‘Uniqueness of Siddha Medicine in treating diseases of the Ear, Nose and Throat’

Abstract

In our body, head is an important organ. Ear, nose and throat are part of five senses. In Siddha system, Ear, Nose and Throat diseases comes under head diseases (Thalai noikal). Patients of almost age group and both genders may have diseases of ENT in any time of their life. Ear infections are generally seen in children under the age of six. Throat diseases are more prone to the age group of sixty years. The incidence ratio will be more susceptible for Men than women. The main purpose of the review is to document the uniqueness and up to date review of ear, nose and throat in accordance with Siddha Medicine. The WHO identifies that ENT problem as a massive public health problem. This review also deals with the emerging branch ear, nose and throat diseases & its types, causes, symptoms, treatments which are represented in Siddha.

INTRODUCTION
The awareness regarding Traditional medicine is gradually raising and people are
getting more benefits from their health troubles with natural treatments. In a scientific setting,
as like western medicine the current traditional systems of medicine is also developing with
departmental specialities like Siddha Obstetrics & Gynaecology, Siddha Varmam &
Thokkanam, Siddha Dermatology etc. Our fore fathers came across with many diseases of
ENT and documented quiet a lot of formulations from both herbal and herbo-mineral origins.
ENT diseases come under the special entity Sevi Noigal, Mooku noigal, Kaluthu noigal,
Naaku noigal’ in Siddha. Apart from the conventional internal medicines, special external
therapies like fomentation, oil bath and cauterization are also advised.

According to Sarebendrar’s Classical text, the ear diseases and their symptoms are listed
out in the table.

Vadha sevinoi Migraine, pus discharge, ant moving sensation in ear
Pitha sevinoi Pain & burning sensation, yellowish discharge
Kapha sevinoi White and sticky pus discharge, feeling heaviness in jaw and
head region
Ratha sevinoi pricking pain
Sanni sevinoi intolerable pain, black or White or red colour discharge
Sevidu unable to hear any sound
Kadhaipu itching, pain, blockage
Cheel kadhu pain or painless pus discharge
Kathu katti yellow or red or blood, watery discharge
Ornamental ulcer pain, burning

Controlling fourteen forces (14 vegangal) like vomiting, tear etc

CONCLUSION
While going through the Siddha literatures, one can find the marvellous in-depth
knowledge and wisdom of the ancient Siddhars in understanding and documenting the
disease classifications and their management. In Siddha system, apart from the conventional
medicines, diseases of the ear are specially treated by oil bath. Nose diseases are effectively
treated with Nasiyam and Pugai. Throat diseases are treated by both internal and external
medicines. Siddha ENT is emerging as a vital field and much more research are to be carried
out to scientifically document more medicines for the benefit of the Man-kind.

Microbiomes are the microbiological environment of genetic material that resides within our body. They are beneficial to human beings in many ways which include host metabolism, nutrition and immune functions. The microbiomes help the human immune system in differentiating the beneficial and harmful microorganisms , but in case of antibiotic drug administration, inorder to safeguard themselves they exhibit resistance against those specific antibiotics. They also transmit the drug resistance to the human gene. To overcome this drug resistance Siddhars have found unique remedies namely Viresanam, Vamanam and Nasiyam and anjanam based on the basic principles of Siddha system of medicine. In this paper, we have briefly discussed the mechanism by which these three methods help in overcoming the ill effects caused by microbiomes.

Lasuna Thaabitham or tonsillitis (tonsil+itis) meaning inflammation of the tonsil, caused either by viral or bacterial infection is characterised by sore throat and fever. The incidence is high in small children and the generalised treatment includes either anti inflammatory or anti-biotics. Chronic Tonsillitis is corrected by tonsillectomy (surgical removal of the tonsils). This study intends to suggest some Siddha medicines for the cure of tonsillitis. Siddha System of Medicine has internal, external and local applications for the treatment of tonsillitis. Single herbs such as Akarakaram (Anacyclus pyrethrum), Athimathuram (Sweet flag), Arathai (Alpinia galangal), Aathondai (Capparis zeylanica), Chukku (Zingiber officinale), Milagu (Piper nigrum), Val milagu (Piper cubeba), Adathodai (Justicia beddomei) are used for the cure of tonsillitis. Chukku Amukkara Pattru (a poultice having dried ginger and winter cherry) is indicated for local, topical application. The compound medicines indicated for the treatment of tonsillitis include Lasuna Thaabitha Chooranam, Kadugadhi kirudham, Kandavizhtham, Thuthuvalayadhi urundai, Vengara madhu and Poondu then. In addition, the literary review including aetiology, signs and symptoms, patho-physiology and dietary management of the condition, ‘Lasuna thabitham’ as per Siddha texts are also discussed.

Objective: To evaluate the antiasthmatic activity of leaves of Vitex negundo. Materials and Methods: Ethanolic extract (AE) and various fractions like petroleum ether (PF), aqueous (AF) and ethyl acetate (EAF) of leaves of Vitex negundo were prepared. The antiasthmatic activity of AE, PF, AF and EAF was evaluated by various experimental models like mast cell degranulation by compound 48/80, passive cutaneous anaphylaxis, and egg-albumin induced asthma. Dexamethasone (5mg/kg) was used as a reference standard. Results: AE, EAF, and AF showed significant protection of rat mesenteric mast cells from disruption caused by compound 48/80. Animals treated with AE, EAF, and AF showed significantly lesser the amount of dye leakage as compared to control animals at the site of heterologous antibody injection. Animals treated with AE, EAF, and AF showed significantly lesser the amount of eosinophils, serum bicarbonate level and lung body weight ratio and significantly higher tidal volume as compared to untreated, egg-albumin sensitized animals. Conclusion: Present study concluded that AE, EAF, and AF of leaves of Vitex negundo are found to be effective in various experimental models of asthma. Stabilization of mast cells, inhibitory effects on immediate hypersensitivity reactions and antieosinophilic activity appear to be involved in its mode of action.

In siddha system of medicine references are found in literature regarding two routes of drug administration namely Agamarunthu (Internal medicines) and Puramarunthu (External medicines). They are thirty-two each in number. Allergic rhinitis due to inflammation of bronchial mucous membrane is the major cause for the development of bronchial asthma. Administration of Nasiyam reduces the inflammation in the condition of allergic rhinitis in bronchial asthma. This paper concludes the efficacy of Nasiyam, method of application, its probable mechanism of action and also some siddha formulations of Nasiyam in the treatment of allergic rhinitis in Bronchial asthma.
