higher levels of LDL cholesterol (178 ± 46 vs. 165 ± 39 mg/dl, p < 0.05), lower levels of HDL cholesterol (44 ±12 vs. 48 ± 15 mg/dl, p < 0.05) and higher levels of triglycerides (200 ± 137 vs. 170 ± 120 mg/dl, p < 0.05). A nearly significant trend between homocysteine concentrations and the extent of angiographically visible atherosclerosis was noted. Conclusion: Patients with angiographically visible CAD have higher levels of homocysteine than patients without CAD. A correlation seems to exist between the extent of CAD and homocysteine levels.
